Erratum: Local Anesthetic Thoracoscopy for Undiagnosed Pleural Effusion

Forekomsten og forekomsten av pleurale sykdommer øker over hele verden, spesielt pleural effusjoner, som har mer enn 50 anerkjente årsaker 1,2. Pleural malignitet er den viktigste årsaken til residiverende pleuravæske, hovedsakelig på grunn av metastaser fra ekstrapleurale lunge-, bryst- eller lymfommaligniteter3. Eksisterende retningslinjer anbefaler pleurabiopsier dersom sykehistorie, fysisk undersøkelse, radiologi og pleuratap for cytologi, kultur og biokjemi ikke gir diagnose4. Pleurabiopsier kan fås enten bildestyrt eller under direkte syn. Ultralyd (US)- eller CT-veiledede perkutane kjernebiopsier tillater prøvetaking fra de fleste lesjoner i den costale delen av parietal pleura, med et diagnostisk utbytte på henholdsvis 84 % og 93 % i en nylig systematisk oversikt5. USA-veiledet pleurabiopsi er overlegen når det gjelder lavere komplikasjonsrate (4% vs. 7%), ingen bestråling og tilgjengelighet som sengeprosedyre5. Kasuistikker viser at lesjoner i mediastinal pleura kan prøvetas med ekkoendoskop6.
Thorakoskopi tillater direkte visuell inspeksjon og prøvetaking av lesjoner i både mediastinal, diafragmatisk eller costal pleura, noe som gjør den til gullstandarden ved diagnostisering av tilbakevendende pleural effusjon av ukjent årsak 4,7. Thorakoskopi utføres enten som lokalbedøvende thorakoskopi (LAT) eller videoassistert torakoskopisk kirurgi (VATS)8. LAT (også kjent som medisinsk thorakoskopi eller pleuroskopi) er en enkeltportprosedyre, rutinemessig utført av pulmonologer i bronkoskopisuiten, og den har et diagnostisk utbytte for malignitet på 93%9,10. LAT kan utføres med enten et halvstivt eller stivt endoskop, vanligvis koblet til en videobildekilde. Noen få publikasjoner diskuterer fordeler og ulemper ved disse tilnærmingene 8,11,12,13. Kort fortalt ligner det semistive torakokkoskopet et bronkoskop, og biopsier oppnås ved bruk av lignende tang. Sikkerheten ved LAT er høy, med en dødelighet på mindre enn 0,5%, med dette sterkt relatert til eksisterende medisinske tilstander 3,4. Kontraindikasjoner av LAT er færre enn for merverdiavgift og inkluderer fullstendig utslettelse av pleurarommet på grunn av vedheft, hudinfeksjon på inngangsstedet, respirasjonssvikt, hjertestabilitet og ukorrigerbar koagulopati 7,8,12. VATS er en sykehus, 2 eller 3-port prosedyre utført av thoraxkirurger, som må foregå i en kirurgisk stue, som krever generell anestesi, intubasjon, single-lungeventilasjon og postoperativ opptak. Merverdiavgiften har en diagnostisk avkastning og komplikasjonsrate som ligner på LAT, men åpner for at mer komplekse prosedyrer kan gjennomføres10,16.
Retningslinjene anbefaler LAT som førstevalg thorakoskopi på grunn av høyt diagnostisk utbytte, lav risiko, lavere kostnader og mulighet for dagsaksbehandling, reservere merverdiavgift for utvalgte tilfeller4. Valget av et stivt eller halvstivt torakokkoskop bestemmes vanligvis av lokal preferanse. LAT er ikke en ny prosedyre, da den ble beskrevet så tidlig som på midten av 19-tallet, popularisert av Jacobaeus i 1910, i økende grad brukt til å behandle pleural tuberkulose til 1950-tallet, og "gjenoppdaget" på 1980-tallet som et viktig verktøy for å diagnostisere tilbakevendende pleural effusions 8,17.

local anesthetic thoracoscopy for undiagnosed pleural effusion

Lokalbedøvelse thoracoskopi (LAT) er en minimalt invasiv diagnostisk prosedyre som får anerkjennelse blant brystleger for å håndtere udiagnostiserte pleural effusjoner. Denne enkeltportprosedyren utføres med pasienten under mild sedasjon og involverer en kontralateral decubitusstilling. Det utføres i en steril setting, vanligvis en bronkoskopi suite eller kirurgisk teater, av en enkelt operatør med støtte fra en prosedyrefokusert sykepleier og en pasientfokusert sykepleier.
Prosedyren begynner med en thorax ultralyd for å bestemme det optimale inngangspunktet, vanligvis i IV-V interkostalrommet langs midaxillarlinjen. Lidokain / mepivacaine, med eller uten adrenalin, brukes til å bedøve huden, thoraxvegglagene og parietal pleura. En utpekt trokar og kanyle settes inn gjennom et 10 mm snitt, og når pleurahulen med forsiktig rotasjon. Thoracoscope føres gjennom kanylen for systematisk inspeksjon av pleurahulen fra apex til membran. Biopsier (typisk seks til ti) av mistenkelige parietale pleura-lesjoner oppnås for histopatologisk evaluering og, om nødvendig, mikrobiologisk analyse. Biopsier av visceral pleura unngås vanligvis på grunn av risikoen for blødning eller luftlekkasje. Talkpoudrage kan utføres før du setter inn et brystrør eller inneliggende pleurakateter gjennom kanylen. Hudinnsnittet sutureres, og intrapleural luft fjernes ved hjelp av et tre-rom eller digitalt brystdreneringssystem. Brystrøret fjernes når det ikke er luftstrøm, og lungen har utvidet seg tilfredsstillende. Pasientene skrives vanligvis ut etter 2-4 timers observasjon og følges opp poliklinisk. Vellykket LAT er avhengig av nøye pasientutvelgelse, forberedelse og ledelse, samt operatøropplæring, for å sikre sikkerhet og et høyt diagnostisk utbytte.

Lokalbedøvende thorakoskopi (LAT) er avgjørende for å diagnostisere tilbakevendende, udiagnostisert pleural effusjon når en retningslinjebasert opparbeidelse ikke gir en spesifikk årsak. LAT kan utføres som en dag-case-prosedyre av brystleger. Her presenterer vi en trinnvis tilnærming for en vellykket og sikker prosedyre.

For å begynne, utfør en pasienthelsekontroll, som bekrefter hans samtykke og helsemessige forhold. Forbered og sjekk eventuelle medisiner som beroligende midler. Plasser pasienten i en lateral decubitusstilling på en pute, effusjonssiden opp, og sørg for at overvåking av pasientens vitale tegn er på plass.Med ultralydveiledning markerer du forventet inngangsside på thorax med butt innrykksmarkør. Bekreft på nytt med ultralyd etter markering. Når utøveren har skrubbet og tatt på seg passende sterilt utstyr, klargjør og kontroller utstyret og lokalbedøvelsen som skal brukes i prosedyren.Parallelt kan de tilberedte beroligende medisinene administreres av en assistent. Desinfiser innsettingsstedet grundig og opprett et sterilt felt. Injiser deretter bedøvelsesoppløsningen på innføringsstedet.Mens bedøvelsen trer i kraft, klargjør og kontroller kameraet og annet utstyr. Deretter lager du et 10 millimeter langt snitt på huden. Utvid snittet med en kirurgisk saks.Etter å ha dissekert ned noen millimeter, sett inn en lukkende sutur. Med et par smalspisstang, dissekere lagene forsiktig mellom huden og pleura til pleurahulen er kommet inn. For noen pasienter kan det også være nødvendig å bruke bredere tang.Hvis nødvendig, bekreft plasseringen av kanalen ved hjelp av lokalbedøvelsesnålen. Sett nå inn trokaren og kanylen med forsiktig trykk. Fjern deretter trokaren.Sett nå inn et sugerør via kanylen for å fjerne gjenværende væske ved hjelp av et sugesystem. Sett torakokkoskopet inn via kanylen og inspiser pleurahulen, vri skopet forsiktig for å få et klart syn på et så bredt område som mulig. Legg merke til posisjonen, utseendet og bevegelsen av membranen, lungen og pleuraoverflatene.Hvis biopsier skal utføres med det stive kikkertsiktet, bytt til en nullgraders linse med biopsitang festet. Få pleural biopsier fra mistenkelige områder ved å gripe pleura med tang og forsiktig peeling lagene bort trinnvis. Områder med nodularitet kan biopseres ved hjelp av en mer tradisjonell klypetilnærming.Etter hver biopsi, fjern omfanget og inspiser prøven for å sikre tilstrekkelighet. Når biopsier er fullført, inspiser pleurahulen for tegn på betydelig blødning. For å fullføre, fjern trokaren og sett brystrøret etter lat inn i pleurarommet ved hjelp av portkanalen.Fest brystrøret med en hudsutur, og sørg for at suturen holdes atskilt fra lukkesuturen. Pakk den avsluttende suturen med en bomullsgasspute. Dekk deretter prosedyrestedet med tilstrekkelig polstring og dressing.
