前腰椎椎に腹部アプローチをとる固定術である前腰椎椎体間固定術(ALIF)は、1983年にオブライエンによって最初に説明されました1。出血が少なく、筋肉や神経の損傷が少なく、腰椎前弯症を回復し、後腰椎椎体体間固定術よりも隣接する分節変性への影響を軽減する能力があるという利点により、ALIFは現在、腰椎すべり症、脊椎変形、腰椎感染症、および変性腰椎椎間板疾患の治療に広く使用されています2,3.ただし、この手順は、血管、神経、尿管の損傷などの合併症を引き起こす可能性もあります。早期の血管損傷は特に一般的であり、症例の10.4%で発生します4,5,6,7,8,9。
手術中に顕微鏡を使用すると、より明確な手術野が可能になり、組織の損傷を減らすという点で安全性が高くなり、外科的切開が小さくなります8,10。ただし、低侵襲前腰椎椎体間固定術 (Mini-ALIF) は依然として高度な視覚化と安定性が必要であり、適切なスプレッダーを使用する必要があります。SynframeシステムのActiv Oスプレッダーや現在使用されているMiaspas-ALIFスプレッダー(Aesculap)などの初期のフレーム腹腔鏡スプレッダーは、主に従来の開腹ALIF手術に使用されます11が、顕微鏡安定性が低く、設置要件が複雑で、コストが高いため、腰椎前部手術での顕微鏡検査の使用が妨げられています。
本研究では、椎体に固定されたブロックとリトラクターを介して、顕微鏡下で手術野を開き、神経減圧を行う顕微鏡支援Mini-ALIFの新しいスプレッダーシステムを紹介します。本研究は、顕微鏡支援前部減圧融合術の技術的側面を調査し、Mini-ALIFに適したスプレッダーシステムを導入することを目的としています。このシステムは、良好な術中椎間板曝露、重要な構造の良好な識別、椎間腔の適切な広がり、および必要な椎間高さの回復を提供することができ、経験の浅い外科医にとって非常に役立ちます。

本研究は、顕微鏡支援前部減圧固定術の技術的側面を調査し、低侵襲前腰椎椎体間固定術(Mini-ALIF)に適したスプレッダーシステムを導入することを目的としています。この記事では、顕微鏡下での腰椎前部手術の技術的な説明です。2020年7月から2022年8月の間に当院で顕微鏡支援Mini-ALIF手術を受けた患者さんの情報を遡及的に収集した。反復測定ANOVAを使用して、期間間の画像指標を比較しました。42人の患者が研究に含まれました。.術中出血の平均量は180mLで、平均手術時間は143分でした。.平均追跡期間は18ヵ月であった。腹膜破裂の1例を除けば、他の重篤な合併症は発生しなかった。術後の孔と椎間板の高さは、どちらも手術前よりも平均して高かった。スプレッダーアシストマイクロミニALIFはシンプルで使いやすいです。術中の椎間板曝露、重要な構造の識別、椎間腔の適切な広がり、必要な椎間高さの回復が可能で、経験の浅い外科医にとって非常に役立ちます。
