Les brûlures sont parmi les formes de traumatisme les plus dévastatrices, avec des taux de mortalité atteignant 12%, même dans les centres spécialisésdans les grands brûlés 1,2,3. Selon des rapports récemment publiés, ~ 486 000 patients brûlés nécessitent des soins médicaux chaque année aux États-Unis, avec près de 3 500 décès 1,2,3,4,5,6. Les brûlures constituent un défi majeur pour le système immunitaire des patients et créent une plaie ouverte importante, qui est lente à guérir, les laissant vulnérables à la colonisation cutanée, pulmonaire et systémique par des bactéries nosocomiales et opportunistes. La dérégulation immunitaire combinée à l’infection bactérienne est associée à une augmentation de la morbidité et de la mortalité chez les patients brûlés7.
Un modèle de brûlure et d’infection animale est essentiel pour étudier la pathogenèse des infections bactériennes à la suite de lésions cutanées et d’une suppression immunitaire associée à un traumatisme par brûlure. De tels modèles permettent la conception et l’évaluation de nouvelles méthodes de traitement des infections bactériennes chez les patients brûlés. Les rats et les humains partagent des caractéristiques physiologiques et pathologiques cutanées similaires qui ont été précédemment documentées8. De plus, les rats sont de plus petite taille, ce qui les rend plus faciles à manipuler, plus abordables et plus faciles à se procurer et à entretenir que les modèles animaux plus grands.
Ces caractéristiques font des rats un animal modèle idéal pour étudier les brûlures et les infections9. Malheureusement, la technique d’induction des brûlures est incohérente et souvent décrite de manière minimale10,11,12,13,14. Le présent protocole est conçu pour mettre au point une procédure simple, rentable et reproductible pour créer une brûlure constante sur toute l’épaisseur dans un modèle de rat qui simule le scénario clinique et peut être utilisé pour évaluer l’immunosuppression et l’infection bactérienne.

Les méthodologies d’induction des brûlures ne sont pas décrites de manière cohérente dans les modèles de rats. Un modèle uniforme de brûlure, qui représente le scénario clinique, est nécessaire pour effectuer des recherches reproductibles sur les brûlures. Le présent protocole décrit une méthode simple et reproductible pour créer des brûlures de toute épaisseur de ~20% de la surface corporelle totale (TBSA) chez les rats. Ici, une tige de cuivre de 22,89 cm2 (5,4 cm de diamètre) chauffée à 97 °C dans un bain-marie a été appliquée sur la surface de la peau du rat pour provoquer la brûlure. Une tige de cuivre avec une conductivité thermique élevée a pu dissiper la chaleur plus profondément dans le tissu cutané pour créer une brûlure de pleine épaisseur. L’analyse histologique montre un épiderme atténué avec des dommages coagulatifs à toute l’épaisseur du derme et du tissu sous-cutané. De plus, ce modèle est représentatif des situations cliniques observées chez les patients hospitalisés souffrant de brûlures à la suite de brûlures telles qu’une dysrégulation immunitaire et des infections bactériennes. Le modèle peut récapituler l’infection bactérienne systémique par les bactéries à Gram positif et à Gram négatif. En conclusion, cet article présente un modèle de brûlure de rat robuste et facile à apprendre qui imite les situations cliniques, y compris la dysrégulation immunitaire et les infections bactériennes, ce qui est d’une utilité considérable pour le développement de nouveaux antibiotiques topiques pour les brûlures et les infections.

Le protocole présenté ici est hautement reproductible et a entraîné une brûlure au troisième degré de pleine épaisseur chez les rats. Un modèle imitant le scénario clinique des brûlures et des infections est nécessaire pour faire avancer la recherche sur les brûlures. Le présent protocole démontre un modèle simple et reproductible d’infection par brûlure chez le rat, comparable à celui de l’homme. Ce modèle de brûlures est capable de récapituler les situations cliniques observées chez les patients brûlés hospitalisés à la suite de brûlures telles que la dysrégulation immunitaire et les infections bactériennes. Cette méthode d’induction de brûlures chez le rat est facile à apprendre et peut être utilisée pour le développement et l’évaluation de nouveaux médicaments tropicaux pour les brûlures et les infections. Ce modèle peut être utilisé pour évaluer la cicatrisation des plaies ainsi que la progression de l’infection bactérienne au site de la brûlure.Cette méthode d’induction de brûlure peut également être utilisée dans des modèles murins et animaux pour l’induction de brûlures et pour étudier l’infection bactérienne des brûlures. Commencez à préparer le rat anesthésié pour la brûlure en le déplaçant sur un coussin chauffant en position couchée. À l’aide d’un applicateur à embout de coton, appliquez du lubrifiant pour les yeux sur les deux yeux pour éviter le dessèchement de la cornée.Rasez la région dorsale du rat avec une tondeuse électrique dans un grand rectangle allant des omoplates jusqu’à la base de la queue. Essuyez les poils lâches de la zone rasée en utilisant le tissu imbibé de solution saline. Appliquez la lotion d’épilation sur la zone rasée à l’aide d’un applicateur à embout de coton.Après trois minutes, essuyez la zone avec une éponge de gaze humide deux fois pour enlever la lotion et prévenir l’irritation de la peau. Éteignez l’isoflurane et retirez le cône nasal avant de placer le rat dans la cage de récupération. Le jour de l’expérience de brûlure, réglez la température du bain-marie à 97 degrés Celsius et une heure avant l’expérience, placez 420 grammes de quatre tiges de cuivre dans le bain-marie.Une fois que le rat ne répond pas au pincement des orteils sur tous les membres, placez-le sur le coussin chauffant en position couchée et injectez 20 milligrammes par kilogramme de poids corporel de morphine via la racine intrapéritonéale, ou IP, pour la gestion de la douleur. Ensuite, vérifiez la température du bain-marie. Réglez la minuterie et enfilez les gants résistants à la chaleur.Prenez une tige de cuivre chauffée du bain-marie et touchez-la sur la région du dos pendant sept secondes. Appliquez immédiatement les trois brûlures suivantes l’une après l’autre en utilisant une tige par site de brûlage et produisez une brûlure totale d’environ 20% de la surface corporelle avec contact. Après la brûlure, réanimez l’animal en injectant 0,1 millilitre par gramme de poids corporel de solution de Ringer lactée par la voie IP.Une fois cela fait, éteignez l’isoflurane. Retirez le cône de nez et placez le rat sur le tapis chauffant. Le jour de la brûlure et de l’infection, centrifuger la culture de Pseudomonas aeruginosa ou Staphylococcus aureus séparément à 4 000 fois g pendant 5 minutes.Lavez la pastille avec une solution saline normale avant de la diluer avec une solution saline à une densité optique de 0,1 à 600 nanomètres. Diluer 200 microlitres de la suspension bactérienne avec 800 microlitres de solution saline et obtenir l’inoculum bactérien désiré de 2 par 10 aux 7e unités formant colonie ou UFC par millilitre. 15 minutes après la brûlure, injectez par voie sous-cutanée 50 microlitres d’inoculum dilué de la bactérie près de la zone brûlée à l’aide d’une aiguille de calibre 29.Placez le rat sur le coussin chauffant pour la récupération. Après 15 à 20 minutes, transférez l’animal récupéré dans une cage propre. Immédiatement après la brûlure, évaluez morphologiquement la lésion cutanée en termes de couleur et de marge.Après l’euthanasie, analyser la numération globulaire complète pour déterminer l’effet de l’induction de brûlures sur le système immunitaire de l’hôte. Recueillir le tissu du rat euthanasié dans un tube de collecte de 10 millilitres. Homogénéiser les échantillons à l’aide d’un homogénéisateur tissulaire et diluer en série les homogénats tissulaires dans une solution saline normale.Plaque 100 microlitres d’homogénat non dilué et toutes les dilutions de chaque échantillon de tissu prélevé sur des rats infectés par P. Aeruginosa sur des plaques de gélose au cétrimide. Après avoir incubé les plaques pendant 16 à 18 heures à 37 degrés Celsius dans un incubateur, comptez les colonies bactériennes sur les plaques. Multipliez-les par le taux de dilution pour obtenir le nombre d’UFC par millilitre et normalisez-le avec le poids du tissu pour calculer le tissu du programme UFC.Ce protocole hautement reproductible a entraîné une brûlure de pleine épaisseur au troisième degré chez les rats. La couleur de la brûlure est passée du blanc au brun en 24 heures à 72 heures après la brûlure. Dans l’analyse histologique, les échantillons de peau d’animaux brûlés ont montré des lésions dans toutes les couches 24, 48 et 72 heures après la brûlure, tandis que la peau non brûlée a montré une distinction claire des couches épidermiques, dermiques et sous-cutanées.Une destruction de la couche épidermique et des dommages à l’épaisseur totale du derme avec la graisse sous-cutanée et le muscle squelettique ont été observés dans des échantillons de peau brûlée. Dans les 24 heures suivant la combustion, une combustion pleine épaisseur était de plus de 2,61 millimètres de profondeur. Dans l’évaluation de la clairance bactérienne, une récupération bactérienne a été observée chez tous les rats brûlés.Au cours de l’infection à P. aeruginosa PAO1, le nombre de bactéries récupérées 24 heures après l’infection de la peau de rats brûlés était inférieur à l’inoculum initial de 1 par 10 à la 6e UFC. Cependant, il a augmenté 48 et 72 heures après l’infection. Chez S. aureus, infection à ATC25923, une augmentation de 2 log 10 a été observée à tous les points temporels de la peau par rapport à l’inoculum initial, ce qui suggère que l’établissement de l’infection à S. aureus était dû à la réplication active dans les tissus.Le tissu sous-cutané et les muscles présentaient une charge bactérienne plus élevée que le poumon et la rate. Les données ont montré que les rats brûlés ont développé une infection systémique 24 heures après l’inoculation de P. aeruginosa et 48 heures après l’inoculation de S. aureus. Enlevez complètement les poils sur le dos du rat et maintenez la température et le temps d’induction de la brûlure comme mentionné dans le protocole.Les cheveux restants et la température différente des tiges de cuivre peuvent affecter la qualité de la brûlure. Le modèle de brûlure chez le rat peut être exploré pour évaluer de nouveaux traitements topiques pour le traitement des infections par brûlure et pour évaluer différents pansements.
