TOETVA er karakterisert ved full eksponering og disseksjon av sentrale lymfeknuter, noe som er betydelig fordelaktig ved behandling av differensiert tyreoideakarsinom med cN1a10,13,14,15. Det skal imidlertid bemerkes at på grunn av det begrensede operasjonsområdet er det relativt vanskelig å håndtere de store svulstene som ligger i skjoldbruskkjertelens øvre pol. Konvensjonell åpen tyreoidektomi har blitt trygt utført ved hjelp av et cervikalt snitt i mer enn 100 år1. Selv om de fleste pasienter har effektiv arrheling og den kosmetiske effekten generelt er akseptabel, trekker et permanent arr på nakken alltid umiddelbar oppmerksomhet fra vanlige observatører2. Nesten 20% av post-thyroidectomy pasienter opplever selvbevissthet, og mer enn 10% vurdere ytterligere behandlinger for å fjerne arr3. Videre er det også rapportert en negativ innvirkning av cervikalsnittet på helserelatert livskvalitet (HRQOL)4. Siden 2008 har teknikker for naturlig åpning transluminal endoskopisk kirurgi for transoral skjoldbrusk kirurgi blitt utviklet. Disse kan utføres via den orale vestibulære tilnærmingen eller den sublinguale tilnærmingen. Førstnevnte er mer populær fordi den er forbundet med færre komplikasjoner. I 2016 publiserte Anuwong den første saksserien på 60 pasienter som gjennomgikk transoral endoskopisk tyreoidektomi vestibulær tilnærming (TOETVA) og identifiserte en utmerket prognose11. I sammenligning med fjerntilgangsmetodene, anses TOETVA som virkelig minimalt invasiv fordi området av klaffdisseksjonen ligner konvensjonell åpen skjoldbruskektomi, og det etterlater ingen arr på kroppen10.
TOETVA, en revolusjonerende endoskopisk metode, oppfyller kvinners kosmetiske behov og gir enkel tilgang til den bilaterale skjoldbruskkjertelen og det sentrale rommet12. Det preges av fullstendig eksponering og disseksjon av sentrale lymfeknuter, noe som er gunstig ved behandling av differensiert skjoldbruskkjertelkreft med cN1a10,13,14,15. transoral endoscopic thyroidectomy vestibular approach for thyroid lobectomy

Manuskriptet beskriver transoral endoskopisk tyreoidektomi vestibulær tilnærming (TOETVA) for thyroid lobektomi. Pasienten er plassert i liggende stilling med forlengelse og fiksering av nakken. Ett 20 mm tverrsnitt og to 5 mm snitt gjøres gjennom slimhinnen i munnvestibylen for kamera- og instrumentplassering etter desinfeksjon av hud og munnhule. Arbeidsområdet er etablert og vedlikeholdt av hudopphengsanordningen, som er laget av uabsorberbar streng (3-0) og gummibånd, og CO2 insufflasjonstrykket. Lobektomi med medial-til-lateral teknikk og profylaktisk ipsilateral sentral halsdisseksjon utføres samtidig hos pasienter med papillær tyreoideakreft (PTC). Prøven ekstraheres gjennom 20 mm snittet. Biskjoldbruskkjertelen blir umiddelbart søkt etter i prøven og automatisk transplantert til venstre brachioradialis. Et dreneringsrør settes inn gjennom tilbaketrekkingshullet i skjoldbruskkjertelen, og absorberbare suturer brukes til å lukke slimhinnesnittene i oral vestibul og linea alba cervicalis. Profylaktika administrert intravenøst anbefales de første 24 timene etter operasjonen, og orale antibiotika brukes i 7 dager postoperativt.

Vi setter opp et rutinemessig pasientforløp for pasienter med TOETVA på senteret. Laryngoskopi og skjoldbrusk ultralyd utføres på hver pasient før operasjonen. Paratyreoideahormon (PTH), skjoldbruskfunksjon, 25-dihydroksyvitamin D (25-OH-VD) og serumkalsium måles rutinemessig før operasjonen, og alle unntatt skjoldbruskfunksjonen måles på nytt 1 dag etter operasjonen. Gjennomsnittlig alders- og kjønnssammensetning blant pasientene var forskjellig mellom de to gruppene. Yngre kvinnelige pasienter velger TOETVA-metoden.Operasjonstiden for TOETVA var lengre enn åpen kirurgi. Ingen signifikante forskjeller i antall høstede lymfeknuter ble observert mellom de to gruppene. Når det gjelder metastatiske lymfeknuter, var forholdet mellom ekstratyreoidal forlengelse i tumorstørrelse forskjellig mellom de to gruppene, noe som betyr at egenskapene til svulstene varierte mellom de to gruppene.Det ble ikke funnet signifikante forskjeller i postoperativt paratyreoideahormon og postoperativt kalsium mellom de to gruppene. Når RLN er i nærheten av blødning, bør bruk av en elektrotermm eller en ultralydskalpell for hemostase uten å finne ut anatomien til RLN og omgivelsene unngås.
