השיטה המסורתית לבידוד תאי גזע שמקורם בשומן אנושי (hASCs) כוללת עיכול אנזימטי המבוצע עם collagenase ואחריו שטיפת שלבים באמצעות צנטריפוגה4. בעוד בידוד אנזימטי נחשב בדרך כלל יעיל יותר מאשר טכניקות מכניות אחרות במונחים של תפוקת תאים וכדאיות, רכיבים שמקורם מן החי בשימוש, כגון collagenase, נחשבים יותר מניפולציה מינימלית על ידי מנהל המזון והתרופות האמריקאי. משמעות הדבר היא שיש סיכון מוגבר באופן משמעותי לתגובות חיסוניות או העברת מחלות, ובכך להגביל את התרגום של טיפול hASC להגדרות קליניות 5,6.

מלבד זאת, מניפולציה נוספת עם תוספת קסנוגנית, כגון סרום בקר עוברי (FBS), המשמש כתוסף גורם גדילה לתרבית תאים, קשורה לסיכון מוגבר לתגובתיות חיסונית וחשיפה לזיהומים ויראליים, חיידקיים או פריונים של החולה המארח 9,10. תגובות חיסוניות והתפתחות פריחה באורטיקריפורמים כבר תוארו אצל אנשים שקיבלו מספר מנות של תאי גזע מזנכימליים המיוצרים עם FBS11. יתר על כן, FBS נתון לשונות אצווה לאצווה, אשר יכולה להשפיע על איכות המוצר הסופי12.
בהתאם להנחיות תנאי ייצור נאותים (GMP), יש להימנע מדיסוציאציה אנזימטית של רקמות, ויש להחליף את FBS בתוספים נטולי קסנוגניות. צעדים אלה, יחד עם פרוטוקולים אמינים יותר הניתנים לשחזור, חיוניים לתמיכה ביישום של טיפול תאי 3,13.
בהקשר זה, הוצע ליזט טסיות דם אנושי (hPL) כתחליף ל- FBS מכיוון שהוא תוסף ללא תאים, המכיל חלבונים ומועשר בגורמי גדילה, והוא הוצג מוקדם יותר בקרב מוצרים מבוססי תאים ברמה קלינית כתוסף של מדיום גדילה לתרבית תאים חוץ גופית והרחבה14,15. מכיוון ש- hPL הוא מוצר שמקורו בבני אדם, הוא משמש לעתים קרובות כתחליף ל- FBS במהלך תרבית החוץ גופית של hASCs המיועדים ליישומים קליניים, ובכך מפחית בעיות לגבי תגובות אימונולוגיות וזיהומים הקשורים לתרגום FBS15,16.
למרות עלויות הייצור הגבוהות יותר, כבר הוכח כי בהשוואה ל- FBS, hPL תומך בכדאיות התא עבור סוגי תאים רבים, מגביר את ההתפשטות, מעכב הזדקנות, מבטיח יציבות גנומית ומשמר את האימונופנוטיפ התאי גם במעברי תאים מאוחרים; כל המרכיבים הללו תומכים במעבר למוסף תרבות זה11.
מטרת העבודה הזו הייתה לפתח פרוטוקול סטנדרטי לבידוד ולתרבית של hASCs בשיטה מלאה נטולת בעלי חיים, מבלי לשנות את הפיזיולוגיה של התא ואת תכונות הגבעול בהשוואה ל-hASCs קלאסיים בתרבית FBS (איור 1).
