整形外科手術で利用された最初のロボットアプリケーションは、1992年に採用されたROBODOCシステムでした1。それ以来、ロボット支援手術システムは急速に発展してきました。ロボット支援手術は、手足のアライメントと関節の生理学的運動学を回復する外科医の能力を高めることにより、関節形成術を改善します2。脊椎手術では、ロボットを使用した椎弓根スクリューの配置は安全で正確です。また、外科医の放射線被曝も減少します3。しかし、ロボット支援手術に関する研究は、外傷性整形外科疾患の不均一性のために限られていました。整形外科外傷に対するロボット手術に関する既存の研究は、主にロボット支援仙腸関節スクリューと骨盤リング骨折の恥骨スクリュー固定4、大腿骨頸部5のカニューレスクリュー固定、髄内釘6,7のエントリーポイントと遠位ロックボルト、経皮的骨折の減少8,9、および軍事分野における重傷患者の治療に焦点を当てています10。
経皮スクリュー技術は、2Dおよび3Dナビゲーションサポートを使用して実行できます。仙腸、前柱、後柱、寛骨臼上、および魔法のネジは、骨盤および寛骨臼のファクチャーの最も一般的な経皮的技術です11。経皮的経腸骨-経仙骨スクリュー技術は、外科医にとって依然として困難です。この手順には、骨盤の解剖学的構造とX線透視法の理解、正確な位置決め、および長期的な手の安定性が必要です。遠隔操作ロボットシステムは、これらの要件を十分に満たすことができます。この研究では、遠隔操作ロボットシステムを利用して、骨盤輪骨折に対する経皮的経腸骨-経仙骨スクリュー固定を完了します。このプロトコルの詳細とワークフローを以下に示します。
ロボットシステム マスタースレーブ整形外科位置決めおよびガイダンスシステム(MSOPGS)は、主に7自由度(DOF)の手術ロボット(スレーブマニピュレータ)、フォースフィードバックを備えたマスターマニピュレータ、およびコンソールの3つの部分で構成されています。このシステムには、手動牽引、マスター/スレーブ操作、リモートモーションセンター(ROM)、および緊急時の4つの動作モードがあります。 図 1 に MSOPPG を示します。その主な構成要素を以下に簡単に説明します。

経腸骨-経仙骨スクリュー固定は、臨床診療では困難な作業です。遠隔操作ロボットシステム支援手術は、経腸骨-経仙骨スクリュー配置の難しさに対する解決策を提供します。この技術を使用すると、外科医は手術室の外で手順を実行できるため、放射線被曝の危険を回避できます。また、ロボット支援手術は、従来法に比べて位置維持能力が高くなっています。このテクノロジーは、3Dナビゲーション、バーチャルリアリティ、拡張現実、および複合現実テクノロジーと組み合わせて、骨盤寛骨臼に経皮的なスクリューを配置することができます。効率的な固定のために、医師はこの技術を使用する前に、透視下でフリーハンドを使用して仙腸骨ネジを配置することに熟練している必要があります。まず、大腿骨に2本のシャンツピンを挿入することにより、透視プレートベースを使用して死体の骨盤を仰臥位に固定します。仰臥位では、後上腸骨棘の両方を同時に板の上、腰椎を床に平行に置きます。術前計画ソフトウェアのMed CADモジュールを使用して、骨盤の画像の円柱を作成し、直径と長さを入力して円柱のサイズを定義します。円柱をS1またはS2椎体に置き、軸および冠状画像で円柱の正中線の向きを調整します。各画像で円柱のエッジと皮質骨の関係を確認してください。透視手術台の仰臥位に骨盤を固定します。次に、ロボットを手術台に対して45度で同側に配置し、Cアームを反対側の手術台に垂直にします。次に、マスターマニピュレータのワークステーションを手術室の外に置きます。グリッドポジションメーカーを同側を粘着テープで固定します。仙骨の真の側面図上のグリッド位置マーカーでターゲット領域を選択します。コンソールの手動牽引モードが選択されて開始されていることを確認し、ロボットアームをS1またはS2経腸骨-経仙骨スクリューエントリポイントの一般的な領域にドラッグします。仙骨の真の側面図を視覚化し、マスターマニピュレータをマスタースレーブ動作モードで操作し、遠位スリーブの先端をガイドワイヤ入力領域に配置するように調整します。リモートモーションセンター(RCM)モードを選択した後、側面仙骨ビューのCアーム透視を続行します。ガイドワイヤスリーブの中心を同心円に調整して、ネジチャネルと一致します。ロボットアームをロックし、電気ドリルを使用して反対側の回腸に2.5ミリメートルのガイドワイヤーを挿入します。次に、手動牽引モードでロボットを取り外します。Cアームを入口と出口の角度に回して、ガイドワイヤーが突破したか、内部および後仙骨皮質と仙骨神経管に接触したことを確認します。ガイドワイヤーに沿って対側腸骨皮質に7.3ミリメートルの半ネジを挿入します。骨盤の入口、出口、および側面図のネジの位置を評価します。取得したCTスキャンデータをDICOM形式で術前計画ソフトウェアにインポートし、骨盤の冠状、矢状、および軸方向の画像でネジの位置を取得します。術後のCT再構成画像とスクリュー配置を評価するためのX線写真は、皮質骨にネジが貫通しておらず、すべてのネジが完全に海綿骨にあることを示しました。正中線部位の矢状CT再構成像は,再スライス軸部像に見られるように,スクリューがS1に位置し仙骨管に入らなかったことを示唆した。また、再スライスコロナCT再構成画像に見られるように、スクリューは安全です。仙骨の真の側面図は、スクリューが完全に骨内にあり、入口と出口の画像で仙骨前部と後部の仙骨皮質および仙骨神経管から安全な距離にあることを明らかにしました。この手順のリモートコントロールが可能です。このプロトコルの重要な点は、マスター/スレーブ動作モードとRCMモードの精度と安定性です。この方法は、今後、遠隔医療に欠かせないものとなることが期待されています。
