Kvantifisering av pre- og etter vaksinasjon influensa virus spesifikt antistoff titers er viktig for nødvendig for vaksine studier. Basert på surrogat tiltak mot virusinfeksjon, for eksempel seroprotection (> 1:40) eller serokonversjon (4-fold titer økning), vaksinasjon strategier kan være optimalisert9. Influensa-viruset er assosiert med betydelig sykelighet, dødelighet og høy helsetilbud koster1,2,3,4. Spesielt er eldre, nyfødte, gravide og pasienter med kronisk sykdom utsatt for mer alvorlige kliniske utfall. Vaksinasjon mot sirkulerende influensa viruset stammer er derfor det primære mål å redusere byrden av sykdom i disse høyrisikogrupper. Økningen av personlige immunrespons etter vaksinasjon, f.eks, influensa-spesifikke antistoffer over en beskyttende terskel, reduserer den personlige risikoen av infeksjon og generelt sannsynligheten for viral overføring innen en befolkning 5. en detaljert forståelse av vaksine-indusert humoral immunrespons i ulike befolkningsgrupper og ulike aldersgrupper er nøkkelen å besvare viktige klinisk spørsmål6,7,8 , 9, som: Hvorfor har noen eldre pasienter infeksjoner til tross for tidligere vaksinasjon? Hva er en "god" og "tilstrekkelig" vaksine-indusert beskyttelse? Hvor ofte bør en vaksine brukes til et svekket immunapparat pasient å nå beskyttende titers? Hva er den mest effektive dosen? Hva er virkningen av en roman adjuvant på etter vaksinasjon antistoff titers? Måling av vaksine-spesifikke antistoffproduksjon kan hjelpe svare på disse viktige spørsmålene og bedre vaksinasjon resultater.
Kvantifisering av virus-spesifikke antistoffer titers kan utføres med ulike immunologiske metoder. Dette inkluderer solid-fase10 eller perle-baserte ELISA11 analyser, HI analysen12og nøytraliserende analyser13. ELISA-baserte metoder gjør screening av relativt store mengder serumprøver mot ulike antigener. Også utforskes patogen-spesifikke immunglobulin (Ig) M og IgG separat. Selv om egenskapene til et antigen, f.eks, lineær aminosyresekvens eller virus-lignende partikler kan påvirke bindingen av antistoffer, spekteret av potensielle epitopes er veldig bredt og gir ikke informasjon på om et antistoff svaret har funksjonelle relevans.
Derimot nøytralisering analysen bestemmer potensialet av antistoffer å funksjonelt hemme infeksjon av celler og gjenspeiler nøytralisering potensielle. Men denne metoden er svært arbeidsintensiv, krever dyrking av cellelinjer og live virus, og derfor det er tidkrevende, dyrt og krever spesialutstyr.
Denne artikkelen beskriver en trinnvis Verdens helseorganisasjon WHO-baserte HI protokoll12 for å kvantifisere influensa-spesifikke antistoffer titers. Hemagglutination er en karakteristisk effekt av noen virus fører til agglutinerende av erytrocytter. Hemming av denne effekten med pasienten sera lar målingen for hemmende antistoff, som gjenspeiler en nøytraliserende virkning.
Vi har endret arbeidsflyten av WHO-protokollen for å tillate en mer effektiv håndtering av flere eksempler samtidig og dermed redusere tidsnok. Første protokollen beskriver fastsettelse av agglutinerende potensialet i en bestemt influensa antigen. Dermed bestemmes riktig influensa antigen konsentrasjon for andre protokollen. Denne delen skal gjentas med hver nye viral antigen, samt hvert parti av blod.
Andre protokollen beskriver fastsettelse av influensa-spesifikke antistoffer titers. Presentert protokollene er optimalisert for etterforskningen av influensavirus og koagulasjon men det kan også brukes for musen serumprøver eller celle-kultur supernatants fra stimulert immunceller, f.eksvirus-spesifikke B-celler. Resultatene kan beregnes som absolutt målt titers. I mange vaksine studier vises de geometriske gjennomsnittet titers og 95% konfidensintervall for hver bestemt populasjon. For tolkning, seroprotection eller serokonversjon brukes ofte til å beskrive mottakelighet for en populasjon visse virus. Seroprotection er definert som en titer ≥1:40 og serokonversjon som en mer enn 4-fold titer øker med oppnåelse av seroprotective titers mellom to tidspunkt (mest brukte pre-vaksinasjon og 30 dager etter vaksinasjon brukes).
Begge protokollene er enkel å bruke og de kan tilpasses et bredt spekter av problemstillinger. Spesielt de kan brukes til å bestemme raskt og pålitelig antistoff titers mot ulike andre virus med kapasitet på hemagglutination, som meslinger, polyomaviruses, kusma eller rubella14,15,16 .

an optimized hemagglutination inhibition (hi) assay to quantify influenza-specific antibody titers

Antistoff titers brukes ofte som surrogat markører for serologisk beskyttelse mot influensa og andre patogener. Detaljert kunnskap om antistoffproduksjon før og etter vaksinasjon er nødvendig å forstå vaksine-indusert immunitet. Denne artikkelen beskriver en pålitelig punkt-til-punkt-protokoll for å avgjøre influensa-spesifikke antistoffer titers. Første protokollen beskriver en metode for å angi antigen beløpene kreves for hemagglutination, som standardiserer konsentrasjonen for senere bruk i andre protokollen (hemagglutination analysen, HA analysen). Andre protokollen beskriver kvantifisering av influensa-spesifikke antistoffer titers mot ulike viral stammer ved hjelp av en seriell utvanning av menneskelige serum- eller celle kultur supernatants (hemagglutination hemming analysen, HI analysen).
Anvendt eksempel viser vi antistoff responsen på en sunn kohort, som fikk en trivalent deaktivert influensavaksine. I tillegg vises det kryssreaktivitet mellom forskjellige influensavirus og metoder for å redusere kryssreaktivitet ved å bruke ulike typer dyr røde blodlegemer (RBCs) er forklart. Diskusjonen fremhever fordelene og ulempene med de presenterte analyser og hvordan fastsettelse av influensa-spesifikke antistoffer titers kan forbedre forståelsen av vaksine-relatert immunitet.

Før og etter vaksinasjon indusert antistoffet svaret mot influensa A H3N2 Vaksine-indusert antistoffet svaret ble vurdert i 26 friske frivillige som fikk en deaktivert trivalent delenhet influensavaksine inneholder influensa A/H1N1/California/2009, A/H3N2/Texas/2012 og B/Massachusetts/02/2012 før 2014 / 2015 influensa sesongen.
