TOETVA er karakteriseret ved fuld eksponering og dissektion af de centrale lymfeknuder, hvilket er signifikant fordelagtigt ved behandling af differentieret skjoldbruskkirtelkarcinom med cN1a10,13,14,15. Konventionel åben thyroidektomi er blevet udført sikkert ved hjælp af et cervikalt snit i mere end 100 år1. Selvom de fleste patienter har effektiv arheling, og den kosmetiske effekt generelt er acceptabel, tiltrækker et permanent ar på halsen altid øjeblikkelig opmærksomhed fra almindelige observatører2. Næsten 20% af post-thyroidektomi patienter oplever selvbevidsthed, og mere end 10% overvejer yderligere behandlinger for at fjerne arret3. Desuden er der også rapporteret om en negativ indvirkning af det cervikale snit på sundhedsrelateret livskvalitet (HRQOL)4. Forskellige fjernadgangsmetoder til skjoldbruskkirtelkirurgi, såsom axillobreast, transaxillære, retroauriculære og subklaviske tilgange, er blevet udviklet for at undgå et synligt halsar 5,6,7,8 ved at flytte det kutane snit til mindre iøjnefaldende steder. 9 Disse tilgange kræver imidlertid dissektion med bred klap for at få adgang til skjoldbruskkirtlen og efterlader stadig kutane ar på snitstederne10.
Siden 2008 er teknikker til transluminal endoskopisk kirurgi med naturlig åbning til transoral skjoldbruskkirtelkirurgi blevet udviklet. Disse kan udføres via den orale vestibulære tilgang eller den sublinguale tilgang. Førstnævnte er mere populær, fordi den er forbundet med færre komplikationer. I 2016 offentliggjorde Anuwong den første caseserie på 60 patienter, der gennemgik den transorale endoskopiske thyroidektomi vestibulære tilgang (TOETVA) og identificerede en fremragende prognose11. I sammenligning med fjernadgangsmetoderne betragtes TOETVA som virkelig minimalt invasiv, fordi området med klapdissektionen ligner konventionel åben thyroidektomi, og det efterlader ingen ar på kroppen10.
TOETVA, en revolutionerende endoskopisk metode, opfylder kvinders kosmetiske behov og giver nem adgang til den bilaterale skjoldbruskkirtel og det centrale rum12. Det kendetegnes ved fuldstændig eksponering og dissektion af de centrale lymfeknuder, hvilket er gavnligt ved behandling af differentieret skjoldbruskkirtelcancer med cN1a10,13,14,15. transoral endoscopic thyroidectomy vestibular approach for thyroid lobectomy

Manuskriptet beskriver den transorale endoskopiske thyroidektomi vestibulære tilgang (TOETVA) til thyroid lobektomi. Patienten placeres i liggende stilling med forlængelse og fiksering af nakken. Et 20 mm tværgående snit og to 5 mm snit foretages gennem slimhinden i den orale vestibule til kamera- og instrumentplacering efter desinfektion af hud og mundhule. Arbejdsområdet etableres og vedligeholdes af hudophængningsanordningen, som er lavet af uabsorberbar snor (3-0) og elastikker samt CO2 -insufflationstrykket. Lobectomi ved hjælp af en medial til lateral teknik og profylaktisk ipsilateral central halsdissektion udføres samtidigt på patienter med papillær skjoldbruskkirtelcancer (PTC). Prøven ekstraheres gennem 20 mm snittet. Parathyroidkirtlen søges straks efter i prøven og transplanteres automatisk til venstre brachioradialis. Et drænrør indsættes gennem tilbagetrækningshullet i skjoldbruskkirtlen, og absorberbare suturer bruges til at lukke slimhindesnittene i den orale vestibule og linea alba cervicalis. Profylaktika administreret intravenøst anbefales i de første 24 timer efter operationen, og orale antibiotika anvendes i 7 dage postoperativt.

Vi etablerede et rutinemæssigt klinisk forløb for patienter med TOETVA i centrum. Laryngoskopi og en skjoldbruskkirtel ultralyd udføres på hver patient før operationen. Parathyroideahormon (PTH), skjoldbruskkirtelfunktion, 25-dihydroxy D-vitamin (25-OH-VD) og serumcalcium måles rutinemæssigt før operationen, og alle undtagen skjoldbruskkirtelfunktionen måles igen 1 dag efter operationen. På vores hospital anvendes Foley-katetre rutinemæssigt til patienter, der skal have en TOETVA-operation på mere end 3 timer...

TOETVA har nogle ulemper, herunder et smalt operationsområde og en forholdsvis udfordrende ydre gren af den overlegne larynxnerveeksponering. Denne protokol forbedrer anvendelsen af TOETVA IN klinisk praksis og reducerer indlæringskurven. TOETVA er karakteriseret ved fuld eksponering og dissektion af de centrale lymfeknuder, hvilket er signifikant fordelagtigt ved behandling af differentieret skjoldbruskkirtelkarcinom med cN1A.Efter bedøvelse patienten ved hjælp af en ultralydskalpel gør arbejdsområdet og udvider det til både sternocleidomastoid muskler lateralt og langs det subplatysmale plan til brysthakket ringere. Opdel linea alba colli med en ultralydskalpel og adskil en del af remmusklen fra skjoldbruskkirtlen. Brug derefter en 20 ml nål til at gennembore huden på niveauet af det ringere hjørne af skjoldbruskkirtlen og læg en trækkrog gennem hullet for at få fat i den adskilte del af remmusklen.Brug ultralydskalpellen til at adskille den resterende del af remmusklen og udsætte den fælles halspulsåre. Find vagusnerven og registrer signalet ved hjælp af intraoperativ neuroovervågning. Hold derefter isthmusens nedre margen ved hjælp af ikke-traumatiske tang.Brug derefter en ultralydskalpel til at skære isthmus fra den ringere del af cricoidbrusk og udsætte luftrøret. For lobektomi af Transoral Endoscopic Thyroidectomy Vestibular Approach, eller TOETVA, lokalisere luftrøret først og bruge det som reference. Brug ikke-traumatiske tang til at holde den øverste pol i skjoldbruskkirtlen og opdele cricothyroid-rummet med ultralydskalpellen.Transekt en del af sternothyroidmusklen for at udsætte skjoldbruskkirtlens øvre pol. Afskær den overlegne skjoldbruskkirtelarterie og vene for at frigøre laterale kirtler fra toppen ned. Løft den øverste pol af kirtlen og find den tilbagevendende larynxnerve eller RLN under direkte visualisering.Derefter adskilles skjoldbruskkirtlen kapsel fra luftrøret. Derefter opdele de ringere lårkar og skære Berrys ledbånd med ultralydskalpellen. Fjern den ensidige lap af skjoldbruskkirtlen fra luftrøret.Efter lokalisering af den ringere parathyroidkirtel, dissekeres de prætrakeale og prelarynxlymfeknuder ved hjælp af en ultralydskalpel. Skær bindevævet foran halspulsåren for at bestemme den laterale grænse for de centrale lymfeknuder. Afslut bindevævet foran luftrøret for at bestemme den mediale grænse for de centrale lymfeknuder.Fej den forreste side af venstre RLN eller den forreste og bageste side af højre RLN for at fuldføre dissektionen af den centrale lymfeknude. Fjern prøverne ved hjælp af endoskopiske poser. Vask det kirurgiske sår gentagne gange med varmt, sterilt destilleret vand.Placer et kirurgisk dræn gennem en lille hudpunktering i den forreste hals. Tilnærm remmusklerne igen. Luk til sidst de orale sår med 5-O absorberbare suturer.Baselinekarakteristika og repræsentative resultater af de 32 tilfælde af TOETVA og de 97 tilfælde af åben kirurgi er vist her. Patienternes gennemsnitsalder og kønssammensætning varierede mellem de to grupper. Yngre kvindelige patienter vælger TOETVA-metoden.Operationstiden for TOETVA var længere end åben kirurgi. Der blev ikke observeret signifikante forskelle i antallet af høstede lymfeknuder mellem de to grupper. Med hensyn til de metastatiske lymfeknuder varierede forholdet mellem ekstrathyroidal forlængelse i tumorstørrelse mellem de to grupper, hvilket betyder, at tumorernes egenskaber var forskellige mellem de to grupper.Der blev ikke fundet signifikante forskelle i postoperativt parathyroideahormon og postoperativt calcium mellem de to grupper. Når RLN er i nærheden af en blødning, bør man undgå at bruge en elektrotherm eller en ultralydskalpel til hæmostase uden at finde ud af RLN's anatomi og de omgivende forhold.
