Kwantificering van vóór en na vaccinatie influenza virus specifieke antilichamen titers is een belangrijk instrument voor vaccin studies nodig. Op basis van de surrogaat maatregelen ter bescherming tegen virusbesmetting, zoals seroprotection (> 1:40) of seroconversie (4-fold titer toename), vaccinatie strategieën kunnen worden geoptimaliseerd9. Infectie met influenzavirus wordt geassocieerd met aanzienlijke morbiditeit, mortaliteit en hoge kosten van de gezondheidszorg1,,2,,3,4. Met name lopen ouderen, pasgeborenen, zwangere vrouwen en patiënten met chronische ziekte risico voor meer ernstige klinische resultaten. Daarom is vaccinatie tegen circulerende influenzastammen virus de primaire maatregel tot vermindering van de ziektelast in deze risicovolle populaties. De toename van de individuele immune reactie na vaccinatie, bijvoorbeeld, griep-specifieke antilichamen boven een beschermende drempel vermindert het individuele risico van infectie en in het algemeen de kans op virale overdracht binnen een populatie 5. een gedetailleerde kennis van de vaccin-geïnduceerde humorale immune reactie in verschillende bevolkingsgroepen en in heel verschillende leeftijdsgroepen is een sleutelelement om te beantwoorden van belangrijke klinische vragen6,7,8 , 9, zoals: Waarom hebben sommige oudere patiënten infecties ondanks eerdere vaccinatie? Wat is een "goede" en "voldoende" vaccin-geïnduceerde bescherming? Hoe vaak moet een vaccin worden toegepast op een patiënt immunosuppressie beschermende titers bereikt? Wat is de meest effectieve dosering? Wat is de impact van een roman adjuvans op na vaccinatie antilichamen titers? De meting van de productie van het vaccin-specifieke antilichaam kan helpen deze belangrijke vragen te beantwoorden en vaccinatie resultaten te verbeteren.
De kwantificering van virus-specifieke antilichamen titers kan worden uitgevoerd met diverse immunologische methoden. Het gaat hierbij om solid-phase10 of kraal gebaseerde ELISA11 testen, HI assay12en neutraliseren assays13. ELISA gebaseerde methoden toestaan de screening van relatief grote hoeveelheden serummonsters tegen de verschillende antigenen. Ook pathogeen-specifieke Immunoglobulin (Ig) M en IgG kan worden afzonderlijk onderzocht. Hoewel de kenmerken van een antigeen, bijvoorbeeld, de lineaire aminozuur sequentie of virusachtige deeltjes invloed op de binding van antilichamen hebben kunnen, het spectrum van potentiële epitopes is zeer breed en geeft informatie niet op, of een antilichaam reactie heeft functionele relevantie.
Daarentegen de neutralisatie bepaling bepaalt het potentieel van antilichamen voor de functioneel remming van de infectie van cellen en dus weerspiegelt de potentiële neutralisatie. Echter, deze methode is zeer arbeidsintensief, vereist kweken van specifieke cellijnen en leven van virussen, en daarom, het is tijdrovend, duur, en vereist speciale apparatuur.
Dit artikel beschrijft een stapsgewijze de Wereldgezondheidsorganisatie WHO gebaseerde HI protocol12 om te kwantificeren van influenza-specifieke antilichamen titers. Hemagglutination is een karakteristiek effect van sommige virussen die leiden tot de agglutinatie van erytrocyten. De remming van dit effect met patiënt sera kunt de meting van remmende antilichaam concentraties, waarin een neutraliserende werking.
We hebben de workflow van het WHO-protocol om een meer efficiënte afhandeling van meerdere monsters tegelijk en waardoor de vereiste tijd gewijzigd. Het eerste protocol beschrijft de bepaling van het potentieel van de agglutinatie-eenheden van een bepaald influenza-antigeen. Daarbij wordt de juiste influenza-antigeen-concentratie bepaald voor het tweede protocol. Dit deel moet worden herhaald met elke nieuwe virale antigeen, evenals elke batch van het bloed.
Het tweede protocol beschrijft de bepaling van influenza-specifieke antilichamen titers. De gepresenteerde protocollen zijn geoptimaliseerd voor het onderzoek van het influenzavirus en menselijk serummonsters echter, het kan ook worden toegepast voor muis serummonsters of celkweek supernatant van gestimuleerd immune cellen, bijvoorbeeldvirus-specifieke B-cellen. Resultaten kunnen worden bepaald als absolute gemeten titers. In vele studies van het vaccin, worden de titers meetkundige gemiddelde en het 95%-betrouwbaarheidsinterval weergegeven voor elke bijzondere bevolking. Voor de interpretatie, de seroprotection of de seroconversie worden vaak gebruikt om te beschrijven de gevoeligheid van een bevolking met een bepaalde virus. Seroprotection is gedefinieerd als een titer van ≥1:40 en seroconversie zoals een titer van meer dan 4-fold te verhogen met de verwezenlijking van seroprotective titers tussen twee tijdstippen (meest meestal vóór vaccinatie en 30 dagen na vaccinatie worden gebruikt).
Beide protocollen zijn makkelijk te gebruiken en ze kunnen worden aangepast aan een brede waaier van onderzoeksvragen. In het bijzonder, ze kunnen worden gebruikt om te bepalen betrouwbaar en snel de titers antilichamen tegen diverse andere virussen met de capaciteit voor hemagglutination, zoals mazelen, polyomaviruses, bof of rodehond14,15,16 .

an optimized hemagglutination inhibition (hi) assay to quantify influenza-specific antibody titers

Antilichamen titers worden vaak gebruikt als surrogaat markers voor serologische bescherming tegen influenza en andere ziekteverwekkers. Gedetailleerde kennis van de productie van het antilichaam pre-en post vaccinatie is vereist voor het begrijpen van de vaccin-geïnduceerde immuniteit. Dit artikel beschrijft een betrouwbaar punt-voor-punt protocol om te bepalen van influenza-specifieke antilichamen titers. Het eerste protocol beschrijft een methode om op te geven van de bedragen van de antigeen vereist voor hemagglutination, die standaardiseert de factor van de concentraties voor latere gebruik in het tweede protocol (hemagglutination assay, HA assay). Het tweede protocol beschrijft de kwantificering van influenza-specifieke antilichamen titers tegen verschillende virale stammen met behulp van een seriële verdunning van menselijk serum of cel cultuur supernatant (hemagglutination inhibitie assay, HI assay).
Als een toegepaste voorbeeld tonen wij de antistofrespons van een gezonde cohort, waaraan een driewaardig geïnactiveerd influenzavaccin aan te leggen. Bovendien, de Kruisallergie tussen de verschillende influenzavirussen wordt weergegeven en methoden om te minimaliseren Kruisallergie met behulp van verschillende soorten dierlijke rode bloedcellen (RBC) worden uitgelegd. De discussie benadrukt de voordelen en nadelen van de gepresenteerde tests en hoe de bepaling van influenza-specifieke antilichamen titers kan de verbetering van het inzicht van de vaccin-gerelateerde immuniteit.

Pre-en post vaccinatie geïnduceerde antistofrespons tegen Influenza A H3N2 De vaccin-geïnduceerde antilichaamrespons in 26 gezonde vrijwilligers die een geïnactiveerd driewaardig subeenheid influenzavaccin aan te leggen met Influenza A/H1N1/Californië/2009, A/H3N2/Texas/2012 en B/Massachusetts/02/2012 voorafgaand aan de 2014 ontvangen evalueerden / 2015 griep seizoen.
