In vergelijking met traditionele tDCS verhoogt HD-tDCS de focaliteit van stimulatie. Typische sites van stimulatie zijn vaak gebaseerd op het 10-20 EEG systeem. Het bepalen van de precieze stimulatie punten buiten dit systeem kan echter moeilijk zijn. Transcraniële directe stroom stimulatie (tDCS) is een niet-invasieve techniek die corticale prikkelbaarheid moduleert met zwakke directe stromingen over de hoofdhuid. Het is bedoeld om causaliteit tussen neurale prikkelbaarheid en gedrag in gezonde mensen1,2,3vast te stellen. Bovendien, als een motorische neurorehabilitatie tool, tDCS wordt veel gebruikt in de behandeling van de ziekte van Parkinson, beroerte, en cerebrale parese4. Uit bestaand bewijs blijkt dat traditionele op pad gebaseerde tdcs de stroomtoevoer door een relatief groter hersengebied van5,6,7produceert. High-Definition Transcraniële directe stroom stimulatie (HD-tdcs), waarbij de middelste ring elektrode over een doel corticale regio wordt omringd door vier terugkeer elektroden8,9, verhoogt de focaliteit door het bezoedelen van vier ring gebieden5,10. Bovendien, veranderingen in prikkelbaarheid van de hersenen geïnduceerd door HD-tDCS hebben aanzienlijk grotere magnitudes en langere duur dan die gegenereerd door traditionele tDCS7,11. Daarom wordt HD-tDCS veel gebruikt in onderzoek7,11.
Niet-invasieve hersenstimulatie (NIBS) vereist gespecialiseerde methoden om ervoor te zorgen dat een stimulatie plaats aanwezig is in de standaard MNI en Talairach Systems12. Neuronavigation is een techniek die het mogelijk maakt om interacties tussen Transcraniële stimuli en het menselijk brein in kaart te brengen. De visualisatie en 3D-beeldgegevens worden gebruikt voor precieze stimulatie. In zowel tdcs als HD-tdcs is een gemeenschappelijke beoordeling van stimulatie sites op de hoofdhuid meestal het EEG 10-20-systeem13,14. Deze meting wordt veel gebruikt voor het plaatsen van de tdcs-pads en optode-houders voor functionele near-infraroodspectroscopie (fnirs) in de beginfase13,14,15.
Het bepalen van de precieze stimulatie punten bij het gebruik van het 10-20-systeem kan moeilijk zijn (bijv. in de temporo-pariëtale junctie [TPJ]). De beste manier om dit op te lossen is om structurele beelden van deelnemers te verkrijgen met behulp van magnetische resonantie imaging (MRI), en vervolgens de exacte sonde positie te verkrijgen door doelpunten te matchen met hun structurele afbeeldingen met behulp van digitaliserende producten15. MRI biedt een goede ruimtelijke resolutie, maar is duur om15,16,17te gebruiken. Bovendien kunnen sommige deelnemers (bijv. personen met metaal implantaten, claustrofobische mensen, zwangere vrouwen enz.) niet worden blootgesteld aan MRI-scanners. Daarom is er een sterke behoefte aan een handige en efficiënte manier om de bovengenoemde beperkingen te overwinnen en de nauwkeurigheid bij het bepalen van stimulatie punten te vergroten.
Dit protocol maakt gebruik van een 3D-digitizer om deze beperkingen te overwinnen. Vergeleken met MRI zijn de belangrijkste voordelen van een 3D-digitizer lage kosten, eenvoudige toepassing en draagbaarheid. Het combineert vijf referentiepunten (d.w.z. cz, FPZ, Oz, linker preauriculaire punt en rechter preauriculaire punt) van individuen met locatie-informatie van de doel stimulatie punten. Vervolgens produceert het een 3D-positie van elektroden op het hoofd van het onderwerp en schat hun corticale posities door te passen met de enorme gegevens uit de structurele afbeelding12,15. Deze probabilistische registratiemethode maakt de presentatie van Transcraniële toewijzingsgegevens in het MNI-coördinatensysteem mogelijk zonder opname van de magnetische resonantie beelden van een onderwerp. De aanpak genereert anatomische automatische labels en Brodmann gebieden11.
De 3D digitizer, gebruikt om ruimte coördinaten te markeren op basis van de gegevens uit structurele beelden, werd voor het eerst gebruikt om de positie van optodes in de fNIRS Research18te bepalen. Voor degenen die HD-tDCS gebruiken, breekt een 3D-digitizer de eindige stimulatie punten van het EEG 10-20-systeem. De afstand van de vier retour elektroden en de middelste elektrode is flexibel en kan zo nodig worden aangepast. Bij gebruik van de 3D-digitizer met dit protocol, werden de coördinaten van de rTPJ verkregen, die buiten het 10-20-systeem ligt. Ook getoond zijn de procedures voor targeting en het stimuleren van de juiste temporo-pariëtale kruising (rTPJ) van het menselijk brein.

De overvloed aan neuroimaging data en snelle ontwikkeling van machine learning heeft het mogelijk gemaakt om hersenactivatiepatronen te onderzoeken. Deze methode maakt gebruik van een 10-10-systeem om de beperkingen van transcraniële direct stroom stimulatie elektrode plaatsing te overwinnen, het verbeteren van de nauwkeurigheid en reproduceerbaarheid van de methode. De belangrijkste voordelen van deze techniek zijn de lage kosten, eenvoudige toepassing en draagbaarheid. Deze methode is geschikt voor gebruik met andere technieken, bijvoorbeeld functionele nabij-infrarood spectroscopie, om de locatie van specifieke hersengebieden van belang te verifiëren.Deze techniek is zeer eenvoudig als u bekend bent met de evenredige eis van het 10-10 systeem. Het aantonen van de procedure zal Chenyu Lv, een afgestudeerde student. Om een elektrode holding dop voor te bereiden, plaats een badmuts op hoofdvorm en meet de afstand tussen de nasion en inion.Om het hoekpunt te lokaliseren, gebruikt u een huidmarkering om het middelpunt van de afstand tussen de nasie en de inion te markeren en de afstand tussen de preauriculaire punten te meten. Markeer het middelpunt van de preauriculaire punten. Het punt waarop beide middelpunten elkaar kruisen is de hoekpunt.Volgens de 10-10 systeem, vind de CP6 en P6, en markeer de juiste middelpunt waarop deze punten elkaar kruisen, genaamd de juiste temporoparietale verbinding op de hoofdhuid. Plaats de badmuts op het hoofd van het onderwerp en gebruik de referentiemarkeringen op de dop om de dop uit te lijnen met het internationale 10-10-systeem voor de hoofdhuidlocatie. Wanneer de dop aanwezig is, gebruikt u een USB-interface om de 3D-digitizer op de computer aan te sluiten en te bevestigen dat de digitizer-software beschikbaar en klaar is.Plaats de bron voor het onderwerp en bevestig het elastische touw van de sensor rond het hoofd van het onderwerp. In de digitizer-software bevestigt u dat het 3D-digitizer-systeem communiceert met de software en gebruik de stylus en een liniaallengte van 10 centimeter om de nul en tien graduaties op te nemen om de nauwkeurigheid van de stylus te bevestigen. Geef in de software de afstand tussen de twee opnamepunten weer.Verzamel vervolgens de positiegegevens van de referentiepunten, centrumelektroden en de vier retourelektroden. Om tegemoet te komen aan de eis van de functionele nabij-infrarood spectroscopie experimenten, selecteren en opslaan van de zender detector en kanaal opties. Om de stimulatie te starten, installeert u volledig opgeladen batterijen in het apparaat en sluit u het conventionele transcraniële directstroomstimulatieapparaat en de vier aan door één stimulatieadapter.Sluit de kabels van de vijf zilverzilvere chloride-gecentreerde ringelektroden aan op de bijpassende ontvangers op de vier door één adapteruitgangskabel. En meet het hoofd van het onderwerp. Plaats de plastic apparaat dop op het hoofd van het onderwerp, en insluiten van de vijf plastic high-definition behuizingen in de badmuts.Zoek de hoekpunt FPZ en OZ van het onderwerp en pas de referentie op de dop aan om af te stemmen op het internationale 10-10-systeem voor hoofdhuidlocaties. Wanneer de dop aanwezig is, gebruikt u de 3D-digitizer om de positiegegevens van de gestimuleerde hersengebieden te verzamelen. Gebruik vervolgens het uiteinde van een plastic spuit om het haar zorgvuldig te scheiden door de opening van de plastic behuizing, totdat de hoofdhuid wordt blootgesteld en de blootgestelde huid te bedekken met de elektrisch geleidende gel.Schakel op het apparaat de vier bij één multi-channel stimulatieadapter in en stel de kwaliteitswaarde in. Controleer of de standaardinstelling is ingesteld om te scannen en druk op de knop Modus selecteren. Schakel van de scan naar de doorgang en druk op de polariteitsknop om te selecteren, hetzij midden anode of kathode.Pas de instellingen op de conventionele transcraniale direct stroom stimulatie apparaat om de stimulus duur, intensiteit en sham conditie instelling op te nemen en druk op de ontspanhendel om over te schakelen naar volle stroom. Zodra alles is ingesteld, drukt u op de startknop om de stimulatie te starten. De D.C.intensity zal opvoeren tot de doelstroom is bereikt, en de timer zal dan de resterende tijd weergeven.Draai aan het einde van de stimulatie de hendel langzaam om de stroom op nul aan te passen voordat u de stroom uitschakelt. Open de plastic dop, verwijder de zilver- of zilverchloride gesinterde ringelektroden uit de behuizing en verwijder de badkap. Maak vervolgens de instrumenten schoon en voorziet het onderwerp van materialen waarmee ze hun haar kunnen reinigen.Met behulp van de near infrared spectroscopie statistische parametrische mapping standalone registratie functie, de ruimtelijke registratie functie genereert MNI coördinaten. Om meetfouten te verminderen, werd de gemiddelde waarde van drie gegevenspunten uit de uiteindelijke MNI-coördinaten van de vijf elektroden berekend. In Broadman gebieden, worden het anatomische etiket en zijn aantal verkregen.Het getal na elke regel geeft het overlappingspercentage aan. In anatomische automatische labels worden het anatomische label en het percentage overlapping verkregen. Voor de Broadman-gebieden en anatomische automatische labels vertegenwoordigt de waarde een percentage overlapping met de hersenschors.Het is belangrijk om ervoor te zorgen dat noch de bron, noch de sensor beweegt tijdens de 3D digitizer meting. Deze procedure kan worden gebruikt voor van gedachte lokalisaties of voor andere lokalisaties die nodig zijn om de functie van de hersenen te bestuderen. Deze techniek kan ook worden gebruikt in gepersonaliseerde hersenstimulatie voor nauwkeurige positionering.
