长段气管缺损可表现为罕见的先天性疾病, 如完整的气管环和气管形成, 以及创伤, 恶性肿瘤和感染。成人超过6厘米或儿童气管长度的30% 时, 这些缺陷不能通过手术重建来治疗。试图用自体组织、尸体移植和人工结构取代气道的努力一直受到慢性感染、造粒、机械衰竭和狭窄的困扰。

由于移植狭窄和延迟上皮化，组织工程气管的临床应用受到限制。正向气管置换的小鼠模型允许研究驱动再生的细胞机制。气道的长段缺陷可以超过目前疗法能够治愈的。组织工程气管提供这些条件的替代器官的潜力。该模型可应用于具有转基因和血统追踪特性的动物。然后，我们可以调节导致移植再生的宿主因子。生产脚手架的主要挑战之一是测量和确定每次电击参数是否相同。因此，每次生产脚手架时，都需要测量从收集器的距离、湿度和温度，以确保脚手架每次都相同。该模型成功的关键是改进显微外科技术和维护正确的麻醉平面，以允许在整个过程中自发通风。由于主气管的内径约为一毫米，移植物和原生组织处理和缝合放置非常重要，在视觉上也表现出最佳表现。首先在六氟异丙醇中溶解 8 重量百分比聚乙烯四硫酸盐或 PET，然后将所得溶液加热至 60 摄氏度。在加热溶液时，在六氟异丙醇中溶解三重百分比聚氨酯或聚氨酯，一旦PET溶液冷却，结合溶液。用聚合物混合物装载装有 20 量钝尖针的 60 毫升注射器。使用注射器和高压直流电源设置为针上正 14 千伏，另一个高压直流电源设置为负 3 千伏，每小时 5 毫升的流速，以及 20 厘米的尖端到基板距离，将聚合物电镀到直径为 1 毫米的不锈钢棒上，以每分钟 350 次旋转的速度转动。继续电击，直到达到 300 微米脚手架壁厚。然后，将脚手架从杆上滑下来，将脚手架在真空下放置一夜，以清除任何残留溶剂，然后用每厘米350毫焦耳的紫外光剂量对脚手架进行消毒，约15分钟。在无菌条件下，用细剪刀和微副钳从六至八周大的雌性、C57-Black/6小鼠的后肢上去除皮肤，并使用杜蒙五号钳子和杜蒙号5/45钳子去除筋膜和肌腱。分离骨骼，让每个骨骼在两端被切割，并使用装有 25 量针的 5 毫升注射器将骨髓冲洗到含有 30 毫升 RPMI 介质的培养皿中。当所有骨骼都冲刷后，通过70微米尼龙细胞过滤器将骨髓过滤到50毫升的锥形管中，然后将管子转移到生物安全柜。轻轻地将过滤过的单核细胞溶液加入到含有5毫升多糖和三酸钠的15毫升管的一侧，无需混合层，通过密度梯度分离将血浆与白血球和红血球分离。离心结束时，收集由骨髓单核细胞组成的中间透明层，通过离心以PBS的一比一的比例清洗骨髓单核细胞。将颗粒重新用五毫升 PBS 重新洗涤，然后重新用 10 毫升新鲜 RPMI 介质进行计数。然后，收集细胞与额外的离心，并稀释细胞到1倍10到7细胞每5微升新鲜RPMI浓度。在播种脚手架之前，必要时将聚合物切割至五毫米长，用五微升的 RPMI 介质将脚手架淋湿五分钟。在孵育结束时，去除多余的介质，用五微升骨髓单核细胞溶液加载脚手架流明10分钟。然后，在脚手架的流明中插入一根 21 量针，并将移植物在 37 摄氏度的培养箱中过夜，将嫁接物放在一毫升 RPMI 介质中。诱导全身麻醉后，确认接受动物对手趾捏缺乏反应，并给动物的眼睛涂抹软膏。将手术场地上的头发从下巴夹到夹骨，将动物放在手术垫上，放在背背仰卧位置。利用无菌技术，连续用波维酮碘、70%乙醇、波维酮-碘擦拭系列对手术现场进行消毒，并在12点用头部在解剖显微镜下移动小鼠。使用细剪刀和微Adson钳子将中线切口从夹骨切到葫血骨，并使用杜蒙五号和七号细钳和无菌棉签清洁筋膜，然后再将自固定大肠杆菌缩回器插入切口。使用钳子打开带状肌肉，露出甲状腺软骨、软骨和气管，并钝化地将气管与组织两侧平行的复发喉神经分离，然后将气管与食道的周长分离。使用 20 量针和手术标记，染色气管的前部部分。使用一对 Vannas-Tubingen 弹簧剪刀和一双 Dumont 7 号细钳在第三气管软骨环下方切开一个切口，以切除气管的三环部分。用细弯曲钳握住气管，使用无菌的 9-0 尼龙缝合线将气管的端部固定到胸骨，以创建临时气管切除术，并使用 20 量针和手术标记来染色移植物的前部。要植入移植物，请使用9-0无菌尼龙缝合线、Dumont号7号细钳和针架插入近侧后、近侧和近前缝线，并将动物转180度。当小鼠就位时，释放临时气管切除术，并在离移植物前部五毫米远的地方做侧切口，以方便远侧肛门切除术。要完成近侧心肺病，请以与近近性解剖学相似的方式放置缝合线，并重新近似移植位置和带状肌肉。然后，以运行模式用 9-0 无菌尼龙缝合线关闭切口，将动物单独放在放在加热垫上的恢复笼中，并进行监控，直到完全恢复。植入两周后，在组织工程气管移植组织部分，基底细胞角蛋白5和14染色可以观察到一些基底上皮细胞。在植入后七天内，在移植物的发光表面也检测到基础细胞。经过移植和组织学分析，移植狭窄被确定为移植植入小鼠的主要促成因素，表现出呼吸窘迫和呼吸困难的迹象。请注意，由于手术技术，移植物和原生气管的伸缩是一个常见发现。F4/80 的染色揭示了移植物的狭窄区域内的宿主巨噬细胞，以及基底上皮细胞的存在。避免操纵复发性喉神经，并确保近近和近线缝合到位，以允许气密密封在肛门病。由于该协议涉及来自生物安全一级代理小鼠的骨髓单核细胞，因此必须穿戴适当的个人防护设备以避免污染。
