또한, EEG 정보 fMRI 분석이 수행될 때 fMRI의 해당 BOLD 변화를 안내하기 위해 EEG의 주요 특징이 명확하게 존재해야 한다는 점을 해결하는 것이 중요합니다. 따라서 EEG-fMRI 기록을 수행할 때 이전에 표적 EEG 특징을 입증한 환자를 고려하는 것이 중요합니다. 간질 환자의 간질 기간 동안, 비정상적이고 간질 발생 가능성을 시사하는 IED는 BOLD 변화16를 참조하는 잘 알려진 EEG 특징이지만, 이 사례는 이 경우를 포함하지 않는다. interictal EEG-fMRI 기록에서 IED를 얻는 것을 목표로 할 때 실험자는 스캐닝 세션 동안 충분한 간질 분비물을 보장하기 위해 두피 EEG에서 관찰되는 빈번한 IED(최소 3회/시간)가 있는 환자를 고려해야 합니다. 간질은 전 세계적으로 거의 7천만 명의 사람들을 괴롭힌다1. 간질이 잘 조절되지 않는 150 명 중 1 명은 매년 간질로 인한 갑작스런 예기치 않은 사망 (SUDEP)에 굴복합니다. 또한, 간질 사례의 약 30%-40%는 의학적 관리에 불응한다2. 절제, 단절 또는 신경 조절의 형태로 된 신경외과적 치료는 난치성 간질 환자의 삶을 변화시키고 생명을 구하는 조치가 될 수 있습니다.
동시 뇌파도 및 기능적 자기 공명 영상(EEG-fMRI)은 뇌 활동을 비침습적으로 측정하는 독특한 결합 기술이며, 간질에서 발작 발병을 이해하고 국소화하는 데 이점을 제공합니다 3,4,5,6. 두피 EEG는 발작 발병 영역을 측면화하고 국소화하는 데 사용할 수 있지만 심부 간질 유발 원인을 평가하는 것과 관련하여 제한된 기능으로 인해 공간 해상도가 상대적으로 낮습니다. fMRI는 깊은 영역을 포함하여 뇌 전체에 걸쳐 좋은 공간 해상도를 가지고 있지만 fMRI만으로는 발작에 특이적이지 않습니다. 그러나 두피 EEG는 fMRI에서 혈중 산소 수준 의존성(BOLD) 활성화 또는 비활성화 영역의 해석을 알려줄 수 있으므로 간질에 특정한 fMRI 기술을 생성할 수 있습니다. 따라서 동시 EEG-fMRI의 구현은 간질 사건의 '장소'와 '시기'를 모두 국소화하는 것과 관련된 시공간 과정을 매핑하는 데 사용할 수 있습니다.
