Het volgende protocol beschrijft hoe LAT in een klinische setting moet worden uitgevoerd. Het protocol is in overeenstemming met de klinische praktijk en richtlijnen van de ziekenhuizen van de auteurs (Odense, Næstved, Lleida, Bristol en Preston). Voorafgaand aan de procedure wordt schriftelijke geïnformeerde toestemming van de patiënt verkregen. Erratum: Local Anesthetic Thoracoscopy for Undiagnosed Pleural Effusion

De incidentie en prevalentie van pleurale aandoeningen nemen wereldwijd toe, met name pleurale effusies, die meer dan 50 erkende oorzaken hebben 1,2. Pleurale maligniteit is de belangrijkste oorzaak van terugkerende pleurale effusie, meestal als gevolg van metastasen van extrapleurale long-, borst- of lymfoommaligniteiten. Bestaande richtlijnen bevelen pleurale biopsieën aan als de medische geschiedenis, lichamelijk onderzoek, radiologie en pleuratap voor cytologie, cultuur en biochemie geen diagnose kunnen stellen4. Pleurale biopsieën kunnen zowel beeldgestuurd als onder direct zicht worden verkregen. Echografie (VS) of CT-geleide percutane kernbiopten maken bemonstering mogelijk van de meeste laesies in het ribbengedeelte van het pariëtale borstvlies, met een diagnostisch rendement van respectievelijk 84% en 93%, in een recente systematische review5. VS-geleide pleurale biopsie is superieur in termen van een lager complicatiepercentage (4% versus 7%), geen bestraling en beschikbaarheid als procedure aan het bed5. Casusrapporten tonen aan dat laesies in het mediastinale borstvlies kunnen worden bemonsterd met behulp van echo-endoscopen6.
Thoracoscopie maakt directe visuele inspectie en bemonstering van laesies in zowel het mediastinale als het middenrif of het ribbenborstvlies mogelijk, waardoor het de gouden standaard is bij de diagnose van terugkerende pleurale effusie met een onbekende oorzaak 4,7. Thoracoscopie wordt uitgevoerd als een lokale anesthesiethoracoscopie (LAT) of video-geassisteerde thoracoscopische chirurgie (VATS)8. LAT (ook bekend als medische thoracoscopie of pleuroscopie) is een procedure met één poort, routinematig uitgevoerd door longartsen in de bronchoscopiesuite, en heeft een diagnostisch rendement voor maligniteit van 93%9,10. LAT kan worden uitgevoerd met een halfstijve of stijve endoscoop, meestal aangesloten op een videobeeldbron. In enkele publicaties worden de voor- en nadelen van deze benaderingen besproken 8,11,12,13. Kortom, de halfstijve thoracoscoop lijkt op een bronchoscoop en biopsieën worden verkregen met behulp van een vergelijkbare pincet. De stijve endoscoop heeft een grotere diameter, is goedkoper en maakt grotere biopsieën mogelijk, hoewel dit zich niet gemakkelijk vertaalt in duidelijke verschillen in diagnostische opbrengst 10,14,15. De veiligheid van LAT is hoog, met een sterftecijfer van minder dan 0,5%, waarbij dit sterk verband houdt met reeds bestaande medische aandoeningen 3,4. Contra-indicaties van LAT zijn minder dan voor VATS en omvatten volledige vernietiging van de pleurale ruimte als gevolg van verklevingen, huidinfectie op de plaats van binnenkomst, respiratoire insufficiëntie, hartinstabiliteit en niet-corrigeerbare coagulopathie 7,8,12. VATS is een procedure in het ziekenhuis, met 2 of 3 poorten, uitgevoerd door thoraxchirurgen, die moet plaatsvinden in een operatiekamer, waarvoor algemene anesthesie, intubatie, beademing met één long en postoperatieve opname nodig zijn. VATS heeft een diagnostisch rendement en complicatiepercentage dat vergelijkbaar is met dat van LAT, maar maakt het mogelijk om complexere procedures uit te voeren10,16.
Richtlijnen bevelen LAT aan als de eerste keuze thoracoscopie vanwege het hoge diagnostische rendement, het lage risico, de lagere kosten en de mogelijkheid voor dagcasemanagement, waarbij VATS wordt gereserveerd voor geselecteerde gevallen4. De keuze voor een stijve of halfstijve thoracoscoop wordt meestal bepaald door de lokale voorkeur. LAT is geen nieuwe procedure, zoals het al in het midden van de 19e eeuw werd beschreven, gepopulariseerd door Jacobaeus in 1910, in toenemende mate werd gebruikt om pleurale tuberculose te behandelen tot de jaren 1950, en in de jaren 1980 werd "herontdekt" als een belangrijk hulpmiddel om terugkerende pleurale effusies te diagnosticeren 8,17.

Lokale anesthesiethoracoscopie (LAT) is een minimaal invasieve diagnostische procedure die erkenning krijgt onder thoraxofoscopen voor het behandelen van niet-gediagnosticeerde pleurale effusies. Deze procedure met één poort wordt uitgevoerd met de patiënt onder milde sedatie en omvat een contralaterale decubituspositie. Het wordt uitgevoerd in een steriele omgeving, meestal een bronchoscopiesuite of operatiekamer, door een enkele operator met ondersteuning van een proceduregerichte verpleegkundige en een patiëntgerichte verpleegkundige.
De procedure begint met een thoracale echografie om het optimale toegangspunt te bepalen, meestal in de IV-V intercostale ruimte langs de midaxillaire lijn. Lidocaïne/mepivacaïne, met of zonder adrenaline, wordt gebruikt om de huid, thoracale wandlagen en pariëtale pleura te verdoven. Een speciale trocar en canule worden ingebracht via een incisie van 10 mm, waarbij de pleuraholte met zachte rotatie wordt bereikt. De thoracoscoop wordt via de canule ingebracht voor systematische inspectie van de pleuraholte van de top tot het middenrif. Biopten (meestal zes tot tien) van verdachte pariëtale pleura-laesies worden verkregen voor histopathologische evaluatie en, indien nodig, microbiologische analyse. Biopsieën van het viscerale borstvlies worden over het algemeen vermeden vanwege het risico op bloedingen of luchtlekken. Talkpoudrage kan worden uitgevoerd voordat een thoraxbuis of verblijfsplooikatheter via de canule wordt ingebracht. De huidincisie wordt gehecht en intrapleurale lucht wordt verwijderd met behulp van een drainagesysteem met drie compartimenten of een digitaal thoraxdrainagesysteem. De thoraxbuis wordt verwijderd zodra er geen luchtstroom meer is en de long naar tevredenheid is uitgezet. Patiënten worden meestal na 2-4 uur observatie ontslagen en poliklinisch opgevolgd. Succesvolle LAT is afhankelijk van zorgvuldige selectie, voorbereiding en beheer van patiënten, evenals opleiding van de operator, om de veiligheid en een hoog diagnostisch rendement te garanderen.

Voer om te beginnen een gezondheidscontrole van de patiënt uit, waarbij u zijn toestemming en gezondheidstoestand opnieuw bevestigt. Bereid en controleer eventuele medicijnen zoals kalmerende middelen. Plaats de patiënt in een laterale decubituspositie op een kussen, met de effusiezijde naar boven, en zorg ervoor dat de vitale functies van de patiënt worden gecontroleerd.Markeer met ultrasone geleiding de verwachte ingangszijde op de thorax met een stompe inkepingsmarkering. Bevestig opnieuw met echografie na markering. Zodra de arts de juiste steriele uitrusting heeft geschrobd en aangetrokken, bereidt en controleert u de apparatuur en de plaatselijke verdoving die bij de procedure zullen worden gebruikt.Tegelijkertijd kunnen de bereide kalmerende medicijnen worden toegediend door een assistent. Desinfecteer de inbrengplaats grondig en creëer een steriel veld. Injecteer vervolgens de verdovingsoplossing op de inbrengplaats.Terwijl de verdoving inwerkt, bereidt u de camera en andere apparatuur voor en controleert u deze. Maak vervolgens een incisie van 10 millimeter lang op de huid. Verbreed de incisie met een chirurgische schaar.Breng na het ontleden van een paar millimeter een afsluitende hechting in. Ontleed met een pincet met smalle punt voorzichtig de lagen tussen de huid en het borstvlies totdat de pleuraholte is binnengegaan. Voor sommige patiënten kan het ook nodig zijn om een bredere tang te gebruiken.Bevestig indien nodig de positie van het kanaal opnieuw met behulp van de plaatselijke anesthesienaald. Breng nu de trocar en de canule met lichte druk in. Verwijder vervolgens de trocar.Breng nu een zuigbuis via de canule om de resterende vloeistof te verwijderen met behulp van een zuigsysteem. Breng de thoracoscoop via de canule in en inspecteer de pleuraholte, waarbij u de scoop voorzichtig draait om een zo breed mogelijk gebied vrij te zien. Let op de positie, het uiterlijk en de beweging van het middenrif, de longen en de pleuraoppervlakken.Als biopsieën moeten worden uitgevoerd met behulp van de stijve scoop, schakel dan over op een lens van nul graden met een biopsietang bevestigd. Verkrijg pleurale biopsieën van verdachte gebieden door het borstvlies met de pincet vast te pakken en de lagen voorzichtig stapsgewijs weg te pellen. Gebieden met nodulariteit kunnen worden gebiopteerd met behulp van een meer traditionele knijpbenadering.Verwijder na elke biopsie de scoop en inspecteer het monster om er zeker van te zijn dat het geschikt is. Zodra de biopsieën zijn voltooid, inspecteert u de pleuraholte op tekenen van significante bloedingen. Om te eindigen, verwijdert u de trocar en steekt u de post-lat thoraxbuis in de pleurale ruimte met behulp van het poortkanaal.Zet de thoraxbuis vast met een huidhechting en zorg ervoor dat de hechting gescheiden wordt gehouden van de sluithechting. Wikkel de sluithechting in met een wattenstaafje. Bedek vervolgens de plaats van de procedure met voldoende vulling en verband.
