Iskæmi-reperfusionsskade (IRI) opstår, når der er en forbigående hypoxi på grund af obstruktion af blodstrøm (iskæmi) efterfulgt af en efterfølgende ny iltning af væv (reperfusion). I huden, er iskæmi-reperfusion (IR) menes at være en af ​​de medvirkende faktorer til patofysiologien af ​​tryksår, hvor langvarig sengeleje prædisponerer langsigtede hospitalspatienter til skade. Hos disse patienter er både hud og underliggende muskler konstant udsat for vægt presset i områder af knoglefremspring, hvilket resulterer i lokaliserede skader, hvis venstre ubehandlet, kan blive nekrotisk 1. De involverede i en IRI skader er dobbelt. Under iskæmi, okklusion af blodkar medfører et drastisk fald af oxygen levering til væv. Dette resulterer i et fald i ATP og pH, som inaktiverer ATPaser involveret i cellulær metabolisme. Til gengæld cellulære calcium niveauer spike, og stresset eller beskadiget calen undergår apoptose eller nekrose 2. Frigivelsen af intracellulære indhold eller skader i forbindelse molekylære mønstre (DAMP), ligesom HMGB1, bidrager til det inflammatoriske respons 3. Den anden insult optræder under reperfusion. Skønt oxygen og pH-niveauer er genoprettet under reperfusion, resulterer dette i genereringen af ​​reaktive oxygenspecies (ROS), som fører til oxidationen af ​​intracellulære lipider, DNA og proteiner. Derfor er pro-inflammatoriske mediatorer aktiveret, hvilket sætter en sekundær inflammatorisk reaktion, der involverer rekruttering af immunceller til det inflammatoriske sted 2. Mens den kaskade af biokemiske begivenheder, der fører op til det inflammatoriske respons er blevet godt beskrevet, er den rumlige og tidsmæssige regulering af immun celle aktiviteter ikke godt forstået. Her beskriver vi en robust IR model på museøre huden ved hjælp af simpel magnet fastspænding. Kombineret med multifoton intravital imaging (MP-IVM), vietableret en model til at studere in vivo inflammatoriske responser, der opstår efter reperfusion finder sted. Rationalet bag udviklingen og brugen af ​​denne teknik er at forsøge at forstå, hvordan både interstitielle og infiltrerende celler reagerer på IR i realtid. Eksisterende modeller af IR ved hjælp af fastspænding teknik på huden flanke er meget invasiv, da de kræver kirurgisk implantation af stålplader i huden flanke, hvilket gør dem mindre end ideelle til immunologiske undersøgelser 4. Et lignende ikke-invasiv fastspænding teknik er blevet beskrevet i musehud flanke 5,6. Men på grund af indarbejdelsen af intravital imaging komponent i denne metode, vi valgte i stedet, øret huden som den målrettede IR site, da det omgår bevægelser grundet vejrtrækning og tilbyder stabilitet under billeddannelse 7,8. Endvidere leukocytundergrupper der spænder interstitium er identiske mellem øret huden og huden flanke, selv omtal og proportioner kan variere lidt 9. Desuden har de fleste data hentet fra disse IRI modeller er begrænset til makroskopiske evalueringer (sortering af sår) og mikroskopiske analyser af endpoint inflammatoriske indikatorer 10. inducing ischemia-reperfusion injury in the mouse ear skin for intravital multiphoton imaging of immune responses

Ischemia-reperfusion injury (IRI) occurs when there is transient hypoxia due to the obstruction of blood flow (ischemia) followed by a subsequent re-oxygenation of the tissues (reperfusion). In the skin, ischemia-reperfusion (IR) is the main contributing factor to the pathophysiology of pressure ulcers. While the cascade of events leading up to the inflammatory response has been well studied, the spatial and temporal responses of the different subsets of immune cells to an IR injury are not well understood. Existing models of IR using the clamping technique on the skin flank are highly invasive and unsuitable for studying immune responses to injury, while similar non-invasive magnet clamping studies in the skin flank are less-than-ideal for intravital imaging studies. In this protocol, we describe a robust model of non-invasive IR developed on mouse ear skin, where we aim to visualize in real-time the cellular response of immune cells after reperfusion via multiphoton intravital imaging (MP-IVM).
