삼십 년이 넘는 기간 동안 연구자들은 생성된 데이터가 실제 임상 상태를 보다 일관되고 더 잘 반영할 수 있도록 쥐 폐 이식 모델을 수정하고 개선해 왔습니다. 우리 실험실에서 이 모델을 수행하는 동안 우리는 문합에 대한 커프 기술, 수혜자의 4번째 늑간 공간 식별, 수혜자의 시술 중 폐 팽창 및 상처 봉합, 분석을 위한 샘플 수확의 네 가지 개선 영역을 결정했습니다.
문합에 대한 커핑 기술 수정은 기증자 폐 1,2,3,4,5,6의 처리 시간을 단축하고 미세외과 의사가 문합 절차를 더 빠르고 기술적으로 쉽게 만들어 전체 이식 절차를 개선할 수 있습니다. 이식된 폐에 필요한 혈액과 기류를 공급하기 위해 적절한 크기의 커프를 사용하는 것이 중요하지만 폐정맥(PV), 폐동맥(PA) 또는 기관지(Br)5,7,8,9. 새로운 미세외과 의사는 기증자 시술 중에 기증자 폐를 성공적이고 쉽게 조달할 수 있지만 수혜자의 시술 중에 폐를 이식하는 것은 더 복잡하고 미세외과 의사의 경험에 달려 있습니다. 수혜자의 왼쪽 폐에 접근하기 위해 4번째 늑간 공간을 찾으려는 시도는 주관성을 유지하고 시술 시간을 늘릴 수있는 더 어려운 단계 중 하나입니다. 따라서 흉부 측정과 심장의 두근거림을 이용하여4,5,6,10,11,12를 해부할 정확한 부위 흉벽을 찾아늑간 공간 위치를 식별하는 데 도움이 되는 간단하고 객관적인 방법을 소개합니다.
우리는 또한 장기 손상의 잠재적 원인인 기증자 폐 팽창에 대한 개선을 제안합니다. 기증자 폐는 재관류가 시작될 때까지 수축됩니다. 4번째 늑간 공간을 봉합하는 동안 기증자 폐는 일반적으로 PEEP를 2 cmH 2 O에서 6 cmH2O로 증가시켜팽창시킵니다. 과팽창으로 인한 폐 손상을 최소화하기 위해 5번째 갈비뼈보다 아래쪽에 있는 4번째 갈비뼈 주위에 간단한 이중 매듭으로 3개의 6-0 나일론 봉합사를 배치하는 기술을 제안합니다. 상처를 봉합 할 때가되면 3 개의 봉합사의 끝을 양손에 지혈제로 잡고 양쪽을 잡아 당겨 상처를 한꺼번에 봉합하고 PEEP를 즉시 2cmH2O로 줄입니다. 이런 식으로, 폐는 가능한 한 최단 시간 동안 확장 될 수 있습니다10.
실험이 끝나면 연구자는 종종 각 이식에서 다양한 유형의 분석을 위해 여러 유형의 샘플을 수집하려고 합니다. 예를 들어, 스냅 냉동 조직, 포르말린 고정 조직, 폐부종을 결정하기 위한 습윤 대 건조 중량 비율 조직 및 기관지정맥 세척액(BAL)은 모두 이식이 얼마나 잘 진행되었는지 평가하는 데 사용할 수 있습니다. 기관지폐 기관지폐 수액을 수집하는 전통적인 방법은 수혜자의 천연 엽과 기증자의 이식된 엽 둘 다로부터 혼합된 풀링된 샘플을 허용한다13,14,15. 이를 극복하기 위해 우리는 이식된 폐와 천연 폐의 상태에 대한 보다 정확한 통찰력을 얻을 수 있는 폐문 영역을 클램핑하는 방법을 제시합니다. 또한 폐의 각 측면에서 기관지폐 기관지폐액을 수집하는 데 사용되는 PBS의 양은 농도로 측정되는 사이토카인 및 케모카인과 같은 수많은 용해성 인자를 포함하고 있기 때문에 고려하는 것이 중요합니다. 예상되는 폐활량에 주입 된 체액의 양을 정상화하면 비교에 도움이 될 수 있습니다. 오른쪽에 4개의 엽(葉)이 있고 왼쪽에 1개의 엽(葉)이 있으며, 쥐의 5개 엽(葉)은 각각 다른 부피와 표면적을 가지고 있다(16).
