경련성 뇌성마비는 뇌성마비 아동의 절반 이상에게 영향을 미치며1, 힘줄 구축, 비정상적인 골격 발달, 이동성 저하를 유발하여 영향을 받는 아동의 삶의 질에 큰 영향을 미친다2. 경련성 뇌성마비 치료를 위한 주요 수술 방법인 선택적 등쪽 신경근 절개술(SDR)은 많은 국가에서 완전히 검증되고 권장되었습니다 3,4. 그러나 박판의 정확한 절단, 신경근의 위치 지정 및 해리, 신경 섬유의 절단을 포함한 SDR 수술의 복잡하고 고위험 특성은 임상 실습에서 SDR에 이제 막 참여하기 시작한 젊은 의사들에게 중요한 도전 과제입니다. 또한 SDR의 학습 곡선은 매우 가파르다.
전통적인 정형외과 수술에서 외과의는 모든 수술 전 2차원(2D) 영상을 정신적으로 통합하고 3D 수술 계획을 세워야 한다5. 이 접근법은 복잡한 해부학적 구조와 SDR과 같은 외과적 조작을 포함하는 수술 전 계획에 특히 어렵습니다. 의료 영상 및 컴퓨터 기술의 발전으로 컴퓨터 단층 촬영(CT) 및 자기 공명 영상(MRI)과 같은 2D 축 영상을 처리하여 환자별 해부학적 구조6를 갖춘 3D 가상 모델을 생성할 수 있다. 향상된 시각화를 통해 외과의는 이 처리된 정보를 분석하여 환자의 상태에 맞는 보다 자세한 진단, 계획 및 외과적 개입을 수행할 수 있습니다. 최근 몇 년 동안 정형외과에서 멀티모달 이미지 융합 기술의 적용이 점차 주목을 받고 있다7. 이 기술은 CT와 MRI 이미지를 융합하여 digital3D 아날로그 모델의 정확도를 크게 향상시킬 수 있습니다. 그러나 SDR의 수술 전 모델에서 이 기술의 적용은 아직 연구되지 않았습니다.
SDR 수술 중 박판과 척수 신경의 정확한 위치 지정과 정확한 절단은 성공적인 결과를 위해 매우 중요합니다. 일반적으로 이러한 작업은 전문가의 경험에 의존하며 수술 중 C-arm에 의해 반복적으로 확인되므로 복잡하고 시간이 많이 소요되는 수술 과정이 발생합니다. 3D 디지털 모델은 향후 SDR 수술 탐색의 기초 역할을 하며 추궁 절제술 절차의 수술 전 계획에도 활용할 수 있습니다. 이 모델은 CT의 뼈 구조와 MRI의 척수 신경 구조를 융합하고 수술 계획에 따라 절단이 표시된 요추 부분에 다른 색상을 할당합니다. 이러한 SDR용 홀로그램 3D 프린팅 모델은 수술 전 계획 및 시뮬레이션을 용이하게 할 뿐만 아니라 정확한 절단을 위해 수술 중 로봇 팔에 정확한 3D 탐색 좌표를 출력합니다.
