We report two cases in which patients on chronic hemodialysis presented with morbid unilateral edema of the upper extremity and chest on the side of a currently functioning arteriovenous access. Both patients were known to the vascular surgery service and had previously undergone multiple attempts to create and maintain vascular access. Both severe and disabling edema and the need to maintain dialysis access were of significant concern. These patients were taken to the operating room to address both issues. Upper extremity venography with central venous runoff revealed central vein stenosis and/or occlusion resistant to angioplasty and stenting. Ultimately, these two patients received the Hemodialysis Reliable Outflow vascular access device. The experiences in this study demonstrate the novel use of a relatively new vascular access device for salvage of a malfunctioning arteriovenous fistula or graft as well as treatment of symptoms and complications resulting from long standing vascular access.

Salvaging vascular access and treatment of severe limb edema: case reports on the novel use of the hemodialysis reliable outflow vascular access device.

Nous rapportons deux cas dans lequel les patients sous h&eacute;modialyse chronique pr&eacute;sent&eacute;e un oed&egrave;me unilat&eacute;ral morbid du membre sup&eacute;rieur et poitrine sur le c&ocirc;t&eacute; d&#39;un acc&egrave;s art&eacute;rioveineux fonctionnant actuellement. Les deux patients ont &eacute;t&eacute; connus du service de chirurgie vasculaire et avaient d&eacute;j&agrave; fait l&#39;objet des tentatives multiples pour cr&eacute;er et maintenir un acc&egrave;s vasculaire. Oed&egrave;me s&eacute;v&egrave;re et invalidant et la n&eacute;cessit&eacute; de maintenir l&#39;acc&egrave;s de dialyse &eacute;taient tous deux de graves pr&eacute;occupations. Ces patients ont &eacute;t&eacute; prises &agrave; la salle d&#39;op&eacute;ration pour aborder les deux questions. Phl&eacute;bographie des membres sup&eacute;rieurs avec &eacute;coulement veineux central a r&eacute;v&eacute;l&eacute; une st&eacute;nose veineuse centrale ou occlusion r&eacute;sistante &agrave; l&#39;angioplastie et le stenting. En fin de compte, ces deux patients ont re&ccedil;u le dispositif d&#39;acc&egrave;s vasculaire h&eacute;modialyse &eacute;coulement fiable. Wir berichten zwei F&auml;lle, in denen Patienten &uuml;ber chronische H&auml;modialyse mit morbid einseitiges &Ouml;dem der oberen Extremit&auml;t und Brust an der Seite eine aktuell funktionierende arterio-ven&ouml;sen Zugang dargestellt. Beide Patienten der Gef&auml;&szlig;chirurgie-Dienst bekannt waren und hatte zuvor mehrere Versuche zur Schaffung und Erhaltung von Gef&auml;&szlig;zugang unterzogen. Schwere und behindernden &Ouml;dem und der Notwendigkeit, Dialyse-Zugang wurden erhebliche Bedenken. Diese Patienten wurden op getroffen, die beiden Probleme. Obere Extremit&auml;t Phlebographie mit zentralen ven&ouml;sen Abfluss offenbart zentrale Vene Stenose bzw. Okklusion Angioplastie und Stenting. Diese zwei Patienten erhielten letztendlich das H&auml;modialyse zuverl&auml;ssigen Abfluss Gef&auml;&szlig;zugang Ger&auml;t. Die Erfahrungen in dieser Studie werden die neuartige Verwendung eines relativ neuen ZVK-Ger&auml;tes zur Bergung einer Fehlfunktion arterioven&ouml;se Fistel oder Transplantat als auch die Behandlung der Symptome und Komplikationen infolge langj&auml;hrigen ZVK.

Segnaliamo due casi in cui i pazienti su emodialisi cronica presentato con morbosa edema unilaterale dell&#39;arto superiore e torace sul lato un accesso arterovenoso attualmente funzionante. Entrambi i pazienti sono stati conosciuti per il servizio di chirurgia vascolare e in precedenza avevano subito numerosi tentativi di creare e mantenere l&#39;accesso vascolare. Sia edema grave e disabilitante e la necessit&agrave; di mantenere l&#39;accesso alla dialisi sono stati di notevole preoccupazione. Questi pazienti sono stati adottati per la sala operatoria per affrontare entrambe le questioni. Venografia arto superiore con deflusso venoso centrale ha rivelato la stenosi vena centrale e/o occlusione resistente all&#39;angioplastica e stenting. In definitiva, questi due pazienti ha ricevuto il dispositivo di accesso vascolare affidabile efflusso emodialisi. Le esperienze in questo studio dimostrano l&#39;uso di un dispositivo di accesso vascolare relativamente nuovo per il salvataggio di una fistola arterovenosa malfunzionante o innesto cos&igrave; come il trattamento dei sintomi e le complicazioni derivanti dall&#39;accesso vascolare a lungo in piedi romanzo.

Divulgamos dos casos en los que pacientes en hemodi&aacute;lisis cr&oacute;nica presentada con el edema unilateral morboso de la extremidad superior y el pecho en el lado de un acceso arteriovenoso actualmente en funcionamiento. Ambos pacientes eran conocidos por el servicio de cirug&iacute;a vascular y previamente hab&iacute;an sufrido varios intentos de crear y mantener el acceso vascular. Tanto el edema severo y discapacitante y la necesidad de mantener el acceso de di&aacute;lisis eran motivo de preocupaci&oacute;n importante. Estos pacientes fueron llevados al quir&oacute;fano para tratar ambos temas. Venograf&iacute;a de la extremidad superior con el escurrimiento venosa central revel&oacute; estenosis de la vena central y/o oclusi&oacute;n resistente a angioplastia y colocaci&oacute;n de stents. En definitiva, estos dos pacientes recibieron el dispositivo de acceso vascular de hemodialisis salida confiable. Las experiencias en este estudio demuestran el uso nuevo de un dispositivo de acceso vascular relativamente nuevo para el salvamento de un mal funcionamiento de la f&iacute;stula arteriovenosa o injerto as&iacute; como el tratamiento de los s&iacute;ntomas y las complicaciones resultantes de acceso vascular de larga data.

Titanium (Ti) is commonly utilized in many cardiovascular devices, e.g. as a component of Nitinol stents, intra- and extracorporeal mechanical circulatory assist devices, but is associated with the risk of thromboemboli formation. We propose to solve this problem by lining the Ti blood-contacting surfaces with autologous peripheral blood-derived late outgrowth endothelial progenitor cells (EPCs) after having previously demonstrated that these EPCs adhere to and grow on Ti under physiological shear stresses and functionally adapt to their environment under flow conditions ex vivo. Autologous fluorescently-labeled porcine EPCs were seeded at the point-of-care in the operating room onto Ti tubes for 30 min and implanted into the pro-thrombotic environment of the inferior vena cava of swine (n = 8). After 3 days, Ti tubes were explanted, disassembled, and the blood-contacting surface was imaged. A blinded analysis found all 4 cell-seeded implants to be free of clot, whereas 4 controls without EPCs were either entirely occluded or partially thrombosed. Pre-labeled EPCs had spread and were present on all 4 cell-seeded implants while no endothelial cells were observed on control implants. These results suggest that late outgrowth autologous EPCs represent a promising source of lining Ti implants to reduce thrombosis in vivo.

De titanio (Ti) se utiliza com&#xFA;nmente en muchos dispositivos cardiovasculares, por ejemplo, como un componente de los stents de Nitinol, circulatorio mec&#xE1;nico intra y extracorp&#xF3;rea dispositivos de asistencia, pero se asocia con el riesgo de formaci&#xF3;n trombo&#xE9;mbolos. Aut&#xF3;logo marcadas con fluorescencia EPC porcina fueron sembradas en el punto de atenci&#xF3;n en la sala de operaciones en Ti tubos durante 30 minutos y se implanta en el medio ambiente pro-tromb&#xF3;tico de la vena cava inferior de la especie porcina (n = 8). Despu&#xE9;s de 3 d&#xED;as, los tubos de Ti se explantaron, desmontado, y la superficie de contacto con la sangre fue fotografiada. Un an&#xE1;lisis ciego encontr&#xF3; los 4 cabezas de serie de c&#xE9;lulas implantes para estar libre de co&#xE1;gulos, mientras que los 4 controles sin EPC eran totalmente ocluido o parcialmente trombosado. Pre-etiquetados EPC se hab&#xED;a extendido y estaba presente en todos los cuatro cabeza de serie, los implantes de c&#xE9;lulas, mientras que no se observaron las c&#xE9;lulas endoteliales de los implantes de control.
