In eerste instantie keken we in de mondholte om de locatie van de luchtpijp19 te vinden. Acuut longletsel (ALI) is een veel voorkomend klinisch syndroom. Onder een verscheidenheid aan pathogene factoren leidt de verstoring van de fysiologische barrière van de longepitheelcellen en vasculaire endotheelcellen tot verhoogde alveolaire permeabiliteit, waardoor verminderde longvolgzaamheid, longoedeem en ernstige hypoxemie1 worden veroorzaakt. Acute respiratory distress syndrome (ARDS) is de meest ernstige vorm van ALI. Ongecontroleerde ontsteking en oxidatieve stressschade worden beschouwd als de belangrijkste oorzaken van ALI en de meer ernstige ARDS2. Wanneer alveolaire epitheelcellen direct gewond raken als gevolg van trauma, wordt de ontstekingsreactieketen van alveolaire macrofagen geactiveerd, wat leidt tot ontsteking in de long3. Wereldwijd zijn er meer dan 3 miljoen patiënten met acute ARDS per jaar, en zij zijn goed voor ongeveer 10% van de opnames op de intensive care; Bovendien is het sterftecijfer in ernstige gevallen zo hoog als 46%4,5,6. Daarom is het nodig om een geschikt diermodel van ALI vast te stellen om de pathogenese ervan te bestuderen. De muis is het meest gebruikte proefdier in de studie van ALI, omdat de luchtwegen de menselijke luchtwegen goed kunnen simuleren voor ALI-studies. Bovendien manifesteert ALI zich als massale inflammatoire celinfiltratie, verhoogde pulmonale vasculaire permeabiliteit en longoedeem. De veranderingen in inflammatoire cytokines in serum en de long dry-wet weight ratio weerspiegelen de mate van ALI7.
Op dit moment omvatten de belangrijkste methoden voor het modelleren van LPS-geïnduceerde ALI bij muizen intranasale en chirurgische tracheale intubatie 8,9. Hier stellen we een nieuwe methode voor om LPS in de luchtpijp af te leveren via niet-invasieve orofaryngeale intubatie. Deze methode maakt gebruik van een verlichte intubator om de luchtpijp van de muis te vinden en levert vervolgens LPS af in de luchtpijp en long. Deze methode levert LPS nauwkeuriger aan de longen dan de intranasale toedieningsmethode. In vergelijking met chirurgische tracheale intubatie vereist deze methode geen operatie, vermijdt het veroorzaken van wonden en vermindert het de pijn bij muizen10. Daarom kan deze methode worden gebruikt om een overtuigender muismodel van ALI vast te stellen.

Het muismodel voor acute longschade (ALI) geïnduceerd door lipopolysaccharide (LPS) of endotoxine is nog steeds een van de meest gebruikte modellen in dierstudies van acuut longletsel of acute ontsteking. De huidige meest gebruikte methoden in muismodellen met acuut longletsel zijn een intraperitoneale injectie van LPS en tracheostomie voor de tracheale infusie van LPS. De eerste methode mist echter longtargeting en beschadigt andere organen, en de laatste methode induceert operatief trauma, infectierisico en een lage overlevingskans. Hier raden we een niet-invasieve orofaryngeale endotracheale intubatiemethode aan voor LPS-instillatie bij muizen. Bij deze methode wordt LPS niet-invasief in de luchtpijp ingebracht via de orofaryngeale holte om in de long te worden ingebracht met behulp van een apparaat voor endotracheale intubatie. Deze methode zorgt niet alleen voor longtargeting, maar voorkomt ook schade en het risico op overlijden bij de dieren. We verwachten dat deze aanpak op grote schaal zal worden gebruikt op het gebied van acuut longletsel.

De voorgestelde methode voor LPS-instillatie bij muizen werd geverifieerd door de expressie van het inflammatoire cytokine TNF-α en de longdroog-natgewichtsverhouding 12 uur na LPS-instillatie te evalueren. Ons protocol stelt een nieuwe niet-invasieve toedieningsmethode van LPS voor van de luchtpijp naar de longen, nauwkeuriger. Mensen die deze methode voor de eerste keer proberen, moeten veel moeite doen om de positie van de luchtpijp te vinden. Om het intubatieplatform voor te bereiden, passeert u het touw door de twee gaten en bindt u de uiteinden vast aan de kleine uitsteeksels aan de bovenkant van de riser.Bevestig vervolgens twee elastiekjes aan elk uiteinde van de paperclip en plak ze op de achterkant van de riser. Bevestig ten slotte de riser aan de basis in een hoek van 90 graden. Monteer de canule op een canulepen en doe het lampje van de pen aan.Desinfecteer vervolgens een kleine chirurgische tang en Pasteur-pipet met 70% alcohol. Om de oplossing van de teststof te bereiden, voegt u drie milligram lipopolysaccharide of LPS toe in één milliliter pH 7,2 PBS. Los vervolgens 10 milligram pentobarbital-natrium op in één milliliter normale zoutoplossing en filter de oplossing met een spuitfilter van 0,45 micrometer.Plaats de verdoofde muis op het intubatieplatform. Bevestig vervolgens de bovenste voortanden met het touwtje en de twee voorvoeten met de elastiekjes. Trek met een pincet de tong eruit en houd hem vast.Duw de canule langzaam langs de mond naar de mandibulaire epiglottis. Lokaliseer de luchtpijp met het licht van de canulepen en steek de canule er langzaam in. Trek vervolgens de intubatiepen terug en laat de canule binnen.Steek de Pasteurpipet in het canulegewricht en druk op de kop. Na succesvolle endotracheale intubatie, met behulp van een platte microsyringe, injecteert u de bereide LPS-oplossing met een dosis van drie milligram per kilogram via de canule. Als u klaar bent, haalt u de microspuit en canule eruit.Verwijder de muis van het intubatieplatform en plaats deze in een aparte kooi om te herstellen. Observeer de ademhalingstoestand van de muis totdat deze herstelt en weer bij bewustzijn komt. 12 uur na LPS-tracheale instillatie werden de expressie van een inflammatoir cytokine, TNF-alfa en de droog-natgewichtsverhouding van de long geëvalueerd en vergeleken met andere instillatiemethoden.De serum TNF-alfaspiegels in de niet-invasieve orofaryngeale intubatiegroep waren significant verhoogd in vergelijking met de blanco controlegroep. De long-droog-natgewichtsverhouding was ook verhoogd en bereikte hetzelfde niveau als in de chirurgische tracheale intubatiegroep. Het is belangrijk om de luchtpijp niet te krabben tijdens het vinden van de locatie van de luchtpijp.De techniek kan ook worden gebruikt om andere geneesmiddelen af te leveren die de longen op een niet-invasieve manier moeten bereiken.
