Um die aktuellen Probleme bei der Reparatur von Sehnen und Bändern anzugehen, ist ein entsprechender Zellkandidat erforderlich, der für zellbasierte Therapien geeignet ist. Ein Forschungsansatz im Bereich des Tissue Engineering für die Sehnenreparatur ist die Erforschung von aus dem Knochenmark stammenden mesenchymalen Stromazellen (BM-MSCs) und aus Fettgewebe gewonnenen Stromazellen (ASCs) als mögliche Strategien. Diese Zellen verfügen über multipotente Fähigkeiten, eine große Häufigkeit und ein regeneratives Potenzial in vivo. Darüber hinaus haben sie eine verbesserte Heilungskapazität und verbesserte funktionelle Ergebnisse in Tiermodellen gezeigt1. Nichtsdestotrotz weisen diese Zellen eine eingeschränkte Selbsterneuerungsfähigkeit, eine phänotypische Diversität und vor allem eine begrenzte Fähigkeit zur Sehnenbildung auf. Die Technologie der induzierten pluripotenten Stammzellen (iPSC) bietet aufgrund ihrer bemerkenswerten Selbsterneuerungsfähigkeit und ihrer unübertroffenen Entwicklungsanpassungsfähigkeit eine Lösung für diese Einschränkungen. Unserem Forschungsteam und anderen ist es gelungen, iPS-Zellen erfolgreich in mesenchymale stromazellähnliche Einheiten (iMSCs) zu differenzieren2,3. Daher haben iMSCs das Potenzial, eine allogene Quelle für Sehnenzelltherapieanwendungen zu sein.
Die Skleraxis (SCX) ist ein Transkriptionsfaktor, der für die Sehnenentwicklung essentiell ist und gilt als der früheste nachweisbare Marker für differenzierte Tenozyten. Darüber hinaus aktiviert SCX nachgeschaltete Sehnendifferenzierungsmarker, darunter Typ-1a1-Kettenkollagen 1 (COL1a1), Irokesenschnitt (MKX) und Tenomodulin (TNMD), unter anderem 4,5,6. Andere Gene, die während der Sehnenreifung exprimiert werden, sind das Tubulin-Polymerisations-fördernde Proteinfamilienmitglied 3 (TPPP3) und der Thrombozyten-abgeleitete Wachstumsfaktor-Rezeptor alpha (PDGFRa)7. Diese Gene sind zwar essentiell für die Entwicklung und Reifung von Sehnen, kommen aber leider nicht nur im Sehnengewebe vor, sondern werden auch in anderen Muskel-Skelett-Geweben wie Knochen oder Knorpel exprimiert 5,7.
Neben der Expression von Markern während der Sehnenentwicklung ist die Mechanostimulation ein wesentliches Element für die embryonale Sehnenentwicklung und -heilung 4,5,6. Sehnen sind mechanoresponsive und ihre Wachstumsmuster ändern sich als Reaktion auf ihre Umgebung. Auf molekularer Ebene beeinflussen biomechanische Signale die Entwicklung, Reifung, Erhaltung und Heilungsreaktionen von Tenozyten8. Verschiedene Bioreaktorsysteme wurden verwendet, um physiologische Belastungen und biomechanische Signale zu modellieren. Einige dieser Modellsysteme umfassen ex vivo Gewebebeladung, 2D-Zellladesysteme, die biaxiale oder uniaxiale Spannung anwenden, und 3D-Systeme mit Gerüsten und Hydrogelen 9,10. 2D-Systeme sind vorteilhaft, wenn es darum geht, die Auswirkungen der mechanischen Stimulation auf sehnenspezifische Gene oder die Morphologie der Zellen im Zusammenhang mit dem Zellschicksal zu untersuchen, während 3D-Systeme Zell-EZM-Interaktionen genauer replizieren können 9,10.
Bei 2D-Ladesystemen ist die Spannung zwischen den Zellen und dem Kultursubstrat homogen, was bedeutet, dass die Belastung des Zytoskeletts der Zellen vollständig kontrolliert werden kann. Im Vergleich zur biaxialen Belastung ist die einachsige Belastung physiologisch relevanter, da Tenozyten in vivo überwiegend einer einachsigen Belastung durch Kollagenbündel ausgesetzt sind 9. Es wurde festgestellt, dass Sehnen bei täglichen Aktivitäten einer einachsigen Zugbelastung von bis zu 6 % Dehnungausgesetzt sind 11. Insbesondere haben frühere Studien gezeigt, dass eine Belastung im physiologischen Bereich von 4 % bis 5 % die tenogene Differenzierung fördert, indem die sehnenbezogene Markerexpression wie SCX und TNMD erhalten bleibt und die Kollagenproduktion erhöhtwird 9,10. Stämme von mehr als 10 % können traumatisch, aber nicht physiologisch relevant sein12,13.
Hier wird ein Protokoll vorgestellt, das den synergistischen Effekt der mechanischen und biologischen Stimulation berücksichtigt, der für die Bildung von Tenozyten essentiell sein kann. Wir beschreiben zunächst eine reproduzierbare Methode zur Induktion von iPS-Zellen in iMSCs durch kurzzeitige Exposition von Embryoidkörpern gegenüber Wachstumsfaktoren, die durch MSC-Oberflächenmarker mittels Durchflusszytometrie bestätigt wird. Anschließend beschreiben wir eine lentivirale Transduktionsmethode, um iMSCs so zu manipulieren, dass sie eine stabile Überexpression von SCX (iMSCSCX+) aufweisen. Für die weitere Zellreifung werden die iMSCSCX+ in Fibronektin-beschichtete Silikonplatten eingeschleust und mit dem CellScale MCFX-Bioreaktor einem optimierten uniaxialen Spannungsprotokoll unterzogen. Das tenogene Potenzial wurde durch die Beobachtung der Hochregulierung von frühen und späten Sehnenmarkern sowie der Kollagenablagerung bestätigt14. Diese Methode zur Generierung von iTenozyten ist ein Proof-of-Concept, der eine unbegrenzte, allogene Standardquelle für Sehnenzelltherapieanwendungen bieten könnte.

Die heutigen Herausforderungen in der Sehnen- und Bänderreparatur erfordern die Identifizierung eines geeigneten und wirksamen Kandidaten für eine zellbasierte Therapie zur Förderung der Sehnenregeneration. Mesenchymale Stromazellen (MSCs) wurden als potenzielle Tissue-Engineering-Strategie für die Sehnenreparatur untersucht. Obwohl sie multipotent sind und in vivo ein regeneratives Potenzial haben, sind sie in ihrer Selbsterneuerungsfähigkeit begrenzt und weisen eine phänotypische Heterogenität auf. Induzierte pluripotente Stammzellen (iPS-Zellen) können diese Einschränkungen aufgrund ihrer hohen Selbsterneuerungsfähigkeit und ihrer beispiellosen Entwicklungsplastizität umgehen. In der Tenozytenentwicklung ist die Skleraxis (Scx) ein entscheidender direkter molekularer Regulator der Sehnendifferenzierung. Darüber hinaus hat sich gezeigt, dass die Mechanoregulation ein zentrales Element ist, das die embryonale Sehnenentwicklung und -heilung steuert. Aus diesem Grund haben wir ein Protokoll entwickelt, um den synergistischen Effekt biologischer und mechanischer Stimulation zu erfassen, der für die Bildung von Tenozyten unerlässlich sein kann. iPS-Zellen wurden zu mesenchymalen Stromazellen (iMSCs) induziert und mittels Durchflusszytometrie mit klassischen mesenchymalen Stromazellmarkern charakterisiert. Als nächstes wurden die iMSCs mit Hilfe eines lentiviralen Vektors transduziert, um SCX stabil zu überexprimieren (iMSCSCX+). Diese iMSCSCX+ Zellen können durch einachsige Zugbelastung mit einem 2D-Bioreaktor zu iTenozyten weiter gereift werden. Die resultierenden Zellen wurden durch die Beobachtung der Hochregulierung von frühen und späten Sehnenmarkern sowie der Kollagenablagerung charakterisiert. Diese Methode zur Generierung von iTenozyten kann verwendet werden, um Forscher bei der Entwicklung einer potenziell unbegrenzten allogenen Zellquelle von der Stange für Sehnenzelltherapieanwendungen zu unterstützen.

Sehnenverletzungen sind eines der häufigsten Muskel-Skelett-Probleme in den USA, von dem jährlich schätzungsweise 33 Millionen Menschen betroffen sind. Unsere Forschung konzentriert sich auf die Entwicklung eines Zell- und Gewebe-Engineering-Ansatzes zur Reparatur von Sehnen, der die Patientenergebnisse drastisch verbessern kann. Zu den aktuellen Technologien, die zur Förderung der Sehnenforschung eingesetzt werden, gehört die Verwendung von Stammzellen aus verschiedenen Quellen wie induzierten pluripotenten Stammzellen in Kombination mit dem Einsatz von Bioreaktoren, gentechnischen Biomaterialien und anderen Gewebezüchtungstechniken, um die für die Sehne erforderlichen Umweltreize nachzuahmen.Unser Verständnis darüber, wie sich Sehnen von ihrem Ursprung an entwickeln, ist begrenzt. Es gibt mehrere bekannte Gene, die während der Sehnenreifung sehr stark exprimiert werden. Sie sind jedoch leider nicht nur im Sehnengewebe zu finden.Dies erschwert es den Forschern, eine erfolgreiche Sehnendifferenzierung standardisiert zu beurteilen. Ein Protokoll nutzt das bemerkenswerte IPC-Differenzierungspotenzial und das, was wir bisher über die Sehnenentwicklung wissen. Unser Ziel ist es, eine allogene Zellquelle für Sehnenzelltherapieanwendungen zu entwickeln.Unsere Labore konzentrierten sich auf die Aufklärung der Signalwege, die mit Sehnensulfat verbunden sind, um die Sehnenentwicklung besser zu verstehen. Wir wollen dieses Wissen in Kombination mit Tissue-Engineering-Technologien und Biomaterialien umsetzen, um effektivere Alternativen für die traditionelle Sehnenreparatur zu entwickeln.
