Erratum: Local Anesthetic Thoracoscopy for Undiagnosed Pleural Effusion

Forekomsten og forekomsten af pleurale sygdomme stiger over hele verden, især pleurale effusioner, som har mere end 50 anerkendte årsager 1,2. Pleural malignitet er den førende årsag til tilbagevendende pleural effusion, hovedsagelig på grund af metastaser fra ekstrapleurale lunge-, bryst- eller lymfommaligniteter3. Eksisterende retningslinjer anbefaler pleurale biopsier, hvis sygehistorie, fysisk undersøgelse, radiologi og pleural tap til cytologi, kultur og biokemi undlader at give en diagnose4. Pleurale biopsier kan opnås enten billedstyret eller under direkte syn. Ultralyd (US) eller CT-guidede perkutane kernebiopsier tillader prøveudtagning fra de fleste læsioner i den kystnære del af parietal pleura med et diagnostisk udbytte på henholdsvis 84% og 93% i en nylig systematisk gennemgang5. USA-vejledt pleurabiopsi er overlegen med hensyn til en lavere komplikationsrate (4% vs. 7%), ingen bestråling og tilgængelighed som sengeprocedure5. Caserapporter viser, at læsioner i mediastinal pleura kan udtages ved hjælp af ekkoendoskoper6.
Thorakoskopi tillader direkte visuel inspektion og prøveudtagning af læsioner i både mediastinal, membran eller costal pleura, hvilket gør det til guldstandarden i diagnosen tilbagevendende pleural effusion af en ukendt årsag 4,7. Thorakoskopi udføres enten som lokalbedøvende thorakoskopi (LAT) eller videoassisteret thorakoskopisk kirurgi (VATS)8. LAT (også kendt som medicinsk thorakoskopi eller pleuroskopi) er en enkeltportprocedure, rutinemæssigt udført af pulmonologer i bronkoskopi-suiten, og den bærer et diagnostisk udbytte for malignitet på 93%9,10. LAT kan udføres med enten et halvstift eller stift endoskop, normalt forbundet til en videobilledkilde. Nogle få publikationer diskuterer fordele og ulemper ved disse tilgange 8,11,12,13. Kort sagt ligner det halvstive thoracoscope et bronkoskop, og biopsier opnås ved anvendelse af lignende tang. Det stive endoskop har en større diameter, er billigere og giver mulighed for større biopsier, selvom dette ikke let oversættes til markante forskelle i diagnostisk udbytte 10,14,15. Sikkerheden ved LAT er høj med en dødelighed på mindre end 0,5%, med dette stærkt relateret til allerede eksisterende medicinske tilstande 3,4. Kontraindikationer af LAT er færre end for VATS og omfatter fuldstændig udslettelse af pleurarummet på grund af adhæsioner, hudinfektion på indgangsstedet, respirationssvigt, hjertestabilitet og ukorrekt koagulopati 7,8,12. VATS er en procedure på hospitalet, 2 eller 3 porte, der udføres af thoraxkirurger, som skal finde sted i et kirurgisk teater, der kræver generel anæstesi, intubation, enkeltlungeventilation og postoperativ indlæggelse. Moms har et diagnostisk udbytte og en komplikationsrate svarende til LAT, men gør det muligt at gennemføre mere komplekse procedurer10,16.
Retningslinjer anbefaler LAT som førstevalg thorakoskopi på grund af dets høje diagnostiske udbytte, lave risiko, lavere omkostninger og muligheden for dagsagsbehandling, idet moms forbeholdes udvalgte tilfælde4. Valget af et stift eller halvstift thorakoskop bestemmes normalt af lokal præference. LAT er ikke en ny procedure, da den blev beskrevet allerede i midten af det 19. århundrede, populariseret af Jacobaeus i 1910, i stigende grad brugt til behandling af pleural tuberkulose indtil 1950'erne og "genopdaget" i 1980'erne som et vigtigt redskab til at diagnosticere tilbagevendende pleurale effusioner 8,17.

local anesthetic thoracoscopy for undiagnosed pleural effusion

Lokalbedøvende thorakoskopi (LAT) er en minimalt invasiv diagnostisk procedure, der vinder anerkendelse blandt brystlæger til styring af udiagnosticerede pleurale effusioner. Denne enkeltportprocedure udføres med patienten under mild sedation og involverer en kontralateral decubitusposition. Det udføres i en steril indstilling, typisk en bronkoskopi suite eller kirurgisk teater, af en enkelt operatør med støtte fra en procedurefokuseret sygeplejerske og en patientfokuseret sygeplejerske.
Proceduren begynder med en thorax ultralyd for at bestemme det optimale indgangspunkt, normalt i IV-V interkostale rum langs den midaxillære linje. Lidokain / mepivacain, med eller uden adrenalin, bruges til at bedøve huden, thoraxvæglagene og parietal pleura. En udpeget trocar og kanyle indsættes gennem et 10 mm snit og når pleurhulen med forsigtig rotation. Thoracoscope indføres gennem kanylen til systematisk inspektion af pleurhulen fra toppen til membranen. Biopsier (typisk seks til ti) af mistænkelige parietale pleura læsioner opnås til histopatologisk evaluering og om nødvendigt mikrobiologisk analyse. Biopsier af den viscerale pleura undgås generelt på grund af risikoen for blødning eller luftlækager. Talkum poudrage kan udføres, før der indsættes et brystrør eller indlagt pleurakateter gennem kanylen. Hudsnittet sutureres, og intrapleural luft fjernes ved hjælp af et tre-rums eller digitalt brystafløbssystem. Brystrøret fjernes, når der ikke er nogen luftstrøm, og lungen er tilfredsstillende udvidet igen. Patienterne udskrives normalt efter 2-4 timers observation og følges op på ambulant basis. Vellykket LAT er afhængig af omhyggelig patientudvælgelse, forberedelse og styring samt operatøruddannelse for at sikre sikkerhed og et højt diagnostisk udbytte.

LAT kan udføres som en dag-sag procedure af brystlæger. Her præsenterer vi en trinvis tilgang til en vellykket og sikker procedure.

Til at begynde med skal du udføre en patientsundhedskontrol, der bekræfter hans samtykke og sundhedsmæssige forhold. Forbered og kontroller medicin såsom beroligende midler. Placer patienten i en lateral decubitus position på en pude, effusion side opad, og sørg for, at patientens vitale tegn overvågning er på plads.Med ultralydsvejledning skal du markere den forventede indgangsside på brystkassen med en stump indrykningsmarkør. Bekræft igen med ultralyd efter markering. Når udøveren har skrubbet og iført sig det passende sterile udstyr, skal du forberede og kontrollere udstyret og lokalbedøvelsen, som vil blive brugt i proceduren.Parallelt kan de tilberedte beroligende lægemidler administreres af en assistent. Desinficer indsættelsesstedet grundigt og opret et sterilt felt. Injicer derefter bedøvelsesopløsningen på indsættelsesstedet.Mens bedøvelsesmidlet træder i kraft, skal du forberede og kontrollere kameraet og andet udstyr. Lav derefter et 10 millimeter langt snit på huden. Udvid snittet med en kirurgisk saks.Efter dissekering ned et par millimeter, indsæt en afsluttende sutur. Med et par smalle spidstang dissekeres lagene forsigtigt mellem huden og pleura, indtil pleurhulen er indtastet. For nogle patienter kan det også være nødvendigt at bruge bredere tang.Bekræft om nødvendigt kanalens position ved hjælp af lokalbedøvende nål. Indsæt nu trokaren og kanylen ved hjælp af let tryk. Fjern derefter trokaren.Indsæt nu et sugerør via kanylen for at fjerne den resterende væske ved hjælp af et sugesystem. Indsæt thoracoscope via kanylen og inspicer pleurhulen, drej kikkertsigtet forsigtigt for at få et klart overblik over så bredt et område som muligt. Bemærk positionen, udseendet og bevægelsen af membranen, lungen og pleurale overflader.Hvis biopsier skal udføres ved hjælp af det stive omfang, skal du skifte til en nul-graders linse med biopsipincet fastgjort. Få pleurale biopsier fra mistænkelige områder ved at gribe lungehinden med tangen og forsigtigt skrælle lagene væk trinvist. Områder med nodularitet kan biopsieres ved hjælp af en mere traditionel pinch tilgang.Efter hver biopsi skal du fjerne omfanget og inspicere prøven for at sikre tilstrækkelighed. Når biopsier er færdige, inspicere pleurhulen for tegn på betydelig blødning. For at afslutte skal du fjerne trokaren og indsætte post-lat brystrøret i pleurrummet ved hjælp af portkanalen.Fastgør brystrøret ved hjælp af en hudsutur, og sørg for, at suturen holdes adskilt fra den afsluttende sutur. Pakk den afsluttende sutur med en bomuldsgasbind. Dæk derefter procedurestedet med tilstrækkelig polstring og dressing.
