정형 외과 수술에 활용 된 최초의 로봇 응용 프로그램은 1992 년에 사용 된 ROBODOC 시스템이었습니다1. 그 이후로 로봇 보조 수술 시스템이 빠르게 발전했습니다. 로봇 보조 수술은 사지의 정렬과 관절의 생리적 운동학을 복원하는 외과의의 능력을 향상시켜 관절 성형술을 개선합니다2. 척추 수술에서 로봇을 사용한 척추경 나사의 배치는 안전하고 정확합니다. 또한 외과 의사의 방사선 노출을 줄입니다3. 그러나 로봇 보조 수술에 대한 연구는 외상성 정형 외과 질환의 이질성으로 인해 제한적이었습니다. 기존의 정형외과 외상 로봇 수술에 대한 연구는 주로 골반고리 골절4의 로봇 보조 천장관절 나사 및 치골 나사 고정, 대퇴골 경부의 캐뉼러 나사 고정5, 골수 내 못 박기의 진입점 및 원위 잠금 볼트6,7, 경피적 골절 감소 8,9 및 군사 분야에서의 중상 환자 치료10에 중점을 둡니다.
경피적 스크류 기술은 2D 및 3D 탐색 지원을 사용하여 수행할 수 있습니다. 천장골, 전방 기둥, 후방 기둥, 비구 상부 및 마술 나사는 골반 및 비구 사실에 대한 가장 일반적인 경피적 기술입니다11. 경피적 경골-경천골 스크류 기술은 외과의에게 여전히 어려운 과제입니다. 이 절차에는 골반 해부학 및 X선 형광투시법, 정확한 위치 지정 및 장기적인 손 안정성에 대한 이해가 필요합니다. 원격 작동 로봇 시스템은 이러한 요구 사항을 잘 충족할 수 있습니다. 이 연구는 원격 작동 로봇 시스템을 사용하여 골반 링 골절에 대한 경피적 경골-경천골 나사 고정을 완료합니다. 이 프로토콜의 세부 정보와 워크플로는 아래에 나와 있습니다.
로봇 시스템 마스터-슬레이브 정형외과 포지셔닝 및 안내 시스템(MSOPGS)은 주로 7자유도(DOF)의 수술 로봇(슬레이브 매니퓰레이터), 포스 피드백이 있는 마스터 매니퓰레이터 및 콘솔의 세 부분으로 구성됩니다. 이 시스템에는 수동 견인, 마스터-슬레이브 작동, 원격 동작 중심(ROM) 및 비상 사태의 네 가지 작동 모드가 있습니다. 그림 1 은 MSOPPGS를 보여줍니다. 주요 구성 요소는 아래에 간략하게 설명되어 있습니다.

RCM 모드를 사용하면 수술 기구가 기구 끝을 중심으로 회전할 수 있습니다.
