Привычный выкидыш (РМ) представляет собой потерю двух или более последовательных беременностей и является сложным заболеванием, привлекающим внимание клиницистов в последние годы. Частота РМ у женщин детородного возраста составляет 1%-5%1. Результаты предыдущих исследований показывают, что иммунные факторы тесно связаны с патогенезом РМ 2,3,4,5. Поддержание иммунного гомеостаза на границе между матерью и плодом необходимо для имплантации и развития эмбриона. Иммунные клетки эндометрия выполняют несколько регуляторных ролей для поддержания этого гомеостаза, таких как стимулирование инвазии трофобласта, ремоделирование спиральных артерий и содействие развитию плаценты 6,7,8,9.
Ранее сообщалось об аберрантных иммунных клетках эндометрия у женщин с РМ. Результаты показывают тесную связь между высокой плотностью маточных естественных киллеров (uNK) и возникновением RM10,11,12. По сравнению с обычными иммуногистохимическими экспериментами, количественный иммуногистохимический анализ иммунных клеток эндометрия имеет важное клиническое значение. Оценка интенсивности ИГХ обычно оценивается по четырехбалльной шкале или по сильной и слабой в патологической диагностике и исследованиях 19,20,21. Однако этот полуколичественный метод является субъективным, крайне неточным и демонстрирует значительную внутринаблюдательную и межнаблюдательную изменчивость22. Одним из возможных решений является применение машинного обучения, которое является ценным в цифровом анализе изображений23,24. Обеспечивая количественные измерения, этот подход позволяет более точно оценить инфильтрацию, распределение и плотность иммунных клеток в ткани матки. Эта количественная информация может помочь пролить свет на динамические изменения популяций иммунных клеток во время менструального цикла и при различных патологических состояниях. В целом, возможность количественного анализа иммунных клеток эндометрия с помощью иммуногистохимии дает ценную информацию об иммунном микроокружении матки.
Таким образом, протокол был направлен на разработку и валидацию цифровой платформы иммуногистохимического анализа изображений для количественного анализа иммунных клеток эндометрия, включая uNK-клетки, Tregs, макрофаги, ДК и цитотоксические Т-клетки во время средней лютеиновой фазы у пациентов с РМ.

Залитые парафином ткани эндометрия разделяли на предметные стекла толщиной 4 мкм, и проводили иммуногистохимическое (ИГХ) окрашивание для выявления иммунных клеток эндометрия, включая CD56+ мNK-клетки, Foxp3+ Tregs, CD163+ M2-макрофаги, CD1a+ DC и CD8+ Т-клетки. Панорамные слайды сканировались с помощью цифрового сканера слайдов, а для количественного анализа использовалась коммерческая система анализа изображений. Процентное содержание иммунных клеток эндометрия рассчитывали путем деления количества иммунных клеток в общем количестве клеток эндометрия. Используя коммерческую систему анализа изображений, можно легко и точно проанализировать количественную оценку иммунных клеток эндометрия, которые трудно или невозможно проанализировать с помощью обычного анализа изображений. Данная методика может быть применена для количественной характеристики микроокружения эндометрия, в том числе взаимодействия между иммунными клетками, и его гетерогенности у пациентов с различными репродуктивными пороками. Платформа для количественной оценки иммунных клеток эндометрия может иметь важное клиническое значение для диагностики и лечения пациенток с РМ.
