Chronic pelvic pain in women due to hernias may be misdiagnosed by practicing clinicians. These fascial defects, their symptoms, physical findings, and proper treatment must be known in order to help women experiencing this form of chronic pelvic pain.

Hernias as a cause of chronic pelvic pain in women.

Among the multiple factors involved in the pathophysiology of heart disease, infections have been proposed to play a role in atherosclerosis with most of the available evidence implicating Chlamydia pneumonia, influenza virus and Mycoplasma pneumoniae. Based on a model case presentation, we speculate that in the absence of traditional risk factors and in the context of an ongoing respiratory infection caused by a pro-inflammatory pathogen (M. pneumoniae) along with a past positive serologic history for potentially proven atherogenic microorganism (C. pneumoniae) and infection may elicit potentially pathogenic events on vascular wall cells and leukocytes of atheromatous lesions, supporting the hypothesis that such infections may potentiate atherosclerotic cardiovascular disease (CVD).

Infectious atherosclerosis: is the hypothesis still alive? A clinically based approach to the dilemma.

This article presents the results of an anthropological and ethnobotanical study of the vegetable salts used by the Witoto Indians of the Amazon. It thoroughly documents the species used, the processing of the salts, their chemical composition and their anthropological, nutritional and medicinal relevance.

This paper provides evidence for the identification of the language of the uncontacted indigenous group called Carabayo, who live in voluntary isolation in the Colombian Amazon region. The only linguistic data available from this group is a set of about 50 words, most of them without reliable translations, that were collected in 1969 during a brief encounter with one Carabayo family. We compare this material with various languages (once) spoken in the region, showing that four attested Carabayo forms (a first person singular prefix and words for 'warm', 'father', and 'boy') display striking similarities with Yurí and at least 13 Carabayo forms display clear correspondences with contemporary Tikuna. Tikuna and Yurí are the only two known members of the Tikuna-Yurí linguistic family. Yurí was documented in the 19th century but has been thought to have become extinct since. We conclude that the Carabayo--directly or indirectly--descend from the Yurí people whose language and customs were described by explorers in the 19th century, before they took up voluntary isolation, escaping atrocities during the rubber boom in the early 20th century.

Due to the advances in screening of cirrhotic patients, hepatocellular carcinoma (HCC) is being diagnosed in earlier stages. For this reason the number of patients diagnosed of very early HCC (single tumors ≤ 2 cm) is continuously increasing. Once a patient has been diagnosed with this condition, treatment strategies include liver resection, local therapies or liver transplantation. The decision on which therapy should the patient undergo depends on the general patients performance status and liver disease. Anyway, even in patients with similar conditions, the best treatment offer is debatable. In this review we analyze the state of the art on the management of very early HCC on cirrhotic patients to address the best treatment strategy for this patient population.

Live Donor Liver Transplantation With Older (≥50 Years) Versus Younger (<50 Years) Donors: Does Age Matter?

Cholangiocarcinoma is a malignant tumor of the biliary system that can be classified into intrahepatic (iCCA), perihiliar (phCCA) and distal. Initial experiences with orthotopic liver transplantation (OLT) for patients with iCCA and phCCA had very poor results and this treatment strategy was abandoned. In the last decade, thanks to a strict selection process and a neoadjuvant chemoradiation protocol, the results of OLT for patients with non-resectable phCCA have been shown to be excellent and this strategy has been extended worldwide in selected transplant centers. Intrahepatic cholangiocarcinoma is a growing disease in most countries and can be diagnosed both in cirrhotic and in non-cirrhotic livers. Even though OLT is contraindicated in most centers, recent investigations analyzing patients that were transplanted with a misdiagnosis of HCC and were found to have an iCCA have shown encouraging results. There is some information suggesting that patients with early stages of the disease could benefit from OLT. In this review we analyze the current state-of-the-art of OLT for cholangiocarcinoma as well as the new insights and future perspectives.
