A. Herstellung einer Donor Kaninchen zur Isolierung von mesenchymalen Stammzellen (Chirurgie Zimmer) <ol><li> Die Zellen werden von männlichen New Zealand White (NZW) Kaninchen im Alter von 4 Monaten und ca. 3 kg Körpergewicht isoliert. </li><li> Induce Narkose durch Propofol (10 mg / kg Körpergewicht iv) und opfern mit Natrium-Pentobarbital (100 mg / kg Körpergewicht iv). </li><li> Shave Fell von Hinterbeine, Rücken und Bauch mit einem elektrischen Haarschneider und absaugen Fell. </li><li> Desinfizieren Sie die rasierte Fläche gründlich mit 70% Ethanol. </li><li> Mit stumpfen Pinzette, scharfe Schere (oder Skalpell) und Knochen-Fräser für Gewebe und Bänder. </li><li> Einen Einschnitt entlang der Schädelbasis Oberfläche des Beines und Kalb. </li><li> Reflect Haut und des subkutanen Gewebes durch entweder scharf oder stumpf. </li><li> Separate Muskeln und Bänder von Schienbein und Oberschenkelknochen. Halten Schnitten als dicht am Knochen wie möglich, um eine saubere Zerlegung machen. Nicht trennen Oberschenkelknochen von Tibia zu diesem Zeitpunkt. </li><li> Cut through das Hüftgelenk, um den Kopf des Oberschenkelknochens aus der Gelenkpfanne zu trennen. </li><li> Elevate die Tibia-Femur komplex. </li><li> Verwenden Sie das Skalpell abkratzen alle verbleibenden Weichteile von den Knochen oder reiben die Knochen mit sterilen Tuch Gewebe. Flushing von Kaninchen MSC aus Knochen und Expansion (Cell Culture Hood) <ol><li> Sammle Knochen von Rohren und legen Sie sie in 150 mm-Schalen mit einer sterilen Pinzette. </li><li> Entfernen Sie beide Knochen endet mit einer sterilen Säge und bewegen, um neue Stücke 150 mm Gerichte. </li><li> Füllen Sie eine 10 ml Spritze mit medium (DMEM), legen eine 18-Gauge-Nadel und stecken Sie in die Öffnung des Knochenmarks. </li><li> Dann spülen Markhöhle mit mittlerer bis das Knochenmark in die Schüssel zu spülen. Anschließend, aus dem anderen Ende spülen, wenn möglich. Falls erforderlich, absägen mehr von den Enden. Wenn Knochen brechen sollte, nur spülen Sie das Innere des Knochens. </li><li> Saugen Zellmedium Suspension in der Spritze und spülen Sie das Knochenmark so oft, bis Suspension frei schwimmenden durch das Knochenmark Hohlraum und ohne weitere Knochenmark-Klumpen erscheinen. </li><li> Sobald Knochenmark wurde aus allen Knochen gesammelt worden, die Knochenmark Klumpen, indem sie durch eine 18-Gauge-Nadel stören: füllen Spritze mit Nadel und verdrängen in Medium. </li><li> Danach, filtern die Suspension durch einen Filter in eine Zelle 50 ml Tube. Um Zellen zu verhindern, waschen Kulturschale 2x mit 10 ml Medium und zu filtern sowie. </li><li> Zentrifuge Suspension bei 500 xg für 5 min bei RT. </li><li> Überstand entfernen und resuspendieren Zelle Pellet in 10 ml Medium (DMEM + 1% Pen / Strep). </li><li> Separate Blutzellen aus peripheren mononukleären Blutzellen (PBMC) und mesenchymale Stammzellen (MSC) mit einem Biocoll Trennlösung. </li><li> Füllen Sie 5 ml Biocoll Trennlösung in einen 15-ml-Tube und vorsichtig 5 ml Zell-Suspension auf und Zentrifuge bei 800 xg für 20 min bei RT (ohne Bremse). </li><li> Mögliche Ergebnisse siehe Abbildung 1: Sein dichter als Biocoll, rote Blutkörperchen Sediment auf den Boden, während PBMC und mesenchymalen Stammzellen an der Schnittstelle zu bleiben. </li><li> Entfernen Sie vorsichtig Schnittstelle in einen 15-ml-Tube und wäscht mit 5 ml PBS. </li><li> Zentrifuge wie in Schritt 22 beschrieben, resuspendieren in 5 ml PBS und 2-3x wiederholen. </li><li> Dann wieder Zentrifuge bei 350 xg für 10 min bei RT (mit Bremse). </li><li> Resuspendieren in 10 ml Medium und zählen Zellen in einer Zählkammer. </li><li> Teller Zellen bei einer anfänglichen Aussaatdichte von ca. 5 x 10 6 in 150 mm-Schalen. </li><li> Nach 2-3 Tagen wiederbewegen nicht haftenden Zellen. Möglicherweise müssen Sie mit PBS erste Spülung, um Zelltrümmer zu entfernen. In frischem Vollmedium (DMEM + 10% Fetal Calf Serum (FCS) + 1% Pen / Strep) danach. </li><li> Füttern Zellen alle 3-4 Tage (Abbildung 2). </li><li> Nach 5-10 Tagen Durchgang die Zellen für die erste Zeit. </li></ol> C. Herstellung von Fibringerinnseln in vitro <ol><li> Am Tag der Implantation, lassen anhaftenden Zellen aus den Kolben / Geschirr von einem 3-min Exposition gegenüber 0,25% Trypsin-EDTA. Stoppen Trypsinierung indem Vollmedium. </li><li> Verteilen Sie Zellen in einem 50 ml Falcon-Röhrchen und waschen Sie sie zweimal mit PBS. </li><li> Bestimmen Sie die Lebensfähigkeit der Zellen und Zahlen von Trypanblaufärbung. </li><li> In 50.000 Zellen / Mikrozentrifugenröhrchens und sammeln Pellets durch Zentrifugation bei 500 xg für 5 min bei RT. Bereiten Sie eine mastermix für mindestens eine weitere Gerinnsel nach Bedarf. </li><li> Zellpellet in 17 ul PBS und mischen Sie 25 ul der Fibrinogen-Komponente von TISSUCOL-Kit mit diesem 17 ul MSC Suspension. </li><li> Nehmen Sie eine sterile Platte mit Vorbohrungen (3x3.6 mm) in Übereinstimmung mit den Bohrungen in vivo (Abbildung 3). </li><li> Zuerst impfen 4 ul Thrombin-Lösung (500 IU / ml) in einem Loch, durch sofortige Zugabe von 42 ul Fibrinogen-Zell-Suspension und wieder 4 ul Thrombin-Lösung an der Spitze gefolgt. Mischen Sie nicht die Aussetzung zu vermeiden Blutgerinnung in der Pipettenspitze. Zunächst wird die 50 ul Volumen des pipettierten Fibrinogen-Zell-Suspension der Rand der vorgebohrten Löcher ohne Schmelzen aufgrund der Oberflächenspannung vorstehen. Jedoch nach vollständiger Gerinnung (nach 60 min) das Gerinnsel zusammenzieht und passt in das vorgebohrte Loch. </li><li> Entfernen Sie das Gerinnsel vorsichtig mit einem stumpfen Pinzette und in einen Mikrozentrifugenröhrchens mit PBS. Bereiten 2 Blutgerinnsel / Tier. </li><li> Nehmen Sie die Klumpen an den Operationssaal. </li></ol> D. Implantation von allogenen mesenchymalen Stammzellen in Fibringerinnseln<ol><li> Induce Anästhesie, Kaninchen (NZW, männlich, 3,5-4,0 kg Körpergewicht, 5-6 Monate alt) durch iv-Injektion von Propofol (10 mg / kg Körpergewicht). </li><li> Rasur das Knie auf mit einem elektrischen Haarschneider und Vakuum das Fell betrieben werden. Alle Verfahren genannt werden, bevor in einer Operation Herstellung Raum, um eine Verunreinigung der sterilen Umgebung des Operationssaals vermeiden durchgeführt. </li><li> Nach der Intubation Aufrechterhaltung der Narkose mit 1,5 mg / kg / min Propofol und 0,05 mg / kg / min Fentanyl intravenös. Überwachen Anästhesie mit Kapnographie, Pulsoximetrie und Pulsfrequenz. </li><li> Desinfizieren Sie die Knie rasiert gründlich und decken den Rest der Kaninchen mit einem sterilen Verband. </li><li> Ertasten Sie die Patella und führen Sie einen Hautschnitt medial der Patella. </li><li> Öffnen Sie das Kniegelenk durch eine medial parapatellaren Arthrotomie unter sterilen Bedingungen. Versuchen Sie zu vermeiden Schneiden alle kleinen oberflächlichen Blutgefäße. </li><li> Anschließend verdrängt man das Patella seitlich (Abbildung 4). </li><li> Nach InSpektion des Kniegelenks für jegliche begleitende Knorpelschäden oder gemeinsamen Anomalien, erstellen Sie zwei osteochondralen Defekten (3 mm tief, Achter-Form) in der Trochleagrube mit steriler Luft Betriebsleistung Bohrer (3,6 mm Durchmesser) mit einem Stopp-Gerät (Abbildung 5). </li><li> Reinigen Sie die Mängel und spülen Sie sie mit steriler Kochsalzlösung. </li><li> Vor der Implantation füllen 20 ul Fibrinkleber in die Defekte und verteilt sie gleichmäßig auf dem Boden der fehlerhaften Stelle. </li><li> Dann implantieren Blutgerinnsel im Preßsitz in der Acht-förmigen Defekt. </li><li> Nach Gerinnung, verlagern die Kniescheibe im Trochleagrube und beugen und strecken das Knie ein paar mal. </li><li> Anschließend verdrängt man das Patella seitlich noch einmal und überprüfen Sie, ob Fibrinogen-Zell-Klumpen sind noch an Ort und Stelle. </li><li> Ersetzen Sie die Patella wieder und beenden Sie den Vorgang mit Wundverschluss in Schichten mit einzelnen Knopf Nähte (4-0 Vicryl) und eine kontinuierliche Hautnaht (4-0 Monocryl) (beide mit resorbierbares NahtmaterialMaterial). </li><li> Schließlich versiegeln die Wunde mit einem Spray Dressing wasserdampfdurchlässig. </li><li> Für die postoperative Versorgung wird die Wunde täglich für 7 Tage überprüft. Die Kaninchen erhalten zur postoperativen Analgesie Carprofen 4 mg / kg sc alle 24 h (für 4 Tage) und Buprenorphin 0,03 mg / kg sc alle 12 h (2,5 Tage). Eine Stabilisierung des Knies (z. B. Verband) ist nicht erforderlich. </li></ol>

treatment of osteochondral defects in the rabbit's knee joint by implantation of allogeneic mesenchymal stem cells in fibrin clots

The treatment of osteochondral articular defects has been challenging physicians for many years. The better understanding of interactions of articular cartilage and subchondral bone in recent years led to increased attention to restoration of the entire osteochondral unit. In comparison to chondral lesions the regeneration of osteochondral defects is much more complex and a far greater surgical and therapeutic challenge. The damaged tissue does not only include the superficial cartilage layer but also the subchondral bone. For deep, osteochondral damage, as it occurs for example with osteochondrosis dissecans, the full thickness of the defect needs to be replaced to restore the joint surface 1. Eligible therapeutic procedures have to consider these two different tissues with their different intrinsic healing potential 2. In the last decades, several surgical treatment options have emerged and have already been clinically established 3-6.
Autologous or allogeneic osteochondral transplants consist of articular cartilage and subchondral bone and allow the replacement of the entire osteochondral unit. The defects are filled with cylindrical osteochondral grafts that aim to provide a congruent hyaline cartilage covered surface 3,7,8. Disadvantages are the limited amount of available grafts, donor site morbidity (for autologous transplants) and the incongruence of the surface; thereby the application of this method is especially limited for large defects.
New approaches in the field of tissue engineering opened up promising possibilities for regenerative osteochondral therapy. The implantation of autologous chondrocytes marked the first cell based biological approach for the treatment of full-thickness cartilage lesions and is now worldwide established with good clinical results even 10 to 20 years after implantation 9,10. However, to date, this technique is not suitable for the treatment of all types of lesions such as deep defects involving the subchondral bone 11.
The sandwich-technique combines bone grafting with current approaches in Tissue Engineering 5,6. This combination seems to be able to overcome the limitations seen in osteochondral grafts alone. After autologous bone grafting to the subchondral defect area, a membrane seeded with autologous chondrocytes is sutured above and facilitates to match the topology of the graft with the injured site. Of course, the previous bone reconstruction needs additional surgical time and often even an additional surgery. Moreover, to date, long-term data is missing 12.
Tissue Engineering without additional bone grafting aims to restore the complex structure and properties of native articular cartilage by chondrogenic and osteogenic potential of the transplanted cells. However, again, it is usually only the cartilage tissue that is more or less regenerated. Additional osteochondral damage needs a specific further treatment. In order to achieve a regeneration of the multilayered structure of osteochondral defects, three-dimensional tissue engineered products seeded with autologous/allogeneic cells might provide a good regeneration capacity 11.
Beside autologous chondrocytes, mesenchymal stem cells (MSC) seem to be an attractive alternative for the development of a full-thickness cartilage tissue. In numerous preclinical in vitro and in vivo studies, mesenchymal stem cells have displayed excellent tissue regeneration potential 13,14. The important advantage of mesenchymal stem cells especially for the treatment of osteochondral defects is that they have the capacity to differentiate in osteocytes as well as chondrocytes. Therefore, they potentially allow a multilayered regeneration of the defect.
In recent years, several scaffolds with osteochondral regenerative potential have therefore been developed and evaluated with promising preliminary results 1,15-18. Furthermore, fibrin glue as a cell carrier became one of the preferred techniques in experimental cartilage repair and has already successfully been used in several animal studies 19-21 and even first human trials 22.
The following protocol will demonstrate an experimental technique for isolating mesenchymal stem cells from a rabbit's bone marrow, for subsequent proliferation in cell culture and for preparing a standardized in vitro-model for fibrin-cell-clots. Finally, a technique for the implantation of pre-established fibrin-cell-clots into artificial osteochondral defects of the rabbit's knee joint will be described.
