次世代シーケンシング(NGS)の出現は、癌の診断と治療方法に革命をもたらした1.多地域シーケンシングに結合したNGSは、固形腫瘍2において高い腫瘍内不均一性(ITH)を露呈しており、これは薬物感受性の異なるサブクローンの存在による標的治療の失敗を部分的に説明している2.ゲノムワイドシーケンシング研究によって提起される重要な課題は、個々の癌における乗客(すなわち中性)と運転者の突然変異を区別する必要性である3。いくつかの研究は確かに、特定の腫瘍において、乗客の突然変異がITHの大部分を占める一方で、運転者の変化は同じ個体4の病変の間で保存される傾向があることを示している。また、大きな突然変異負担(肺癌および黒色腫に見られる)は、必ずしも大きな鎖下突然変異負担2を意味するわけではないことに注意することも重要である。したがって、高い程度のITHは、突然変異の負担が低い腫瘍に見出すことができる。

我々のプロトコルは、疾患進行に関与する分子変化の同定を通じて、一次病変と転移性病変の間の遺伝的関連を捕捉することを可能にする。我々の方法は、高感度および特異性を有する臨床検体における遺伝的関連を探求する機会を提供する。これは分子と組織病理学的分析の統合によるものです。がん研究分野における腫瘍内不均一性の評価は、効果的な抗腫瘍戦略の設計において最も重要であり、我々の方法は多くの固形腫瘍タイプに広く適用可能である。目的のライブラリを準備して定量化した後、各ライブラリをヌクレアーゼフリー水中の100ピコモラー濃度に希釈し、各希釈されたライブラリの10マイクロリットルを組み合わせて1本のチューブで1回実行します。次に、プールされたライブラリをピペット処理で混ぜます。シーケンス実行を開始するには、まずシーケンスシステムに接続されているコンピュータでTorrentブラウザを開きます。選択したアプリケーションの汎用テンプレートを使用して新しい実行を計画し、[キット]タブで[イオンプロトンシステム]または[イオンジーンスタジオS5システム]を選択します。チップタイプドロップダウンメニューから適切なチップを選択し、ライブラリキットのドロップダウンリストからIon AmpliSeq 2.0ライブラリキットを選択します。テンプレートキットの[Ion Chef]ボタンを選択し、テンプレートキットのドロップダウンリストから適切な Chef Kit を選択します。シーケンス キットドロップダウン メニューから適切なシーケンシング キットを選択し、バーコード セットのドロップダウン リストから [イオン エクスプレス] を選択します。プラグインタブで、カバレッジ分析プラグインを選択します。[プラン]タブで、参照ライブラリのドロップダウンリストからGRCH37HG19ゲノムを選択します。ターゲットリージョンドロップダウンリストから、シーケンスする適切なパネルを選択します。次に、シーケンス処理するバーコードの数を設定し、各ライブラリのバーコード名とサンプル名を設定します。プールされたライブラリ希釈の場合、ストックライブラリをヌクレアーゼフリー水で25ピコモラー濃度に希釈し、各希釈ライブラリのピペット50マイクロリットルを適切なライブラリサンプルチューブの底に希釈します。プールされた希釈されたライブラリを含むサンプルチューブをライブラリ調製システムにロードし、クローン増幅を開始します。次世代シーケンスの場合は、画面上の指示に従って計測器を初期化します。クローン増幅が終了したら、イオンシェフの計器ドアを開け、チップローディング遠心分離機の蓋を開け、ライブラリ調製システムから2つのチップを取り外します。チップを別々のチップストレージ容器に入れ、チップの1つを機器のチップコンパートメントにセットします。次に、チップコンパートメントの蓋を閉じて、シーケンスを開始します。突然変異解析の場合は、カバレッジ分析プラグインを開き、カバレッジ分析出力を使用して、カバレッジの深さと均一性を検証します。必要に応じて、生殖ラインまたは体系ワークフローを選択して呼び出しバリアントを実行するには、トレントバリアント発信者プラグインを使用します。次に、フィルタされたバリアントバリアントのコール形式ファイルをダウンロードし、NCBI RefSeqデータベースのバリアント効果予測ソフトウェアを使用してバリアントにアコージンを付けます。コピー数の変動を推定するには、イオンレポーターアップローダープラグインを使用して、シーケンシングデータをイオンレポーターソフトウェアにロードし、包括的ながんパネル腫瘍正常ペアワークフローを使用してデータを分析します。ソフトウェアによって割り当てられたスコアに基づいて、突然変異とコピー数のバリエーションを手動でフィルタリングし、統合ゲノミクスビューアで視覚的にバリエーションを検証します。次に、アーティフィファクト呼び出しを生成する異常な読み取り値のアライメントを視覚的に検査し、特定の遺伝子全体のすべてのアンプリコンの正規化されたカバレッジをベースラインの正規化されたカバレッジに対して検査して、コピー数の変動を検証します。それぞれの場合に対して、正常組織または血液または原発腫瘍または転移の順序から突然変異呼び出しをインデックス化する。生殖系列としてフラグを立て、生殖系列DNAのシーケンシングデータからも明らかなコールを破棄する。特定の患者のすべての病変の間で共有される突然変異をクローンまたは創設者としてフラグします。次に、特定の患者の一部ではなくすべての病変で検出される突然変異を、クローンまたは進行性または進行性としてフラグを立てます。本代表的な実験では、409個のがん関連遺伝子のコード配列を標的とした5つの固体偽パピラリー新生物症例の多病変シーケンシングは、8つの遺伝子で合計27の体細胞突然変異を同定した。突然変異は、特定の患者の病変の一部ではなく全ての病変で検出された場合、与えられた患者および進行性または亜クローナルのすべての病変の間で共有される場合、創始者またはクローンとして定義された。一貫して、β-カテニンおよびKMD6Aの免疫ヒストリカル染色は、対応する遺伝子の変異を有する症例の多様な病変の中で均質であった。変異標本におけるKMD6Aの適度な染色は、遺伝子改変がタンパク質の発現ではなく機能を変化させる可能性があることを示唆している。KMD6膵臓腫瘍における機能の喪失は、低酸素マーカーGLUT-1のアップ調節に関連している。それに応じて、KMD6A突然変異を有する症例においてGLUT-1が過剰に発現した。BAP1およびTP53の免疫染色法により、これらの遺伝子の変異がクローナルであることを確認した。この仮想核型は、改変遺伝子の位置、近接性、コピー数の状態を示す代表ケースの中で、シーケンシングデータを用いたコピー数変動解析により、解析対象となる全ての検体における変化が明らかになった。そして、ポイント突然変異とは対照的に、コピー数変動の大部分はクローン下であった。突然変異の検証とインデックス化と、腫瘍の正常なDNA比較を使用することは、結果を無効にする可能性のある実際の呼び出しの生成を避けるために重要です。この手順は、異なる固形腫瘍タイプの病変間の突然変異の同定のためにゲノム全体を尋問することを目的とした全ゲノムシーケンシング研究にも適用できる。がん研究分野では、これらの技術は、パーソナライズされた効果的な標的療法を設計することを目的とした重要な臨床的意味を持つ疾患の生物学を理解するための重要なツールです。
