흉막 질환의 발병률과 유병률은 전 세계적으로 증가하고 있으며, 특히 흉막 삼출은 50가지 이상의 원인으로 알려져 있습니다 1,2. 흉막 악성 종양은 흉막 삼출액 재발의 주요 원인이며, 대부분 흉막외 폐, 유방 또는 림프종 악성 종양으로 인한 전이에 기인한다3. 기존 가이드라인은 병력, 신체검사, 방사선과, 세포학, 배양, 생화학 등을 위한 흉막 탭이 진단을 내리지 못할 경우 흉막 생검을 권장하고 있다4. 흉막 생검은 영상 유도 또는 직접 시력으로 얻을 수 있습니다. 초음파(US) 또는 CT 유도 경피적 코어 생검을 통해 두정흉막의 늑골 부분에 있는 대부분의 병변에서 검체를 채취할 수 있으며, 최근 체계적 문헌고찰에서 진단 수율은 각각 84%와 93%이다5. 미국 유도 흉막 생검은 합병증 발생률이 낮고(4% 대 7%), 방사선 조사가 필요 없으며, 병상 시술로 사용할 수 있다는 점에서 우수하다5. 증례 보고에 의하면 종격동 흉막의 병변은 초음파내시경을 사용하여 검체를 채취할 수 있다6.
흉강경 검사는 종격동, 횡격막 또는 늑골 흉막 모두에서 병변을 직접 육안으로 검사하고 샘플링할 수 있어 알 수 없는 원인의 재발성 흉막 삼출 진단에 있어 황금 표준이 됩니다 4,7. 흉강경 검사는 국소 마취 흉강경 검사(LAT) 또는 비디오 보조 흉강경 수술(VATS)8로 시행됩니다. LAT(의료용 흉강경 검사 또는 흉곽 내시경 검사라고도 함)는 기관지 내시경 검사실에서 호흡기 전문의가 일상적으로 수행하는 단일 포트 절차이며 악성 종양에 대한 진단 수율은 93%9,10입니다. VATS는 흉부외과 의사가 수행하는 병원 내 2포트 또는 3포트 시술로, 수술실에서 진행되어야 하며 전신 마취, 삽관, 단일 폐 환기 및 수술 후 입원이 필요합니다. VATS는 진단 수율과 합병증 발생률이 LAT와 비슷하지만 더 복잡한 절차를 수행할 수 있습니다10,16.
