השכיחות והשכיחות של מחלות pleural עולה ברחבי העולם, במיוחד השתפכות pleural, אשר יש יותר מ 50 סיבות מוכרות 1,2. ממאירות פלאורלית היא הגורם המוביל להתפשטות פלאורלית חוזרת, בעיקר עקב גרורות מממאירויות ריאה, שד או לימפומה חוץ-פלאורליות3. ההנחיות הקיימות ממליצות על ביופסיות פלאורליות אם ההיסטוריה הרפואית, הבדיקה הגופנית, הרדיולוגיה והברז הפלאורלי לציטולוגיה, תרבית וביוכימיה אינם מספקים אבחנה4. ביופסיות פלאורליות ניתן לקבל או מונחה תמונה או תחת ראייה ישירה. ביופסיות ליבה מלעוריות מונחות אולטרסאונד (US) או CT מאפשרות דגימה מרוב הנגעים בחלק הקוסטלי של הצדר הקודקוד, עם תשואה אבחנתית של 84% ו -93%, בהתאמה, בסקירה שיטתית5 שנערכה לאחרונה. ביופסיה פלאורלית מונחית US עדיפה מבחינת שיעור סיבוכים נמוך יותר (4% לעומת 7%), ללא הקרנה, וזמינות כהליך ליד המיטה5. דיווחי מקרה מראים כי נגעים בצדר המדיאסטינלי ניתן לדגום באמצעות אקואנדוסקופים6.
תורקוסקופיה מאפשרת בדיקה חזותית ישירה ודגימה של נגעים הן בצדר המדיאסטינלי, הסרעפתי או הקוסטלי, מה שהופך אותו לתקן הזהב באבחון של השתפכות פלאורלית חוזרת של סיבה לא ידועה 4,7. תורקוסקופיה מבוצעת כתורקוסקופיה בהרדמה מקומית (LAT) או כניתוח תורקוסקופי בסיוע וידאו (VATS)8. LAT (הידוע גם בשם תורקוסקופיה רפואית או pleuroscopy) הוא הליך חד יציאה, המבוצע באופן שגרתי על ידי רופאי ריאות בחבילת הברונכוסקופיה, והוא נושא תשואה אבחנתית לממאירות של 93%9,10. VATS הוא הליך בבית החולים, 2 או 3 יציאות המבוצע על ידי מנתחי חזה, אשר חייב להתבצע בחדר כירורגי, הדורש הרדמה כללית, אינטובציה, הנשמה ריאה אחת, ואשפוז לאחר הניתוח. למע"מ יש תשואה אבחנתית ושיעור סיבוכים דומה לזה של LAT אך מאפשר לבצע הליכים מורכבים יותר10,16.
ההנחיות ממליצות על LAT כתורקוסקופיה המועדפת הראשונה בשל תשואת האבחון הגבוהה שלה, הסיכון הנמוך, העלויות הנמוכות יותר והאפשרות לניהול תיק יום, תוך שמירת מע"מ למקרים נבחרים4. הבחירה של תורקוסקופ קשיח או קשיח למחצה נקבעת בדרך כלל על ידי העדפה מקומית. LAT אינו הליך חדש, כפי שתואר כבר באמצע המאה ה -19, פופולרי על ידי Jacobaeus בשנת 1910, משמש יותר ויותר לטיפול בשחפת pleural עד 1950, ו "התגלה מחדש" בשנות ה -80 ככלי חשוב לאבחון השתפכות pleural חוזרת 8,17.

תורקוסקופיה בהרדמה מקומית (LAT) היא הליך אבחוני זעיר פולשני הזוכה להכרה בקרב רופאי חזה לניהול עירוי פלאורלי לא מאובחן. הליך חד יציאה זה מתבצע עם המטופל תחת טשטוש קל וכולל תנוחת דקוביטוס קונטרלטרלית. היא מבוצעת בסביבה סטרילית, בדרך כלל סוויטת ברונכוסקופיה או חדר ניתוח, על ידי מפעיל יחיד עם תמיכה של אחות ממוקדת הליך ואחות ממוקדת מטופל.
ההליך מתחיל באולטרסאונד חזה כדי לקבוע את נקודת הכניסה האופטימלית, בדרך כלל בחלל הבין-קוסטלי IV-V לאורך קו אמצע בית השחי. לידוקאין/מפיוואקאין, עם או בלי אדרנלין, משמש להרדים את העור, שכבות דופן בית החזה והצדר הקודקוד. טרוקאר וצינורית ייעודיים מוחדרים דרך חתך של 10 מ"מ, ומגיעים לחלל הפלאורלי בסיבוב עדין. בית החזה מוצג דרך הצינורית לבדיקה שיטתית של חלל הפלאורל מהקודקוד לסרעפת. ביופסיות (בדרך כלל שש עד עשר) של נגעים חשודים בצדר הקודקוד מתקבלות לצורך הערכה היסטופתולוגית, ובמידת הצורך, ניתוח מיקרוביולוגי. ביופסיות של הצדר הקרבי נמנעות בדרך כלל בשל הסיכון לדימום או דליפות אוויר. ניתן לבצע פולדראג טלק לפני החדרת צינורית חזה או החדרת קטטר פלאורלי דרך הצינורית. החתך בעור נתפר, והאוויר התוך-פלאורלי מוסר באמצעות מערכת ניקוז חזה בעלת שלושה תאים או דיגיטלית. צינור החזה מוסר ברגע שאין זרימת אוויר, והריאה התרחבה מחדש באופן משביע רצון. החולים בדרך כלל משוחררים לאחר 2-4 שעות של השגחה ומעקב על בסיס אשפוז. LAT מוצלח מסתמך על בחירה קפדנית של מטופלים, הכנה וניהול, כמו גם חינוך מפעילים, כדי להבטיח בטיחות ותשואת אבחון גבוהה.
