痙性脳性麻痺は、脳性麻痺の全小児の半数以上が甚倒し1、腱拘縮、骨格発達異常、可動性の低下を引き起こし、罹患した小児の生活の質に大きな影響を与える2。痙性脳性麻痺の治療のための主要な外科的方法として、選択的背側根切開術(SDR)は、多くの国で完全に検証され、推奨されています3,4。しかし、椎弓板の正確な切断、神経根の位置決めと解離、神経線維の切断など、SDR手術の複雑でリスクの高い性質は、臨床現場でSDRに取り組み始めたばかりの若い医師にとって大きな課題となっています。さらに、SDRの学習曲線は非常に急です。
従来の整形外科手術では、外科医は術前のすべての2次元(2D)画像を精神的に統合し、3D手術計画を作成する必要があります5。このアプローチは、複雑な解剖学的構造やSDRなどの外科的操作を含む術前計画では特に困難です。医用画像とコンピューター技術の進歩により、コンピューター断層撮影(CT)や磁気共鳴画像法(MRI)などの2D軸画像を処理して、患者固有の解剖学的構造を備えた3D仮想モデルを作成できます6。視覚化の改善により、外科医はこの処理された情報を分析して、患者の状態に合わせたより詳細な診断、計画、および外科的介入を行うことができます。近年、整形外科におけるマルチモーダル画像融合技術の応用が徐々に注目されています7。この技術は、CT画像とMRI画像を融合し、digital3Dアナログモデルの精度を大幅に向上させることができます。しかし、SDRの術前モデルへの応用はまだ研究されていません。
SDR手術中の椎弓板と脊髄神経の正確な位置決めと正確な切断は、結果を成功させるために重要です。通常、これらのタスクは専門家の経験に依存し、手術中にCアームによって繰り返し確認されるため、複雑で時間のかかる外科的プロセスが発生します。3Dデジタルモデルは、将来のSDR手術ナビゲーションの基盤として機能し、椎弓切除術の術前計画にも利用できます。このモデルは、CTの骨構造とMRIの脊髄神経構造を融合し、手術計画に従って切断用にマークされた腰椎切片に異なる色を割り当てます。このようなSDR用のホログラフィック3D印刷モデルは、術前の計画とシミュレーションを容易にするだけでなく、正確な3Dナビゲーション座標を術中のロボットアームに出力して正確な切断を行います。
