2010년 9월부터 2021년 12월까지 LCBDE를 받은 담도 수술 이력이 있는 총 177명의 환자가 포함되었습니다. 이 연구는 지난 대학교 제1부속병원의 기관 검토 위원회의 승인을 받았습니다. 모든 환자는 서면 동의서를 제공했습니다.

담즙결석증은 가장 흔한 담도 질환 중 하나로, 재발률이 높다1. 재발성 담즙결석은 종종 여러 개의 결석을 수반하고 내시경적 역행성 담췌관조영술(ERCP)/내시경 괄약근 절개술(EST)이 반복적인 역행성 담도 감염을 유발할 뿐만 아니라 오디 괄약근의 기능을 손상시킬 수 있다는 사실과 함께 재발성 담즙결석 환자는 종종 두 번 이상의 수술이 필요하다2.
최소 침습 수술의 대중화와 복강경 기술의 발전으로 복강경 총담관 탐사술(LCBDE)은 외상 최소화, 빠른 회복 및 오디 괄약근 기능 보존과 같은 장점 덕분에 임상에서 널리 사용되었습니다3. 재발성 담즙결석 환자의 복부 유착은 해부학적 구조의 변화로 이어지므로 이러한 환자는 후속 담관 탐사에서 2차 손상에 취약합니다. 따라서 복부 유착은 복강경 수술의 금기 사항으로 간주되어 왔다 4,5. 추가 기술 개발로 LCBDE는 담도 수술병력이 있는 환자에서 실현 가능한 것으로 예비 확인되었습니다 6,7,8.
그러나 관련 연구는 제한적이며 이 수술 기법에 대한 보다 심층적인 연구가 여전히 필요합니다. LCBDE에 대한 프로그래밍된 수술은 특히 광범위한 복부 유착이 있는 환자의 경우 종종 부족합니다. 이러한 상황을 바탕으로 본 연구는 담도 수술 이력이 있는 환자에서 LCBDE의 외과적 접근과 해부학적 특징을 조사하여 프로그래밍된 절차를 개발하는 것을 목표로 합니다. 외과적 접근법과 해부학적 랜드마크를 기반으로 프로그래밍된 수술은 수술 시간을 단축하고 합병증을 줄이며 수술 안전성을 향상시키는 데 도움이 될 수 있다 9,10.

재발성 담즙결석증의 경우 이전 수술의 복부 유착으로 인해 해부학적 구조가 변경되고 한때 상대적 금기 사항으로 간주되었던 복강경 총담관 탐사(LCBDE)를 위한 또 다른 수술을 수행할 때 2차 손상이 쉽게 발생합니다. 현재 수술 기법의 한계를 고려하여 본 연구에서는 LCBDE의 재수술을 위한 외과적 접근법과 중요한 해부학적 랜드마크를 요약하였다. 총담관을 노출시키기 위해 인대 테레스 간장 접근법, 전방 간 십이지장 인대 접근법, 우측 간 십이지장 인대 접근법 및 하이브리드 접근법을 포함하여 4가지 일반적인 외과적 접근법이 제안되었습니다. 또한, 이 연구는 정수리 복막, 위장관 장막, 인대 테레스 간장, 간의 하변연, 위 전두부, 십이지장, 결장의 간 굴곡 등 7가지 중요한 해부학적 랜드마크를 강조하여 복부 유착을 안전하게 분리하고 총담관을 노출시키는 데 도움이 되었습니다. 또한, 담즙 절제술의 시간을 단축하기 위해 일반적인 담관에서 결석을 제거하기 위해 순차적 방법이 혁신적으로 적용되었습니다. 중요한 해부학적 랜드마크를 식별하고 순차적 방법을 채택하는 것을 포함하여 위의 외과적 접근 방식을 마스터하면 LCBDE의 재수술 안전성이 향상되고 수술 시간이 단축되며 환자의 빠른 회복을 촉진하고 수술 후 합병증을 줄이고 이 기술의 대중화 및 적용에 기여합니다.

이 프로토콜은 담도 수술 병력이 있는 환자에서 복강경 총담관 탐사(LCBDE)의 외과적 접근 방식과 해부학적 랜드마크를 조사하여 프로그래밍된 절차를 개발하는 것을 목표로 합니다. 외과적 접근과 해부학적 랜드마크를 기반으로 프로그래밍된 수술은 수술 시간을 단축하고 합병증을 줄이며 수술 안전성을 개선하고 이 기술의 적용을 더욱 촉진하는 데 도움이 될 수 있습니다. 간 원형 인대 접근법에서는 간 원형 인대를 따라 복부 유착을 분리하여 하부 간 가장자리로 방출 한 다음 간 헬륨으로 방출하여 총 담관 또는 CBD를 노출시킵니다.전방 간 십이지장 인대 접근법에서는 십이지장을 간 문에서 아래쪽으로 분리하여 CBD를 간십이지장 인대 전방에 노출시킵니다. 우측 간십이지장 인대 접근법에서는 간십이지장 인대의 우측 측면부터 아래쪽으로 시작하여 간문에 부착된 결장의 간 굴곡을 분리하여 우측 간장하공간과 대구멍 사이의 영역을 노출시켜 CBD의 위치를 결정합니다. 하이브리드 접근법에서는 주로 접착 위치에 따라 달라지는 입증 된 접근법 중 두세 가지를 결합하십시오.전방 복벽 중간에 유착이 있으면 간 원형 인대 접근법을 사용하십시오.
