従来の開腹甲状腺摘出術は、100年以上前から頸部切開を用いて安全に行われてきました1。ほとんどの患者は効率的な瘢痕治癒があり、美容効果は一般的に許容されますが、首の永久的な瘢痕は常に一般的な観察者からすぐに注意を引きます2。甲状腺摘出術後の患者のほぼ20%が自己認識を経験し、10%以上が瘢痕を取り除くためのさらなる治療を検討しています3。さらに、子宮頸部切開が健康関連の生活の質(HRQOL)に及ぼす悪影響も報告されています4。腋窩乳房、経腋窩、耳介後、鎖骨下アプローチなど、甲状腺手術のためのさまざまなリモートアクセスアプローチが開発され、皮膚切開部を目立たない場所に移動することにより、目に見える首の傷跡5,6,7,8を回避できます。9ただし、これらのアプローチでは、甲状腺にアクセスするためにワイドフラップ解剖が必要であり、切開部位に皮膚瘢痕が残ります10。
2008年以来、経口腔甲状腺手術のための自然開口部経管内視鏡手術のための技術が開発されてきた。これらは、口腔前庭アプローチまたは舌下アプローチ を介して 実行できます。前者は、合併症が少ないため、より人気があります。2016年、Anuwongは、経口腔内視鏡的甲状腺摘出術前庭アプローチ(TOETVA)を受けている60人の患者の最初の症例シリーズを発表し、優れた予後を特定しました11。リモートアクセス法と比較して、TOETVAは、皮弁解剖の領域が従来の開放甲状腺摘出術に類似しており、身体に傷跡を残さないため、真に低侵襲であると考えられています10。
革新的な内視鏡法であるTOETVAは、女性の美容ニーズを満たし、両側甲状腺と中央コンパートメントに簡単にアクセスできます12。それは、cN1a10,13,14,15による分化型甲状腺癌の治療に有益である中枢リンパ節の完全な曝露および解剖によって区別される。しかし、手術スペースが限られているため、甲状腺の上部極にある大きな腫瘍に対処することは比較的困難です。現在の研究では、TOETVAの段階的な手順について説明しています。

transoral endoscopic thyroidectomy vestibular approach for thyroid lobectomy

原稿には、甲状腺葉切除術に対する経口腔内視鏡的甲状腺摘出術前庭アプローチ(TOETVA)が記載されています。患者は首を伸ばして固定しながら仰臥位に置かれる。皮膚と口腔の消毒後にカメラと器具を配置するために、1つの20mm横切開と2つの5mm切開が口腔前庭の粘膜を通して行われます。ワークスペースは、吸収不可能なひも(3-0)と輪ゴムで作られた皮膚懸濁装置と、CO2 気腹圧によって確立および維持されます。甲状腺乳頭癌(PTC)の患者に対して、内側から外側への技術を使用した葉切除術と予防的な同側中心頸部郭清術が同時に行われます。標本は20mmの切開を通して抽出されます。副甲状腺は標本内で直ちに検索され、左腕橈骨に自己移植されます。レナージチューブを開創器孔を通して甲状腺の床に挿入し、吸収性縫合糸を使用して口腔前庭および頸部静脈の粘膜切開を閉じる。術後最初の24時間は静脈内投与される予防薬が推奨され、術後7日間は経口抗生物質が使用されます。
