年次モデルは、罹患したヒト大動脈弁の信頼できる情報源へのアクセスがないため、大動脈弁狭窄症の根底にある病理学的メカニズムを理解するための重要なツールとして浮上しています。従来の大動脈弁狭窄症ウサギモデルでは、大動脈弁に著しい狭窄を誘発するために長期的な栄養補助食品と遺伝子操作が必要であり、実験的研究での使用が制限されています。この技術は、大動脈弁への直接風船損傷によってウサギに大動脈弁狭窄症を誘発する新しい手順を説明しており、大動脈弁狭窄症のヒトの状態を正確に模倣することができます。手順を実演するのは、私の規制担当の研究専門家であるパク・ウネ、キム・ジンム、イ・ウンミです。生理食塩水と市販の造影剤の1対1の混合物で満たされた収縮装置をバルーンカテーテルのルアーロック部分に接続することにより、拡張バルーンカテーテルセットの準備を開始します。バルーンにインフレーション溶液を充填した後、バルーンカテーテルから空気を取り除きます。麻酔をかけられたウサギの辺縁耳介静脈に24ゲージの静脈内またはIVカテーテルを挿入することから手術手順を開始します。ヘパリンヘパリン化生理食塩水1キログラムあたり100単位の輸液セットを接続します。次に、ウサギをマルチパラメータ獣医モニターに接続して、酸素飽和度信号やSpO2温度、血圧などのバイタルサインを継続的に監視します。Cアーム透視装置を備えた手術台にウサギを仰臥位にした後、動物のバリカンを使用して腹側頸部の毛を取り除きます。切開部をヨウ素で滅菌し、ウサギを手術用タオルで覆います。ウサギの心臓の位置を特定するには、Cアームをオンにして、心臓イメージング用の透視モードを選択します。ウサギの位置を調整して、心臓がイメージングフィールドの中心に来るようにします。位置を調整したら、首の皮膚を縦方向に約3センチ切開し、手術用ハサミを使用して筋膜と脂肪組織を切断します。次に、左総頸動脈(LCCA)の約3〜3.5センチメートルが露出するまで、筋肉を慎重に分離します。露出したLCCAの上部と端に3-0シルク縫合糸でLCCAを結紮し、血流を止めます。22ゲージのIVカテーテルをLCCAに挿入します。次に、0.016インチのガイドワイヤーをIVカテーテルを介して左心室に導入し、カテーテルの先端がCアームのイメージング野に適切に配置されるようにします。次に、IVカテーテルを抜いてガイドワイヤーを残してから、ガイドワイヤーの上に4フレンチシースをLCCAに配置してバルーンカテーテルを導入します。8ミリのバルーンカテーテルをガイドワイヤーからCアーム透視ガイド下で大動脈弁に慎重に挿入します。バルーンカテーテルの先端を大動脈弁から約1〜2センチメートル離れた位置に置き、6気圧の圧力インフレータで膨張液をパージしてバルーンを膨らませます。次に、バルーンを左心室またはLV頂点に進め、LV出口に引き戻します。バルーンの空気を抜く前に、この手順を5回繰り返します。バルーンカテーテルとガイドワイヤーを抜いてから、LCCAからシースをゆっくりと取り外し、すぐにLCCAを縫合糸で結び、大動脈弁まで下がります。切開部を生理食塩水で洗浄して血栓を取り除きます。3-0の非吸収性縫合糸で筋肉と皮膚を閉じる前に、穿刺された部位に動脈出血がないか検査します。終わったら、ヨウ素で傷口を滅菌します。手術後、モニタリングパッチとクリップを取り外し、ウサギを集中治療室の保育器に保管します。バルーン損傷から8週間後、麻酔をかけたウサギを仰臥位のエコー台に置きます。胸部を剃った後、超音波トランスデューサージェルを胸部に塗布します。トランスデューサーを調整して、大動脈弁の胸骨周囲長軸図と胸骨周囲短軸図を取得します。モードイメージングを使用して大動脈弁の画像を記録し、後で分析するために画像を保存します。大動脈弁の構造変化評価により、大動脈弁損傷から8週間後、高コレステロールとビタミンD2食を与えられた傷ついたウサギは、対照または野生型のウサギと比較して、尖頭が肥厚し、運動が制限され、高コレステロールとビタミンD2の食餌を与えたウサギは弁損傷なしで行動が制限されたことが明らかになりました。ウサギは犠牲にされ、大動脈弁の組織学的分析のために損傷の8週間後に心臓が切除されました。マッソンのトリクロームで染色された大動脈弁は、高コレステロールおよびビタミンD2ダイエット誘発群の対照群と比較して、負傷者群の大動脈弁尖の厚さの増加を示しました。アリザリンレッド染色とフォンコッサ染色を実施して、高コレステロールおよびビタミンD2食誘発群の弁膜弁尖にごくわずかなカルシウム沈着を示した弁膜症カルシウム沈着の程度を比較しました。対照的に、バルーン損傷群では有意な石灰沈着が観察された。ここでは、ウサギモデルで北極弁狭窄症を誘発するための成功した技術を、大動脈弁への直接バルーン損傷の段階的な説明とともに示しています。このシンプルで再現性のあるモデルは、大動脈弁狭窄症の開始と進行を研究するための有望なアプローチを提供し、大動脈弁狭窄症の根本的な病理学的メカニズムを調査するための貴重なツールを提供します。
