절개 탈장은 복부 수술 후 흔한 합병증이며 수술로만 적절하게 치료할 수 있습니다1. 기존의 개방 절개 탈장과 비교하여 복강경 탈장은 수술 외상이 적고 감염률이 낮으며 수술 후 회복이 빠르다는 장점이 있습니다 2,3. 현재 복강경 탈장은 금기 사항이없는 경우 절개 탈장 치료를위한 선택 방법입니다4.
그러나 복강경 탈장은 기술적으로 복잡합니다. 복강내 온레이 메쉬(IPOM)는 일반적으로 복강경 절개 탈장 복구에 사용되며, 이는 탈장 결손5을 덮기 위해 복강경으로 복강에 메쉬를 배치하는 것을포함합니다. 메쉬는 내장 측면6에 하이드로겔 장벽으로 덮인 새로운 유형의 중형 모노필라멘트 폴리프로필렌 메쉬입니다. IPOM 방법을 사용한 복강경 절개 탈장 복구의 경우 투관침의 배치, 복강 내 유착을 분리하는 기술, 절개 탈장을 봉합하는 기술 및 복강에 메쉬를 배치하고 고정하는 방법을 마스터해야합니다. 특히, 메쉬가 적절하게 배치되고 고정되지 않으면 탈장의 재발뿐만 아니라 장폐색 및 메쉬 감염과 같은 잠재적으로 심각한 합병증이 발생할 수 있습니다 7,8. 따라서 올바른 메쉬 고정 기술을 습득하는 것은 좋은 수술 결과를 얻기위한 중요한 기준입니다.
절개 탈장에 대한 메쉬 고정의 전통적인 방법은 이중 링 탈장 손톱으로 메쉬를 고정하는 것입니다. 메쉬를 복강 내로 넣은 후 메쉬의 가장자리를 먼저 네일 건으로 고정한 다음 탈장 링의 가장자리를 고정합니다9. 그러나 이 방법은 공간 위치가 좋지 않고 메쉬가 변위되기 쉬워 탈장 재발을 유발합니다. 다양한 메쉬 고정 방법을 검토하고 분석함으로써, 메쉬 고정을 위한 새로운 "콘트라포지션 및 정렬" 방법이 제안되고 이 프로토콜(10)에 제시된다. 이 방법에서는 절개 탈장의 크기와 범위를 미리 측정 한 후 메쉬 고정 지점을 미리 표시 할 수 있습니다. 수술 중 메쉬가 복강 내로 배치되면 이전에 표시된 위치에 따라 네일 건 고정 및 봉합사 고정을 수행 할 수 있습니다. 이 방법은 수술의 어려움, 수술 시간, 의료 비용 및 합병증 발생을 줄일 수 있습니다. 이 연구에서이 새로운 방법은 복강경 절개 탈장 수리 수술 중 메쉬 고정을 위해 일반적으로 사용되는 이중 루프 탈장 손톱 고정 방법과 비교됩니다.
