robot-assisted kidney transplantation

この論文では、生体ドナーからのロボット支援腎移植(RAKT)について説明します。ロボットは、患者の分割された脚の間にドッキングされ、仰臥位のトレンデレンブルグ位置に置かれます。腎臓同種移植片は、生体ドナーによって提供されます。血管吻合の前に、尿管にダブルJステントを挿入して腎臓同種移植片を調製し、氷パックガーゼで包んで吻合の温度を下げる。ロボットカメラ用の12mmまたは8mmポートと、ロボットアーム用の3つの8mmポートが配置されています。腹膜ポーチは、腸骨血管と膀胱を解剖する前に、腹膜フラップを腰筋の上に持ち上げることによって、腎臓同種移植用に作成されます。6cmのPfannenstiel切開を行い、腎臓を腹膜ポーチの右腸骨血管の外側に挿入します。

このプロトコルでは、反転することなくロボット支援腎移植を行うための段階的な手順を提案しました。腹膜ポーチに同種移植片を、適切な位置に腎血管を用いて血管吻合を行う方が自然です。この技術は、肝臓や膵臓などの他の固形臓器移植にも適用できます。ロボットシステムを用いた低侵襲手術アプローチの移植分野での有用性を拡大したいと考えています。ロボット支援腎移植は、ロボットシステムについてある程度の教育を受けた後にのみ研究することができます。経験豊富な外科医の助けを借りることをお勧めします。手順を開始する前に、腹腔内解剖を実行するために手術器具を備えたロボットアームを準備します。単極湾曲はさみをアーム2に、開窓バイポーラ鉗子をアーム3に、ProGrasp鉗子をアーム4に配置します。右傍結腸側溝に沿って腹膜を切開し、腎臓同種移植用のポーチを作ることで手術手順を開始します。切開後、右内腸骨血管を全長に沿って解剖し、各血管を血管ループで囲みます。尿管膀胱瘻造設術の場合は、右隅の膀胱を解剖し、膀胱を腹膜切開から分離します。ゲルポートのキャップを開き、スラッシュした氷を6センチメートルのPfannenstiel切開部に挿入し、続いて氷を詰めたガーゼで包んだ腎臓同種移植片を挿入します。次に、同種移植片を右側の腸骨血管の外側の腹膜ポーチに置きます。ロボットアーム4のProGrasp鉗子の制御で、ブルドッグクランプで右外腸骨静脈をクランプします。次に、ポッツはさみを直線的または斜めに使用して、腎静脈の直径を考慮して静脈切開を行います。6/0 ePTFE縫合糸を使用して、右外腸骨静脈への腎静脈同種移植片に端から側まで連続的に吻合します。外腸骨静脈と腎静脈の後壁を管腔内および連続的に縫合する前に、静脈の尾端に結び目を作ります。その後、腸骨静脈の前壁を連続的に縫合します。そして、ヘパリン処理された生理食塩水で内腔を洗い流す。次に、ゲルポートを通るSilasticチューブを使用して、吻合を結びます。同種移植腎静脈をブルドッグクランプでクランプします。次に、右外腸骨静脈のクランプを解除し、続いてブルドッグクランプで右外腸骨動脈をクランプします。動脈パンチなしでポッツハサミで丸い穴を作ることによって、右外腸骨動脈に動脈切開を行います。そして、示されているように、同種移植腎動脈を右外腸骨動脈に吻合します。吻合部位に明らかな出血がない場合は、同種移植腎静脈と動脈のクランプを解除します。そして、氷を詰めたガーゼを取り除きます。次に、灌漑チューブを使用して、温かい生理食塩水をゲルポートを介して同種移植片に塗布します。Lich-Gregoir法に従って尿管膀胱瘻造設術を行うには、ダブルJ尿管ステントの遠位端を膀胱に入れます。切開した膀胱粘膜の後隅から始めて、6/0ポリジオキサノン縫合糸を使用して連続縫合を行います。そして前隅に縫合糸を作ります。前角から後隅まで、連続縫合を行います。また、4/0ポリグラクチンマルチフィラメント吸収性縫合糸を使用して、排尿筋逆流防止トンネルを中断して閉じます。ポリマーロッククリップの助けを借りて、断続的に右傍結腸溝に沿って切開された腹膜で腎臓同種移植片を覆います。ロボット支援腎移植(RAKT)は、3人の肥満患者を含む21人のレシピエントに対して実施されました。1人の患者は腎静脈血栓症のために原発性機能不全を患った。移植片切除術は腎移植後3日目に施行され,移植機能の遅延は認められなかった.少数の患者は、腎移植前に後部テーブルで腎動脈または腎静脈の血管形成術を受けなければなりませんでした。腎移植後の平均入院日数は7.4日であった。RATKの1か月後の平均推定糸球体濾過率(eGFR)は、1平方メートルあたり毎分74.9ミリリットルでした。これまでの報告とは異なり、従来の開放法と同様に、血管吻合前の腸骨血管外側に腎臓同種移植片を配置しました。この技術は、腎血管の予期しないねじれやねじれを防ぐのに役立ちます。腎臓同種移植片を外側に配置した後、静脈および動脈の吻合を適切な角度と長さで行うことができます。特に免疫リスクの高い患者において、腎臓同種移植の生存率や生検で証明された急性拒絶反応を含む長期的な臨床転帰をロボット支援腎移植と開放腎移植の間で比較する必要があります。
