此外，重要的是要解决的是，当进行脑电图知情功能磁共振成像分析时，必须明确存在脑电图的关键特征，以指导功能磁共振成像的相应大胆变化。因此，在进行脑电图-功能磁共振成像记录时，重要的是要考虑先前已证明靶脑电图特征的患者。在癫痫患者的发作间期，IEDs异常且提示致痫潜力，是众所周知的脑电图特征，可参考BOLD变化16，即使此处的示例不包括这种情况。当目标是在发作间期脑电图-功能磁共振成像记录中获取IED时，实验者应考虑通过头皮脑电图观察到频繁IED（至少三个IED/小时）的患者，以确保在扫描过程中有足够的癫痫样放电。简易爆炸装置的数量可以通过动车组的脑电图监测来确定，或者通过参考受试者先前脑电图记录中看到的简易爆炸装置频率（如果存在）。获得的发作间期脑电图-fMRI数据的记录可以为理解和潜在定位癫痫发作区域17带来益处。

当使用处理后的脑电图考虑ESI时，必须仔细考虑电极的总数及其分布，以便它们可以合理地覆盖整个头部。实施ESI所需的最小电极数量为32通道21，22，这超过了临床环境中使用的标准电极数量。因此，建议包括额外的通道，以合理的间距覆盖整个头部。本研究中的通道选择包括21个通道，这些通道通常用于临床中的脑电图监测，以及11个额外的通道，以完全覆盖头部（图1）。
在这里，我们不包括功能磁共振成像分析的细节，因为这超出了我们的研究范围。然而，一个可能的方向是脑电图知情的fMRI分析23。例如，IED的发生时间可以保存为事件触发器，以与功能磁共振成像相关联，这可能导致与事件相关的常规功能磁共振成像分析。在这种情况下，广义线性模型分析可用于查找在IED时显示fMRI信号变化的大脑区域。
我们指出，最近发表的一项研究10 表明，当需要更强大的伪影去除技术时，可以使用碳线环系统16。然而，我们想告知的是，在我们的实验环境中，碳丝环系统与MR条件电极的集成尚未得到研究。
尽管这项研究特别关注癫痫发作间期，但同时脑电图-功能磁共振成像的引入方案可以进一步扩展到发作期或发作后期。但是，在考虑任何自定义设置时，必须遵循特定的注意事项。对于发作后期，我们认识到的一个重要问题是，患者在转运至MRI之前应给予苯二氮卓类药物。至于脑电图的频率分析，据报道，苯二氮卓类不一定会改变特定的频段24，25，并且在适度变化的情况下，这些仅限于躯体感觉运动区域26或额叶27。此外，关于同步脑电图-功能磁共振成像，与盐水注射对照组相比，苯二氮卓类药物注射后的δ脑电图-BOLD相关性没有变化27。BOLD信号仅在Heschel回和辅助运动区域的小区域降低。

癫痫折磨着全世界近7000万人1。每年每150名癫痫控制不佳的人中就有一人死于癫痫猝死（SUDEP）。此外，大约30%-40%的癫痫病例对药物治疗无效2。对于难治性癫痫患者来说，切除、断开或神经调控形式的神经外科治疗可能是一种改变生活和挽救生命的措施。

局灶性 delta 活动缓慢通常提示潜在的脑结构病变或神经元功能不按预期进行的区域，这通常在卒中、头部受伤、脑部感染或痴呆后观察到。然而，局灶性癫痫患者在其致痫区附近或部位表现出局灶性 delta 活动的情况并不少见。此外，尽管局灶性δ活性的特异性低于IED，但它可以指定与癫痫发作相对应的局部结构病理12。此外，脑电图中局灶性发作间期慢波对应于功能磁共振成像上的局灶性 BOLD 激活，与部分性癫痫患者的致痫区域相匹配13.
值得注意的是，在颞叶发作性癫痫中，有时存在一种称为颞间歇性节律性δ（TIRDA）的δ活动，它被认为是IED等效的14。相反，时间间歇多态性增量（TIPDA）不被视为IED等效物15。在处理后的脑电图中，存在清晰的焦点左颞慢波（TIPDA），这是从EMU记录的脑电图中观察到的（图7）。虽然这种δ活性不是IED等效的，但它表明左颞神经元功能障碍。
