膵臓突起は、SMAと上腸間膜静脈(SMV)の後ろに位置する膵頭の左下からの突出です1。膵臓手術の管理は、その深い解剖学的構造と困難な曝露のために、膵臓手術において困難な領域です。したがって、特に膵臓癌の場合、手術は切除断端陽性、リンパ節郭清不完全、および予後不良をもたらす傾向があります2。したがって、改善された手術技術と戦略が緊急に必要とされています。
膵頭部のほとんどの切除手順と切除プロセスは、SMVおよびSMA3の右側に沿って実行されます。このアプローチは、主に膨大部腫瘍に有効であるが、膵頭部がん、特に非繊毛突起の大きな腫瘍には欠点がある2。手順中、SMVとSMAを露出させるために、SMVとSMAを回転させたり引っ張ったりすることが頻繁に必要になります。このようなアプローチではIPDAを明確に解剖することができず、通常、出血や洗浄が不十分になります。この研究では、著者らは、IPDAの早期制御をサポートできるアプローチについて説明し、その結果、出血制御が向上し、失血が少なくなり、解剖が改善されます。
一方、従来の開腹術または腹腔鏡下膵十二指腸切除術では、十二指腸および膵頭部を広く動員するためのコーチャー法が必要である4。ただし、この操作では、外科医が手術中に腫瘍を保持しながら腫瘍を圧迫する可能性があるため、腫瘍細胞は門脈(PV)を介して転移する可能性があります4,5,6。ノータッチアイソレーション技術は、膵十二指腸切除術で人気のある概念の1つです。この外科的介入が膵臓がん患者のがん関連予後を増強できるかどうかは、大規模な臨床試験によって証明されていませんが、廣田らの研究7では、分子マーカー(CEA mRNA)を使用して門脈血中のがん細胞を検出することにより、がん細胞の転移を防ぐ上でのノータッチ技術の重要性が報告されています。彼らの研究では、ノータッチ技術群は、従来の技術群よりも門脈がん細胞の拡がり率が低く、3年生存率が高かった。ノータッチLPDは、現在の著者のチームによって、「腫瘍のない」原則に適合する理想的な腫瘍学的手術プロセスとして以前に報告されています8,9。
