切開ヘルニアは腹部手術後の一般的な合併症であり、手術でのみ適切に治療することができます1。従来の開腹切開ヘルニアと比較して、腹腔鏡下ヘルニアには、外科的外傷が少なく、感染率が低く、術後の回復が速いという利点があります2,3。現在、腹腔鏡下ヘルニアは、禁忌がない場合の切開ヘルニアの治療に最適な方法です4。
ただし、腹腔鏡下ヘルニアは技術的に複雑です。腹腔内オンレーメッシュ(IPOM)は、腹腔鏡下切開ヘルニア修復に一般的に使用され、これには、ヘルニア欠損をカバーするために腹腔鏡下で腹腔内にメッシュを配置することが含まれます5。メッシュは、内臓側6のヒドロゲルバリアで覆われた新しいタイプの中重量モノフィラメントポリプロピレンメッシュである。IPOM法を用いた腹腔鏡下切開ヘルニア修復術では、トロカールの配置、腹腔内癒着の分離技術、切開ヘルニアの縫合技術、腹腔内にメッシュを留置・固定する方法を習得する必要がある。特に、メッシュが適切に配置および固定されていない場合、これはヘルニアの再発、ならびに腸閉塞およびメッシュ感染などの潜在的に重篤な合併症をもたらす可能性がある7,8。したがって、正しいメッシュ固定技術を習得することは、良好な手術結果を達成するための重要な基準です。
切開ヘルニアのメッシュ固定の伝統的な方法は、二重リングヘルニア爪でメッシュを固定することです。メッシュが腹腔内に配置された後、メッシュの端が最初にネイルガンで固定され、次にヘルニアリングの端が固定されます9。しかし、この方法では空間的なポジショニングが悪く、メッシュがずれやすく、ヘルニアの再発につながります。様々なメッシュ固定方法をレビューおよび分析することにより、メッシュ固定のための新しい「対次およびアライメント」方法が提案され、このプロトコル10で提示される。この方法では、切開ヘルニアのサイズと範囲を事前に測定し、その後、メッシュ固定点を事前にマークすることができます。手術中にメッシュを腹腔内に配置した場合、以前にマークされた位置に従ってネイルガン固定および縫合糸固定を行うことができます。この方法は、手術の難易度、手術時間、医療費、合併症の発生を減らすことができます。本研究では、腹腔鏡下切開ヘルニア修復手術時のメッシュ固定に従来用いられていたダブルループヘルニア爪固定法と比較した。

このプロトコルは、腹腔鏡下切開ヘルニア修復におけるメッシュ固定の問題を効果的に解決するための新しい方法を提供します。この方法の利点には、特に初心者にとって、正確なメッシュの位置決め、しっかりとした固定、簡単な学習が含まれます。この方法を習得した後、操作中のメッシュ固定の時間を大幅に短縮できます。テキストと画像のデモンストレーションでは、このメッシュ固定方法のプロセス全体を効果的に示すことができないため、視覚的なデモンストレーションにより、この方法の詳細をすべて理解できます。適切なサイズの癒着防止メッシュを選択することから始め、腹壁表面の切開ヘルニアのおおよその範囲を滅菌定規とマーキングペンで測定してマークします。ヘルニアリング欠損の大きさは、術前のCT検査によっても測定することができる。定規をヘルニア欠損部の縦軸に平行に置き、欠損の最大縦長を測定します。ヘルニアリング欠損のサイズに応じて適切なサイズの癒着防止メッシュを選択してください。メッシュのカバレッジが欠陥の端を少なくとも5センチメートル超えていることを確認してください。メッシュ上の欠陥の縦方向の長さをマークし、縦軸上に5センチメートルの間隔でネイルガンの固定点をマークします。次に、マークされた線に沿って固定点を、位置合わせに関連するメッシュの端まで5センチメートル以上延長します。反対のメッシュの端に沿って2〜3センチメートルごとにネイルガンの固定ポイントを均等にマークします。ネイルガンの固定点が、両側の欠陥の縦軸から3センチメートルの間隔で均一にマークされていることを確認してください。癒着防止面が腹壁に面するようにメッシュを転がし、転がしたメッシュを12ミリメートルの穿刺穴を通して腹腔内に置きます。次に、腹腔鏡ガイダンスの下でメッシュを展開します。展開されたメッシュのマークされた線がヘルニアリング欠損の縦軸と重なっていることを確認します。メッシュの縦軸上のマークされたポイントを、ネイルガンを使用して非吸収性の爪で腹壁に固定します。ネイルガンを使用して、メッシュの端のマークされた点に沿って、メッシュの端を非吸収性の釘で腹壁に固定します。ネイルガンを使用して、メッシュの縦軸の両側のマークされた点に沿って腹壁にメッシュを吸収性爪で固定します。対置およびアライメントメッシュ固定または従来の二重ループヘルニア爪固定は、実験群と対照群でそれぞれ実施され、各群に42人の患者がいました。実験群の平均メッシュ配置時間は約32.5分で、対照群よりも著しく低く、メッシュ配置時間は約44.7分でした。実験群の慢性疼痛の症例は2例のみであり、対照群で観察された8例よりも有意に低かった。しかし,漿液腫形成,メッシュ感染,切開ヘルニア再発,術後入院期間,入院費用について有意差は認められなかった。メッシュの配置が正確で適切なカバレッジであり、正確な固定方法が使用されていることを確認してください。対位およびアライメントメッシュ固定法の適用は、切開ヘルニア修復に限定されず、腹腔鏡下傍ストームヘルニアメッシュ修復および腹腔鏡下食道裂孔ヘルニアメッシュ修復にも使用できます。
