細菌性感染症の予防に抗菌薬が広く用いられる中、薬剤耐性(AMR)は世界的な公衆衛生上の問題となっています1。AMRとの闘いは、疫学的病原体のAMRのモニタリングと、感受性抗菌剤と溶解性バクテリオファージの相乗療法の現在の主要な使命です2。
in vitro 抗菌薬感受性試験(AST)は、治療のモニタリングとAMRレベルの検出の主力です。抗菌薬理学の重要な部分であり、臨床薬学の重要な基盤です。米国の臨床検査基準協会(CLSI)と欧州抗菌剤感受性試験委員会(EUCAST)は、ASTの国際基準を策定および改訂し、ASTメソッドとブレークポイントを継続的に修正および補足して、特定の「生物-抗菌剤」の組み合わせのMICを感受性(S)、耐性(R)、または中間体(I)として決定しました3。4.
1980年代から1990年代にかけて、Alfred 60AST、VITEK System、PHOENIXTM、Cobasbact 5,6,7などの例で、自動マイクロブロス希釈装置が急速に開発され、臨床診療に適用されました。しかし、これらの機器は高価で、高コストの消耗品が必要であり、その検出範囲は臨床患者の投薬用に設計されていました5,6,7。これらの理由から、獣医の臨床検査や大量の高耐性株の検出には適していません。本研究では、96ドットマトリックス接種装置(図1)、画像取得コンバーター(図2)、および対応するソフトウェア8を含むインテリジェントなハイスループットAST/ファージスクリーニングシステムを開発し、寒天希釈法により複数の抗菌剤に対して一度に細菌株のバッチのASTを実施しました。さらに、このシステムは、薬剤耐性菌に対するファージの溶解パターンの検出と分析にも使用され9、ファージライブラリーから溶解性ファージを効率的に選択しました。このシステムは、効率的で、手頃な価格で、操作が簡単であることがわかりました。

耐性菌に対する抗菌剤感受性試験(AST)およびファージハイスループットスクリーニングの効率を向上させ、検出コストを削減するために、AST基準および臨床検査基準協会(CLSI)によって策定された耐性ブレークポイント(R)に従って、96ドットマトリックス接種器、画像取得コンバーター、および対応するソフトウェアを含むインテリジェントなハイスループットAST/ファージスクリーニングシステムが開発されました。中国山東省の家禽から分離された1,500株の サルモネラ 菌株のASTおよび最小発育阻害濃度(MIC)分布(R / 8から8R)の統計を、10の抗菌剤に対してインテリジェントなハイスループットAST/ファージスクリーニングシステムによって実施しました。Lar指数は、「抗生物質が少なく、耐性が少なく、抗生物質がほとんどなくなるまで残存する」という意味で、各MICの加重平均を計算し、Rで割ることによって得られました。このアプローチは、耐性菌株の抗菌薬耐性(AMR)の程度を特徴付けるために耐性の有病率を使用する場合と比較して、精度が向上します。AMRの高い サルモネラ 菌株については、このシステムによりファージライブラリーから溶解性ファージを効率的にスクリーニングし、溶解スペクトルを計算および分析しました。その結果、インテリジェントなハイスループットAST/ファージスクリーニングシステムは、操作性、精度、高効率、安価、メンテナンス性が高いことが示されました。山東省の獣医用抗菌薬耐性モニタリングシステムと組み合わせることで、AMRに関連する科学研究や臨床検出に適していました。
