L’électromyographie a indiqué que la fréquence et l’amplitude des signaux électriques utérins changent au cours de la période gestationnelle 2,16,25. Plusieurs études ont exploré les schémas de propagation utérine des contractions utérines chez les patientes en travail actif 10,17,26,27,28.</s...

Cliniquement, les contractions utérines sont mesurées soit à l’aide d’un cathéter de pression intra-utérin, soit par tocodynamométrie1. Dans le cadre de la recherche, les contractions utérines peuvent être mesurées par électromyographie (EMG), dans laquelle des électrodes sont placées sur la surface abdominale pour mesurer les signaux bioélectriques générés par le myomètre 2,3,4,5,6,7. On peut utiliser les caractéristiques de magnitude, de fréquence et de propagation des sursauts électriques 8,9,10,11,12 dérivés de l’EMG pour prédire le début du travail chez les prématurés. Cependant, dans l’EMG conventionnel, l’activité électrique des contractions utérines n’est mesurée qu’à partir d’une petite région de la surface abdominale avec un nombre limité d’électrodes (deux13 et quatre 7,14,15,16 au centre de la surface abdominale et 64 17 à la surface abdominale inférieure). De plus, l’EMG conventionnel est limité dans sa capacité à étudier les mécanismes du travail, car il ne reflète que les activités électriques moyennes de l’ensemble de l’utérus et ne peut pas détecter les schémas spécifiques d’initiation et d’activation électriques sur la surface utérine pendant les contractions.
Un développement récent appelé imagerie électromyométriale (EMMI) a été introduit pour surmonter les lacunes de l’EMG conventionnel. EMMI permet une imagerie non invasive de l’ensemble de la séquence d’activation électrique du myomètre pendant les contractions utérines 18,19,20,21. Pour acquérir la géométrie corps-utérus, EMMI utilise l’imagerie par résonance magnétique (IRM) pondérée en T122,23,24, qui a été largement utilisée pour les femmes enceintes au cours de leurs deuxième et troisième trimestres. Ensuite, jusqu’à 192 électrodes de type broche placées à la surface du corps sont utilisées pour collecter les enregistrements électriques du myomètre. Enfin, le pipeline de traitement des données EMMI est réalisé pour combiner la géométrie corps-utérus avec les données électriques afin de reconstruire et d’imager les activités électriques sur la surface utérine21. EMMI peut localiser avec précision l’initiation des contractions utérines et les modèles de propagation de l’image pendant les contractions utérines en trois dimensions. Cet article a pour objectif de présenter les procédures EMMI et de démontrer les résultats représentatifs obtenus chez les femmes enceintes.

Au cours d’une grossesse normale, le muscle lisse utérin, le myomètre, commence à avoir des contractions faibles et non coordonnées à la fin de la gestation pour aider le col de l’utérus à se remodeler. Pendant le travail, le myomètre a des contractions fortes et coordonnées pour accoucher du fœtus. Diverses méthodes ont été développées pour surveiller les schémas de contraction utérine afin de prédire le début du travail. Cependant, les techniques actuelles ont une couverture spatiale et une spécificité limitées. La première étape de l’EMMI consiste à utiliser l’imagerie par résonance magnétique pondérée en T1 pour acquérir la géométrie corps-utérus spécifique au sujet. Ensuite, jusqu’à 192 électrodes de type broche placées à la surface du corps sont utilisées pour collecter les enregistrements électriques du myomètre. Enfin, le pipeline de traitement des données EMMI est réalisé pour combiner la géométrie corps-utérus avec les données électriques de la surface du corps afin de reconstruire et d’imager les activités électriques utérines sur la surface utérine. EMMI peut imager, identifier et mesurer de manière sûre et non invasive les régions d’activation précoce et les modèles de propagation dans l’ensemble de l’utérus en trois dimensions.

Notre recherche vise à développer une technologie d’imagerie non invasive appelée imagerie électromyométriale pour générer des images 3D en temps réel des contractions utérines chez l’homme. Notre travail permettra de mieux comprendre le mécanisme des contractions utérines pendant le travail humain. Notre objectif est de répondre à des questions majeures sur la gestion du travail humain et la prédiction de la prématurité et de l’arrêt du travail.Le modèle de corrélation de l’indice d’activation utérine dérivé d’EMMI, du rapport d’activation maximal et de la dilatation cervicale différait entre les groupes nullipares et multipares, ce qui suggère que la mémoire myométriale peut contribuer à une progression plus rapide du travail chez les patientes multipares que chez les patientes nullipares. Les courbes d’activation dérivées d’EMMI ont une nature d’évolution sigmoïde, reflétant potentiellement la dynamique de réponse à la stimulation bioélectrique pendant les contractions. Avec le système d’imagerie électromyométrial non invasif, nous sommes prêts à visualiser le travail humain, les contractions utérines, pour répondre à certaines questions fondamentales sur le travail humain, telles que où les contractions commencent, comment elles se propagent et où elles s’arrêtent.L’imagerie électromyométriale peut également fournir de multiples nouvelles mesures pour refléter la progression du travail humain. Nous prévoyons de rendre EMMI portable et peu coûteux pour des recherches approfondies en remplaçant l’IRM par des ultrasons et un système d’électrodes câblées coûteux par des patchs d’électrodes sans fil jetables. Cela accélérera notre travail sur la création d’un atlas à terme normal de l’EMMI.Cet atlas servira de point de référence pour l’étude de différentes maladies en définissant les contractions normales du travail.
