지난 수십 년 동안 혁신적인 치료법에 대한 수요가 증가함에 따라 중개 의학 분야에 상당한 노력과 자원 투자가 이루어졌습니다1. 세포 기반 제품은 세포 공급원, 제조 공정(분리, 확장 또는 유전자 변형) 및 비세포 보충제(효소, 성장 인자, 배양 보충제 및 항생제)에 의해 결정되는 위험과 관련이 있으며 이러한 위험 요소는 특정 치료 적응증에 따라 다릅니다. 최종 제품의 품질, 안전성 및 효능은 위에 명시된 요소2에 의해 크게 영향을 받을 수 있습니다. 줄기 세포 치료는 생물 안전 원칙을 준수해야합니다. 세포 배양에서 동물 유래 제품으로 인한 병원성 전파의 잠재적 위험은 문제가 될 수 있으며 제조에 도입된 모든 제품에 대한 철저한 테스트가 필수적입니다3.
인간 지방유래 줄기세포(hASC)를 분리하는 전통적인 방법은 콜라게나제(collagenase)를 이용한 효소 분해 후 원심분리를 통한 세척 단계를 거친다4. 효소 분리는 일반적으로 세포 수율 및 생존력 측면에서 다른 기계적 기술보다 더 효율적인 것으로 간주되지만, 콜라게나제와 같은 동물 유래 성분은 미국 식품의약국(FDA)에서 최소한으로 조작하는 것 이상으로 간주됩니다. 이는 면역 반응이나 질병 전파의 위험이 현저히 증가하여 hASC 요법의 임상 환경으로의 번역을 제한한다는것을 의미합니다 5,6.
트립신 기반 소화는 ASC를 분리하는 또 다른 효소 프로토콜입니다. 트립신 농도, 원심분리 속도 및 배양 시간 측면에서 약간의 수정을 가하여 다양한 기술이 설명되었습니다. 안타깝게도 이 방법은 잘 설명되어 있지 않으며, 문헌, 특히 기계적 격리 프로토콜7에 대한 비교가 부족하다. 그러나, 접근법의 번역 가능성의 관점에서, 트립신은 콜라게나제와 동일한 단점을 가지고 있습니다.
기계적 힘에 기초하고 효소를 첨가하지 않고 ASC를 얻기 위한 대체 분리 방법에는 지방 조직 조각의 고속 원심분리(800 x g 또는 1,280 x g, 15분)가 포함됩니다. 이어서, 펠렛을 적혈구 용해 완충액(5분)과 함께 인큐베이션한 후, 배양 배지에서 재현탁하기 전에 600 x g에서 또 다른 원심분리 단계를 거친다. 체외이식편 방법에 비해 첫 번째 날에 더 많은 수의 세포가 분리되었음에도 불구하고, 이전 연구에서는 배양 2주 이후에 증식이 더 낮거나 없는 것으로 나타났다8.
그 외에도, 세포 배양을 위한 성장 인자 보충제로 사용되는 소 태아 혈청(FBS)과 같은 이종 첨가 배지를 사용한 추가 조작은 숙주 환자의 면역 반응성 위험 증가 및 바이러스, 박테리아 또는 프리온 감염에 대한 노출과 관련이 있다 9,10. 면역 반응과 두드러기형 발진 발달은 FBS11로 생산된 중간엽 줄기 세포를 여러 번 투여받은 개인에서 이미 설명되었습니다. 또한, FBS는 배치-배치(batch-to-batch) 변동성을 가지며, 이는 최종 제품 품질에 영향을 미칠 수 있다12.
GMP(Good Manufacturing Practice) 지침에 따라 효소 조직 해리를 피해야 하며 FBS는 이종유전자가 없는 보충제로 대체해야 합니다. 이러한 단계들은 보다 신뢰할 수 있고 재현 가능한 프로토콜과 함께, 세포 치료제의 적용을 지원하는 데 필수적이다 3,13.
이러한 맥락에서, 인간 혈소판 용해물(hPL)은 무세포, 단백질 함유, 성장 인자 강화 보충제이기 때문에 FBS의 대체품으로 제안되어 왔으며, 시험관 내 세포 배양 및 확장을 위한 성장 배지의 첨가제로서 임상 등급 세포 기반 제품들 사이에 일찍이 도입되었다14,15. hPL은 인간 유래 제품이기 때문에 임상 적용을 위한 hASC의 체외 배양 중에 FBS의 대체물로 자주 사용되어 FBS 번역성과 관련된 면역학적 반응 및 감염과 관련된 문제를 줄입니다15,16.
더 높은 생산 비용에도 불구하고 FBS에 비해 hPL은 많은 세포 유형에 대한 세포 생존력을 지원하고, 증식을 증가시키고, 노화를 지연시키고, 게놈 안정성을 보장하고, 후기 세포 계대에서도 세포 면역 표현형을 보존한다는 것이 이미 입증되었습니다. 이러한 모든 요소는 이 문화 보충11로의 전환을 지원합니다.
