組織設計気管の臨床応用は、移植片狭窄および上皮化の遅れのために限定されている。直交性気管置換のマウスモデルは、再生を駆動する細胞機構の研究を可能にする。気道の長いセグメントの欠陥は、現在の治療法が治癒することができるものを超えることができます。組織で設計された気管は、これらの条件の代替臓器を作成する可能性を提供します。このモデルは、トランスジェニックおよび系統トレースプロパティを持つ動物に適用できます。その後、移植片の再生に寄与する宿主因子を調節することができます。スキャフォールドを製造する上での重要な課題の1つは、エレクトロスピニングパラメータが毎回同じであることを測定し、決定することです。したがって、スキャフォールドを生成するたびに、先端からコレクターまでの距離、湿度、温度を測定して、スキャフォールドが毎回同じになるようにする必要があります。このモデルの成功にとって重要なのは、手術を通して自発的な換気を可能にする、マイクロサージカル技術の改良と麻酔の正しい平面の維持である。宿主気管は約1ミリメートルの内径を有するため、移植片およびネイティブ組織の取り扱いと縫合の配置が重要であり、視覚的に最もよく実証される。まず、8重量%のポリエチレンテレフタレート(PET)を六フッ化イソプロパノールに溶解し、得られた溶液を摂氏60度に加熱します。溶液が温められている間、3重量%ポリウレタン、またはPUを六フッ化イソプロパノールに溶解し、PET溶液が冷却されたら溶液を組み合わせる。ポリマー混合物と20ゲージの鈍い先端の針が装備されている60ミリリットルのシリンジをロードする。注射器と高電圧DC電源を針の正の14キロボルトに設定し、別の高圧DC電源をマイナス3キロボルトに設定し、5ミリメートル/時の流量を設定し、ポリマーをエレクトロスピンする20センチメートルの先端から基板までの距離を使用して、1ミリメートルの直径のステンレス鋼棒を1ミリメートル回転に変えます。300マイクロメートルの足場壁の厚さが達成されるまでエレクトロスピニングを続けます。次に、足場をロッドからスライドさせ、残存溶媒を除去するために一晩真空下に足場を置き、約15分間2乗/センチメートル350ミリジュールの紫外線投薬量で足場を殺菌する。無菌状態では、6〜8週齢の女性、C57-Black/6マウスの後肢から細かいはさみとマイクロAdson鉗子を取り除き、デュモンナンバー5鉗子とデュモン番号5/45鉗子を使用して筋膜と腱を取り除きます。骨を分離して両端で切断し、25ゲージの針を備えた5ミリリットルの注射器を使用して、骨髄を30ミリリットルのRPMI培地を含むペトリ皿に洗い流します。すべての骨が洗い流されたら、骨髄を70マイクロメートルのナイロンセルストレーナーを通して50ミリリットルの円錐管にフィルターし、チューブをバイオセーフティキャビネットに移します。濾過された単核細胞溶液を、ポリスクロースと珪酸ナトリウム5ミリリットルを含む15ミリリットルのチューブの側面に、層を混合することなく静かに加え、密度勾配分離によって白血球と赤血球から血漿を分離する。遠心分離の終わりに、骨髄単核細胞からなる中間透明層を採取し、遠心分離によりPBSで骨髄単核細胞を1対1の割合で洗浄する。2回目の洗浄のためにPBSの5ミリリットルでペレットを再懸濁し、続いてカウント用の新鮮なRPMI培地の10ミリリットルで再懸濁を行う。次いで、さらなる遠心分離で細胞を採取し、新鮮なRPMI濃度の5マイクロリットル当たり7細胞に10倍に希釈した。足場を播種する前に、必要に応じてポリマーを5ミリメートルの長さにカットし、5マイクロリットルのRPMI培地で足場を5分間濡らします。インキュベーションの終わりに、余分な培地を取り除き、5マイクロリットルの骨髄単核細胞溶液を足場の腔に10分間ロードする。次に、足場の内腔に21ゲージの針を挿入し、移植片を37°Cインキュベーターで一晩RPMI培地の1ミリリットルに入れます。全身麻酔の誘導後、レシピエント動物のつまみつまみへの応答の欠如を確認し、動物の目に軟膏を適用する。顎から鎖骨まで外科部位の毛をクリップし、動物を後回しリカンベント位置の外科パッドに置きます。無菌技術を用いて、シーケンシャルポビドネヨウ素、70%エタノール、ポビドネヨウ素ワイプシリーズで外科部位を消毒し、12時に頭部を持つ解剖顕微鏡の下でマウスを動かす。細かいはさみとマイクロAdson鉗子を使用して鎖骨から舌骨への正中線切開を行い、デュモン番号5とナンバー7の細かい鉗子と無菌綿棒を使用して鼻隠しを清掃してから、自己保持のコリブリトラクターを切開に挿入します。鉗子を使ってストラップの筋肉を開いて甲状腺軟骨、口骨、気管を露出させ、組織の両側で平行に走る再発喉頭神経から気管を鈍く分離し、続いて食道から気管を周回分離する。20ゲージの針と外科用マーカーを使用して、気管の前部を染色する。ヴァンナス・チュービンゲンのスプリングハサミとデュモン番号7の細かい鉗子のペアを使用して、3番目の気管軟骨リングの下に切開を行い、気管の3リングセクションを切除します。細かい湾曲した鉗子で気管を保持し、滅菌9-0ナイロン縫合糸を使用して気管の遠位端を胸骨に固定し、一時的な気管切開術を行い、20ゲージの針と外科マーカーを使用して移植片の前部を染色します。移植片を移植するには、9-0の無菌ナイロン縫合糸、デュモン番号7の細かい鉗子、および針のホールダーを使用して、近位後部、近位横方向、および近位前部のステッチを挿入し、動物を180度回す。マウスが位置に入ったら、一時的な気管切開術を解除し、側面切開を移植片の前部から5ミリメートル離れて行い、遠位吻合を容易にする。遠位吻合を完了するには、接合糸を近位吻合と同様の方法で配置し、移植片の位置とストラップの筋肉を再近似します。次に、9-0の無菌ナイロン縫合糸で切開をランニングパターンで閉じ、完全に回復するまで監視して加熱パッドに置かれた回収ケージに動物を単独で入れる。移植の2週間後、いくつかの基底上皮細胞は、組織で設計された気管移植組織切片における基底細胞ケラチン5および14染色によって観察することができる。基底細胞はまた、移植後7日で移植片の発光表面上で検出される。排泄および組織学的分析の際に、移植片狭窄は、呼吸困難およびストリドールの徴候を示す移植片移植マウスの主要な要因として同定された。なお、移植片および在来性気管の伸縮は、外科的手法の結果として一般的な発見である。F4/80の染色は、基底上皮細胞の存在と併せて移植片のステナティック領域内の宿主マクロファージを明らかにする。再発性喉頭神経の操作を避け、吻合で気密シールを可能にするために近位および遠位縫合糸が整っていることを確認する。このプロトコルは、生体安全レベル1剤マウスに由来する骨髄単核細胞を扱うため、汚染を避けるために適切な個人用保護具を着用することが重要である。
