Incisiehernia is een veel voorkomende complicatie na een buikoperatie en kan alleen goed worden behandeld met operatie1. Vergeleken met traditionele open incisie herniorrhaphy heeft laparoscopische herniorrhaphy de voordelen van minder chirurgisch trauma, een lagere infectiegraad en sneller postoperatief herstel 2,3. Momenteel is laparoscopische herniorrhaphy de voorkeursmethode voor de behandeling van incisiehernia als er geen contra-indicaties zijn4.
Laparoscopische herniorrhaphy is echter technisch complex. Intraperitoneale onlay mesh (IPOM) wordt vaak gebruikt bij laparoscopische incisie hernia-reparatie, en dit omvat het laparoscopisch plaatsen van een gaas in de buikholte om het herniadefect te bedekken5. Het gaas is een nieuw type middelzwaar monofilament polypropyleen gaas bedekt met een hydrogelbarrière aan de viscerale zijde6. Voor laparoscopische incisiehernia-reparatie met behulp van de IPOM-methode, is het noodzakelijk om de plaatsing van de trocars, de technieken om intra-abdominale verklevingen te scheiden, de technieken voor het hechten van de incisiehernia en de methoden voor het plaatsen en bevestigen van het gaas in de buikholte onder de knie te krijgen. In het bijzonder, als het gaas niet goed is geplaatst en gefixeerd, kan dit leiden tot de herhaling van de hernia, evenals mogelijk ernstige complicaties zoals darmobstructie en mesh-infectie 7,8. Daarom is het beheersen van de juiste mesh-fixatietechniek een belangrijk criterium voor het bereiken van een goed chirurgisch resultaat.
De traditionele methode van mesh-fixatie voor incisiehernia is om het gaas te fixeren met een hernia-nagel met dubbele ring. Nadat het gaas in de buikholte is geplaatst, wordt de rand van het gaas eerst met een spijkerpistool bevestigd en vervolgens wordt de rand van de herniaring gefixeerd9. Deze methode heeft echter een slechte ruimtelijke positionering en het gaas is gevoelig voor verplaatsing, wat leidt tot hernia-herhaling. Door de verschillende mesh-fixatiemethoden te herzien en te analyseren, wordt een nieuwe "contrapositie en uitlijning" -methode voor mesh-fixatie voorgesteld en gepresenteerd in dit protocol10. Bij deze methode worden de grootte en reikwijdte van de incisiehernia van tevoren gemeten, waarna de mesh-fixatiepunten van tevoren kunnen worden gemarkeerd. Wanneer het gaas tijdens de operatie in de buikholte wordt geplaatst, kunnen nagelpistoolfixatie en hechtingsfixatie worden uitgevoerd volgens de eerder gemarkeerde locaties. Deze methode kan de moeilijkheidsgraad van de operatie, de operatietijd, de medische kosten en het optreden van complicaties verminderen. In deze studie wordt deze nieuwe methode vergeleken met de conventioneel gebruikte dubbele lus hernia-nagelfixatiemethode voor mesh-fixatie tijdens laparoscopische incisie hernia-reparatiechirurgie.

Laparoscopische incisie hernia reparatie met behulp van intraperitoneale onlay mesh (IPOM) is een van de meest gebruikte minimaal invasieve methoden voor het repareren van incisie hernia's. De laparoscopische IPOM omvat het implanteren van het gaas in de buikholte door middel van laparoscopie om een buikwandhernia te repareren. Bij de IPOM-operatie wordt na het sluiten van de herniaring een anti-adhesiegaas laparoscopisch geplaatst. De juiste plaatsing van dit gaas is van cruciaal belang voor het succes van de methode en chirurgische vaardigheden zijn vereist om een perfecte plaatsing te bereiken. Als de mesh-plaatsing niet goed wordt beheerst, worden de operatie- en anesthesietijd verlengd. Bovendien kan onjuiste plaatsing van het gaas leiden tot ernstige gevolgen, zoals darmobstructie en mesh-infectie. In dit onderzoek wordt een "contrapositie en uitlijning" mesh-fixatiemethode beschreven, waarbij de fixatiepositie van het gaas vooraf wordt gemarkeerd om de moeilijkheid van het plaatsen van het gaas te verminderen. Een goed geplaatst gaas is volledig vlak op het peritoneum, de randen zijn niet gekruld of gewikkeld en het gaas is stevig bevestigd, zodat er geen verplaatsing is na het verwijderen van de pneumoperitoneumdruk. De "contrapositie en uitlijning" mesh fixatie techniek biedt de voordelen van betrouwbare plaatsing van het gaas en minder complicaties dan andere technieken, en het is gemakkelijk te leren en te beheersen. Het maakt het ook mogelijk om het nagelpistool van tevoren te positioneren op basis van de anatomie van de incisiehernia. Dit maakt het gebruik van het minimaal mogelijke aantal nagels mogelijk en zorgt toch voor een goede fixatie, wat het optreden van complicaties kan verminderen en de kosten van een operatie kan verlagen. De hier beschreven mesh-fixatiemethode is dus zeer geschikt voor klinische toepassingen op basis van de bovengenoemde voordelen.

De voordelen van deze methode zijn nauwkeurige mesh-positionering, stevige fixatie en eenvoudig leren, vooral voor beginners. Na het beheersen van deze methode kan de tijd van mesh-fixatie tijdens bedrijf aanzienlijk worden verkort.Tekst- en beelddemonstratie kan niet effectief het hele proces van deze mesh-bevestigingsmethode weergeven, vandaar dat visuele demonstratie het mogelijk maakt om alle details van deze methode te begrijpen. Begin met het selecteren van een anti-adhesiegaas van de juiste grootte, meet en markeer het geschatte bereik van de incisiehernia op het buikwandoppervlak met een steriele liniaal en markeerpen. De grootte van het hernia-ringdefect kan ook worden gemeten door een preoperatief CT-onderzoek.Plaats de liniaal parallel aan de lengteas van het herniadefect en meet de maximale lengte van het defect. Selecteer een anti-adhesiegaas van de juiste grootte op basis van de grootte van het hernia-ringdefect. Zorg ervoor dat de dekking van het gaas de rand van het defect met ten minste vijf centimeter overschrijdt.Markeer de lengte van het defect op het gaas en markeer de bevestigingspunten van het spijkerpistool met intervallen van vijf centimeter op de lengteas. Verleng vervolgens de bevestigingspunten meer dan vijf centimeter langs de gemarkeerde lijn tot de rand van het gaas dat betrekking heeft op de uitlijning. Markeer de bevestigingspunten van het spijkerpistool gelijkmatig om de twee tot drie centimeter langs de rand van het gaas dat betrekking heeft op de contrapositie.Zorg ervoor dat de bevestigingspunten van het spijkerpistool gelijkmatig zijn gemarkeerd op twee centimeter van de lengteas van het defect aan beide zijden met een interval van drie centimeter. Rol het gaas zodanig dat het anti-adhesieoppervlak naar de buikwand zal kijken en plaats het gerolde gaas in de buikholte door het punctiegat van 12 millimeter. Vouw vervolgens het gaas uit onder laparoscopische begeleiding.Zorg ervoor dat de gemarkeerde lijn van het uitgerolde gaas de lengteas van het hernia-ringdefect overlapt. Bevestig de gemarkeerde punten op de lengteas van het gaas aan de buikwand met niet-absorbeerbare nagels met behulp van een spijkerpistool. Bevestig de rand van het gaas aan de buikwand langs de gemarkeerde punten aan de rand van het gaas met niet-absorbeerbare nagels met behulp van een spijkerpistool.Bevestig het gaas op de buikwand langs de gemarkeerde punten aan beide zijden van de lengteas van het gaas met absorbeerbare nagels met behulp van een spijkerpistool. De contrapositie en uitlijning mesh fixatie of traditionele double-loop hernia nagelfixatie werd uitgevoerd in respectievelijk de experimentele en controlegroepen met 42 patiënten in elke groep. De gemiddelde mesh-plaatsingstijd voor de experimentele groep was ongeveer 32,5 minuten, wat aanzienlijk lager was dan de controlegroep, met een mesh-plaatsingstijd van ongeveer 44,7 minuten.Er waren slechts twee gevallen van chronische pijn in de experimentele groep, significant lager dan de acht gevallen waargenomen in de controlegroep. Er waren echter geen significante verschillen tussen de twee groepen in termen van seromavorming, mesh-infectie, herhaling van incisiehernia, de lengte van het postoperatieve ziekenhuisverblijf en ziekenhuisopnamekosten. Zorg ervoor dat de plaatsing van het gaas nauwkeurig is met voldoende dekking en dat nauwkeurige fixatiemethoden zijn gebruikt.De toepassing van de contrapositie- en uitlijnings mesh-fixatiemethode is niet beperkt tot incisiehernia-reparatie, maar kan ook worden gebruikt voor laparoscopische parastole hernia-mesh-reparatie en laparoscopische slokdarmhiatale hernia-mesh-reparatie.
