1. KOA 마우스 모델 구축

KOA 통증의 원인은 염증 반응, 활막 증식, 연골 변성과 관련이 있을 수 있다2. 활막 조직은 활막 섬유아세포(synovial fibroblast)와 대식세포(macrophage) 3,4,5의 두 가지 유형의 세포로 구성되어 있습니다. 활막 섬유아세포는 활액을 생성합니다. 활막 대식세포는 일반적으로 휴면 상태이며 염증 반응에 의해 활성화됩니다. 활막의 초기 염증은 무릎 관절 통증을 유발한다6.
활막 조직 염증 면역 반응은 KOA의 발병에 중요한 역할을 합니다. 이전 연구에서는 활막염으로 알려진 KOA의 활막 조직에 염증 반응이 있음을 확인했으며, KOA의 활막염 정도는 활막 조직의 염증 세포 침윤과 밀접한 관련이 있습니다 7,8,9. 활막염은 활막의 염증 반응이며, 그 병리학적 특징은 활막 세포의 증식, 새로운 혈관 형성 및 염증 세포의 침윤이다 5,10,11.

은 나노 입자의 크기는 100nm 미만입니다. 눈에 띄는 항염증, 항균 및 항산화 효과로 인해 상처 치유 및 화상 부상과 같은 의료 및 의학의 다양한 측면에서 널리 활용되었습니다15,16. 또한 표적 약물 전달, 의료 영상 및 분자 진단에도 사용됩니다17. 은(Ag)은 구리(Cu), 아연(Zn) 및 철(Fe)15과 같은 다른 금속 나노 입자보다 항염증 및 항균 작용이 더 큽니다. 새로운 유형의 나노 물질인 은 나노 입자는 광범위하고 강력한 항균 특성을 가지고 있습니다. 이전 연구에 따르면 화상 부상 및 복막염 마우스 모델18,19에서 은 나노 입자는 염증 인자의 생성을 효과적으로 억제하고 상처 치유를 촉진할 수 있습니다. 이전 연구에서는 은 나노 입자가 성장 인자와 콜라겐 침착의 합성을 촉진하여 당뇨병 상처의 치유를 향상시킨다는 것을 입증했습니다20.

무릎 골관절염(KOA)은 45세 이상의 사람들에게 가장 흔하게 발생하는 관절의 퇴행성 질환 중 하나입니다. 현재 KOA에 대한 효과적인 치료법은 없으며 유일한 종점 전략은 무릎 인공관절 전치환술(TKA)입니다. 따라서 KOA는 경제적 부담 및 사회적 비용과 관련이 있습니다. 면역 염증 반응은 KOA의 발생과 발달에 관여합니다. 우리는 이전에 유형 II 콜라겐을 사용하여 KOA의 마우스 모델을 확립했습니다. 활막 조직의 증식은 많은 수의 침윤된 염증 세포와 함께 모델에 존재했습니다. 은 나노 입자는 상당한 항염증 효과가 있으며 종양 치료 및 외과적 약물 전달에 널리 사용되었습니다. 따라서 콜라겐분해효소 II 유도 KOA 모델에서 은 나노입자의 치료 효과를 평가했습니다. 실험 결과는 은 나노 입자가 활막 증식과 활막 조직의 호중구 침투를 크게 감소시킨다는 것을 보여주었습니다. 따라서 이 연구는 OA에 대한 새로운 전략의 식별을 보여주고 KOA의 진행을 방지하기 위한 이론적 기초를 제공합니다.

KOA 마우스 모델은 II형 콜라겐분해효소를 사용하여 유도하였다. 모델 유도 후 1주일 후부터 제조된 은 나노입자 콜라겐 혼합물을 4주 동안 일주일에 한 번 관절강에 주입했습니다(그림 1). 각 그룹의 쥐의 체중을 관찰하고 매일 기록했습니다. 그 결과 KOA 마우스의 평균 체중은 정상 대조군의 마우스보다 현저히 낮았다. 그러나 II형 콜라겐분해효소 + AgNPs 투여군에 속한 마우스의 평균 체중은 KOA 마우스에 비해 더 높았지만, 이 차이는 통계적으로 유의하지 않았습니다(그림 2). 30일 후, 무릎 관절의 활막 조직을 마우스로부터 채취하여 병리학적 검사를 실시하였다. 증식, 혈관 증식, 활막의 염증성 침윤 및 연골 손상이 5,10,11로 분석되었습니다. 그 결과, KOA 투여군에서 마우스의 활막 두께가 정상 대조군에 비해 유의하게 높은 것으로 나타났다. 은 나노입자 콜라겐 혼합물을 처리한 그룹에서는 KOA 그룹에 비해 활막의 두께가 감소하였다(그림 3). 정상 대조군에 비해 KOA 마우스의 활막에서 혈관 증식이 있었고, 은 나노입자 콜라겐 혼합물로 처리한 마우스의 활막에서 혈관 증식이 현저히 감소했습니다(그림 4). Safranin-O 염색 결과는 KOA 마우스의 연골 매트릭스가 파괴된 반면, 은 나노 입자 콜라겐 혼합물로 처리된 마우스는 훨씬 더 나은 연골 매트릭스를 보여주었습니다(그림 5). 각 그룹의 형태학적 특징 점수는 앞서 설명한 바와 같이 평가하였다22. 결과는 식염수 투여군의 경우 0 ± 0, II형 콜라겐 투여군의 경우 7 ± 0.63, II형 콜라겐 분해효소 + AgNPs 투여군의 경우 4.2 ± 1.17이었다(그림 6). CD177은 주요 호중구 마커(25)이다. CD177은 정상적인 조건에서 호중구의 40%-60%에서 발현됩니다. 그러나 호중구에서 CD177의 발현은 급성 염증 중에 크게 증가합니다. IHC 염색 결과는 KOA 그룹에 비해 AgNP로 치료한 그룹에서 활막 부위의 침윤 호중구가 유의하게 감소했음을 보여주었으며(그림 7), 이는 AgNP로 치료하면 KOA의 증상을 개선할 수 있음을 시사합니다.

HE 염색 결과 KOA 그룹 마우스의 활막 두께가 대조군에 비해 유의하게 높은 것으로 나타났습니다.나노입자로 처리한 군의 활액 두께는 KOA군에 비해 감소하였다. 활막의 혈관 증식은 KOA 그룹에서 관찰되었으며, 나노입자 처리군에서는 유의하게 감소된 것으로 나타났다.
