이완불능증(Achalasia)은 비정상적인 연동운동과하부 식도 괄약근의 이완 장애를 특징으로 하는 원발성 신경퇴행성 식도 운동 장애이다 1. 이완불능증의 치료는 하부 식도 괄약근의 휴지 압력을 감소시켜 식도 배출을 가능하게 하는 것을 목표로 한다2. 이완불능증 치료에는 경구용 약물치료요법, 내시경적 약물치료요법3, 공압팽창술4, 경구경내시경 근절개술(POEM)5, 외과적 근절개술6 등 여러 가지 방법이 있다.
하부 식도 괄약근의 근육 섬유가 분할되는 외과적 근절개술은 공압 팽창 및 경구 내시경 근절개술과 함께 진행성 이완불능증에 대한 세 가지 결정적인 치료법 중 하나로 설명되었습니다 7,8. 근절개술은 하부 식도 괄약근의 압력을 감소시켜 잠재적인 위식도 역류 질환을 유발할 수 있기 때문에 위저부주름술의 추가는 역류 방지 절차로 수행됩니다 9,10.
복강경 헬러 근절개술은 개흉술, 개복술 및 흉강경11,12과 같은 다른 외과적 접근법에 비해 수술 후 통증이 감소하고 이환율이 감소하여 이완불능증을 치료하는 가장 일반적인 수술 절차가 되었습니다. 로봇 헬러 근절개술은 확대된 고해상도 3차원 시각화 및 최소화된 생리적 떨림과 같은 로봇 접근법이 제공하는 기계적 이점 때문에 이완불능증 치료를 위한 복강경 검사의 최소 침습적 대안으로 부상했습니다13,14,15.
이 기사는 만성 삼킴곤란, 역류 및 체중 감소가 있는 32세 환자의 사례를 제시합니다. 연하 곤란은 처음에는 고체와 관련이 있었고 천천히 액체로 진행되었습니다. 환자는 발열증, 상복부 통증 및 식후 포만감과 같은 다른 임상 증상을 부인했습니다. 악성 종양을 배제하기 위해 처음에 내시경 평가가 수행되었습니다(그림 1). 검사 결과 식도의 팽창과 비틀림, 내시경으로 완전히 흡인 된 음식물 보유가 밝혀졌습니다. 점막의 두꺼워짐도 확인되었으며 종양성 병변은 발견되지 않았습니다. 협대역 영상은 정상적인 혈관 및 점막 패턴을 보였다. 위식도 접합부는 횡격막 crus의 수준에 위치했다.
