In deze studie ontdekten we dat de therapie met een klein naaldmes de pijnsymptomen aanzienlijk verlichtte, de functie van het schoudergewricht verbeterde, de dikte van CHL verminderde en de kwaliteit van leven verbeterde bij patiënten met een bevroren schouder in stadium I-II. De therapie met het kleine naaldmes had een exact effect op frozen shoulder. De operatieplaats is echter controversieel. Frozen shoulder is een ziekte die wordt gekenmerkt door pijn en beperking van actieve en passieve schouderbewegingen, wat veel ongemak veroorzaakt voor het werk en het leven van patiënten1. De wereldwijde incidentie van frozen shoulder is 2%-5%2,3 en hoger bij vrouwen dan bij mannen, wat een bepaalde genetische neiging vertoont4. De meerderheid van de frozen shoulder patiënten is 40-60 jaar oud5. Bovendien brengt de frozen shoulder een zware sociale last met zich mee door de langere herstelcyclus en het ontbreken van een gevestigde behandelingsconsensus in de kliniekpraktijk.
De belangrijkste behandelingsmethoden voor frozen shoulders zijn onder meer oraal of uitwendig gebruik van kruidengeneeskunde, acupunctuur, massage, functionele oefeningen, enzovoort6; De therapietrouw van de patiënten is echter slecht vanwege de frequentie van meerdere behandelingen en de langere tijdrovendheid, en kan niet voldoen aan de dringende behoeften van patiënten om pijn en functionele beperkingen te verbeteren. Daarom is het dringend nodig om effectievere therapieën te identificeren voor de klinische behandeling van frozen shoulder-patiënten.
Kleinnaaldmestherapie is een bijzondere en effectieve uitwendige behandeling van de traditionele Chinese geneeskunde, die de afgelopen decennia snel is ontstaan op het gebied van de traditionele Chinese geneeskunde7. Het heeft de voordelen van een snel effect, een duidelijk genezend effect en een hoge herhaalbaarheid. Bovendien is het bedieningsinstrument goedkoop en gemakkelijk te verkrijgen, het bedieningsproces is eenvoudig en de methode wordt niet beperkt door de omgeving en de toestand. Het normale werk en leven van patiënten worden niet beïnvloed na de therapie met een klein naaldmes, aangezien de micro-incisie snel zal worden hersteld zonder schade aan het lichaam, infectie of andere bijwerkingen te veroorzaken 8,9,10,11. Systematische reviews toonden aan dat de werkzaamheid van een klein naaldmes op een frozen shoulder significant superieur was aan acupunctuur, massage en orale Chinese geneeskunde12,13. Moderne studies tonen aan dat het mechanisme van de behandeling van een klein naaldmes van een frozen shoulder is om de meridianen en collateralen te baggeren, de circulatie van qi en bloed te bevorderen, het laesieweefsel te snijden en te strippen, adhesie te verwijderen, weefselherstel te bevorderen en de dynamische balans van het menselijk lichaam te herstellen14,15.
Het hier gepresenteerde protocol demonstreert de werking van een klein naaldmes bij de behandeling van frozen shoulders, inclusief patiëntbeheer, materiaalvoorbereiding, positionering, operatie en postoperatieve zorg. Het doel van dit protocol is om de pijn en functionele beperkingen te verlichten en het leefvermogen van patiënten met frozen shoulder te verbeteren.
Met de ontwikkeling van de therapie met kleine naaldmessen is de toepassing ervan uitgebreid van de behandeling van nek-, schouder-, taille-, beenpijn en andere spier- en botziekten tot interne geneeskunde, chirurgie, gynaecologie, kindergeneeskunde en andere ziekten, en speelt een steeds belangrijkere rol op het gebied van revalidatiegeneeskunde16. In de afgelopen jaren vertoont het kleine naaldmes een nauwkeurigere, wetenschappelijkere en veiligere therapeutische werkzaamheid dankzij de toepassing van echogeleide visualisatietherapie voor kleine naaldmessen, waarvan de ontwikkeling een nieuwe fase is ingegaan 17,18.

clinical efficacy of small needle knife therapy on stage i-ii frozen shoulder

Frozen shoulder is een soort aseptische ontstekingsziekte van de schouder veroorzaakt door overbelasting, trauma en andere redenen, resulterend in pijn in het schoudergewricht en beperkte functie. Het hier gepresenteerde protocol demonstreert de werking van een klein naaldmes bij de behandeling van frozen shoulders, inclusief patiëntbeheer, materiaalvoorbereiding, positionering, operatie en postoperatieve zorg. Het doel van dit protocol is om de pijn en functionele beperkingen te verlichten en het leefvermogen van patiënten met frozen shoulder te verbeteren. In onze studie werden 76 stadium I-II frozen shoulder patiënten die aan de inclusiecriteria voldeden, willekeurig verdeeld in een controlegroep en een behandelingsgroep (n=38). Patiënten in de controlegroep kregen functionele oefeningen, terwijl de behandelingsgroep therapie met een klein naaldmes kreeg met functionele oefeningen. De visueel analoge scores (VAS), de Constant- en Murley-scores (CMS) en de dikte van het coracohumerale ligament (CHL) onder echografie werden geëvalueerd. Na therapie met een klein naaldmes was de VAS-score significant lager in de behandelingsgroep (5,11 ± 0,89) dan in de controlegroep (5,49 ± 0,65; t=-2,065, p<0,05); de CMS-score was significant hoger in de behandelgroep (64,72 ± 4,78) dan in de controlegroep (60,97 ± 6,00; t=2,947, blz<0,05); de CHL-dikte was significant afgenomen in de behandelingsgroep (2,38 ± 0,36) dan in de controlegroep (2,57 ± 0,42; t=-2,117, p<0,05). Deze resultaten geven aan dat het kleine naaldmes de pijnsymptomen aanzienlijk verlichtte, de schouderfunctie verbeterde, de CHL-dikte verminderde en de kwaliteit van leven verbeterde en daarom een significante therapeutische werkzaamheid had bij patiënten met een bevroren schouder in stadium I-II.

In de controlegroep kreeg 1 patiënt een andere behandeling (intra-articulaire steroïde-injectie) vanwege verergering van de pijn, dus werd de proef beëindigd en werden de overige 37 patiënten in de controlegroep opgenomen in de uiteindelijke statistische analyse. Voor 2 patiënten in de behandelingsgroep kon CHL niet worden gedetecteerd onder echografie, dus de test werd geëlimineerd en de resterende 36 patiënten in de behandelingsgroep gingen de laatste statistische analyse in.
