The aim of this study was to explore a new way of treating drug addiction by ablating the nucleus accumbens (NAC), which has a close relationship with drug-induced psychological dependence, using stereotactic surgery, blocking the mesocorticolimbic dopamine circuit, alleviating craving for drugs and lowering the relapse rate after detoxification. On the basis of animal experiments, stereotactic surgery was performed in 28 patients by making a lesion in the NAC bilaterally to treat opiate drug dependence. Indications, the criterion of therapeutic effect, treatment process and the therapeutic and safety evaluation index of the surgery were formulated particularly. The mean follow-up period was 15 months. Relapse has not occurred in 11 cases up till now. Drug-free time in these patients has been more than half a year in 4 cases (more than a year in 3 cases), and less than half a year in 7 cases. Relapse occurred in 15 cases after surgery. Drug-free time in these patients was more than half a year in 3 cases, between 1 month and half a year in 10 cases and less than 1 month in 2 cases. The therapeutic effect was excellent in 7 cases (26.9%), good in 10 cases (38.5%) and poor in 2 cases (7.7%). Another 7 cases were still under investigation at the time of writing. Relapse rates after surgery were 7.7, 38.5 and 57.5% within 1 month, between 1 month and half a year and after more than half a year, respectively. There were no common complications of surgery such as intracranial hematoma or infection in these patients after operation. Character type was changed slightly in 2 cases, and 4 cases suffered temporary memory loss, which did not affect their daily lives and learning function. They all recovered within 1 month. There were different degrees of effectiveness of treating drug addicts' psychological dependence by making lesions in the NAC bilaterally with stereotactic surgery. No particular complications occurred. The operation is safe and feasible. The mean follow-up time in this study was 15 months. The effectiveness was satisfactory. The relapse rate of drug addicts after detoxification was clearly reduced.

Clinical study for alleviating opiate drug psychological dependence by a method of ablating the nucleus accumbens with stereotactic surgery.

Sur la base de l&#39;exp&eacute;rimentation animale, la chirurgie st&eacute;r&eacute;otaxique a &eacute;t&eacute; r&eacute;alis&eacute;e chez 28 patients en r&eacute;alisant une l&eacute;sion dans le CNA bilat&eacute;ralement pour traiter la d&eacute;pendance aux drogues opiac&eacute;es. Drug-free time chez ces patients a &eacute;t&eacute; plus d&#39;une demi-ann&eacute;e dans 4 cas (plus d&#39;un an dans 3 cas) et moins d&#39;une demi-ann&eacute;e dans 7 cas. Rechute a eu lieu dans 15 cas, apr&egrave;s la chirurgie. Temps sans drogue chez ces patients a &eacute;t&eacute; plus d&#39;une demi-ann&eacute;e dans 3 cas, entre 1 mois et une demi-ann&eacute;e dans 10 cas et moins de 1 mois dans 2 cas. L&#39;effet th&eacute;rapeutique a &eacute;t&eacute; excellent dans 7 cas (26,9 %), bon dans 10 cas (38,5 %) et les pauvres dans 2 cas (7,7 %). Un autre 7 cas &eacute;taient encore &agrave; l&#39;&eacute;tude au moment de l&#39;&eacute;criture. Les taux de rechute apr&egrave;s chirurgie &eacute;taient 7,7, 38,5 et 57,5 % au cours du mois, entre 1 mois et demi par an et apr&egrave;s plus de six mois, respectivement. Il n&#39;y a aucune complication commune de chirurgie telles que h&eacute;matome intracr&acirc;nien ou infection chez ces patients apr&egrave;s l&#39;op&eacute;ration. Type de caract&egrave;re a &eacute;t&eacute; l&eacute;g&egrave;rement modifi&eacute; dans 2 cas et 4 cas a subi des pertes de m&eacute;moire temporaires, qui n&#39;a pas affect&eacute; leur vie quotidienne et l&#39;apprentissage. Ils ont tous r&eacute;cup&eacute;r&eacute;s au cours du mois. Il y a diff&eacute;rents degr&eacute;s d&#39;efficacit&eacute; de traitement de la d&eacute;pendance psychologique aux toxicomanes en faisant des l&eacute;sions dans le CNA bilat&eacute;ralement avec la chirurgie st&eacute;r&eacute;otaxique. Aucune complication particuli&egrave;re s&#39;est produite. L&#39;op&eacute;ration est s&ucirc;re et r&eacute;alisable. Lo scopo di questo studio era quello di esplorare un nuovo modo di trattare la tossicodipendenza di ablazione del nucleus accumbens (NAC), che ha una stretta relazione con la dipendenza psicologica farmaco-indotta, usando la chirurgia stereotassica, bloccando il circuito della dopamina mesocorticolimbic, alleviare craving per farmaci e abbassare il tasso di ricaduta dopo la disintossicazione. Sulla base di esperimenti sugli animali, chirurgia stereotassica &egrave; stata eseguita in 28 pazienti facendo una lesione nel NAC bilateralmente per il trattamento di tossicodipendenza da oppiacei. Indicazioni, il criterio dell&#39;effetto terapeutico, processo di trattamento e l&#39;indice di valutazione terapeutica e la sicurezza della chirurgia sono stati formulati particolarmente. Il periodo di follow-up medio &egrave; stato di 15 mesi. Ricaduta non si &egrave; verificato in 11 casi fino ad ora. Tempo libero dalla droga in questi pazienti &egrave; stato pi&ugrave; di un semestre in 4 casi (pi&ugrave; di un anno in 3 casi) e meno di un semestre in 7 casi. Ricaduta si &egrave; verificato in 15 casi dopo l&#39;intervento chirurgico. L&#39;effetto terapeutico &egrave; stato eccellente in 7 casi (26,9%), buono in 10 casi (38,5%) e poveri in 2 casi (7,7%). Un altro 7 casi erano ancora sotto inchiesta al momento della scrittura. Tariffe di ricaduta dopo l&#39;intervento chirurgico erano 7,7, 38,5 e 57.5% entro 1 mese, tra 1 mese e un anno e mezzo e dopo pi&ugrave; di mezzo anno, rispettivamente. C&#39;erano complicazioni comuni della chirurgia come ematoma intracranico o di infezione in questi pazienti dopo l&#39;operazione. Tipo di carattere &egrave; stato leggermente modificato in 2 casi, e 4 casi ha sofferto la perdita di memoria temporanea, che non incidano sulla loro vita quotidiana e la funzione di apprendimento. Tutti hanno recuperato entro 1 mese. Ci sono stati diversi gradi di efficacia del trattamento della dipendenza psicologica dei tossicodipendenti facendo lesioni nel NAC bilateralmente con chirurgia stereotassica. Si &egrave; verificato senza particolari complicazioni. L&#39;operazione &egrave; fattibile e sicuro. Il tempo medio di follow-up in questo studio &egrave; stato di 15 mesi. L&#39;efficacia &egrave; stata soddisfacente. Il tasso di ricaduta dei tossicodipendenti dopo la disintossicazione &egrave; stato chiaramente ridotto.

El objetivo de este estudio fue explorar una nueva forma de tratar la adicci&oacute;n a las drogas por ablaci&oacute;n el n&uacute;cleo accumbens (NAC), que tiene una estrecha relaci&oacute;n con la dependencia psicologica medicamentosa, mediante cirug&iacute;a estereot&aacute;xica, bloqueando el circuito mesocorticolimbic de dopamina, aliviar el ansia de drogas y reducir la tasa de reca&iacute;da despu&eacute;s de la desintoxicaci&oacute;n. Sobre la base de los experimentos con animales, cirug&iacute;a estereot&aacute;ctica fue realizada en 28 pacientes haciendo una lesi&oacute;n en el NAC bilateral para tratar la dependencia de las drogas opi&aacute;ceas. Indicaciones, el criterio del efecto terap&eacute;utico, proceso de tratamiento y el &iacute;ndice de evaluaci&oacute;n terap&eacute;utico y seguridad de la cirug&iacute;a fueron formulados especialmente. El per&iacute;odo de seguimiento promedio fue de 15 meses. Reca&iacute;da no ha ocurrido en 11 casos hasta ahora. Tiempo libre de drogas en estos pacientes ha sido de m&aacute;s de medio a&ntilde;o en 4 casos (m&aacute;s de un a&ntilde;o en 3 casos) y menos de medio a&ntilde;o en 7 casos. Ocurri&oacute; en 15 casos de reca&iacute;da despu&eacute;s de la cirug&iacute;a. Tiempo libre de drogas en estos pacientes fue de m&aacute;s de medio a&ntilde;o en 3 casos, entre 1 mes y medio a&ntilde;o en 10 casos y menos de 1 mes en 2 casos. El efecto terap&eacute;utico fue excelente en 7 casos (26,9%), buena en 10 casos (38.5%) y pobres en 2 casos (7,7%). Otros 7 casos estaban todav&iacute;a bajo investigaci&oacute;n en el momento de la escritura. Las tasas de reca&iacute;da despu&eacute;s de la cirug&iacute;a eran 7.7, 38.5 y 57,5% dentro de 1 mes, entre 1 mes y medio un a&ntilde;o y despu&eacute;s de m&aacute;s de medio a&ntilde;o, respectivamente. No hubo complicaciones comunes de la cirug&iacute;a como hematoma intracraneal o infecci&oacute;n en estos pacientes despu&eacute;s de la operaci&oacute;n. Tipo de car&aacute;cter cambi&oacute; ligeramente en 2 casos y 4 casos sufrieron p&eacute;rdida de memoria temporal, que no afect&oacute; su vida cotidiana y la funci&oacute;n de aprendizaje. Todos recuperaron dentro de 1 mes. Hubo diferentes grados de eficacia del tratamiento de dependencia psicologica de los adictos de drogas por medio de lesiones para el NAC bilateral con cirug&iacute;a estereot&aacute;ctica. Ningunas complicaciones particulares ocurrieron. La operaci&oacute;n es seguro y factible. El tiempo promedio de seguimiento en este estudio fue de 15 meses. La efectividad fue satisfactoria. La tasa de reca&iacute;da de adictos a las drogas despu&eacute;s de desintoxicaci&oacute;n se redujo claramente.

Basigin/CD147, also named extracelluar matrix metalloproteinase inducer (EMMPRIN), has been implicated in playing very important roles in several aspects of tumor progression. In this study, we examined the inhibitory effects of antisense RNA of CD147 on invasion and angiogenesis of human glioblastoma U251 cells in vitro. The U251 cell line was transfected by a plasmid containing antisense CD147 cDNA. Gelatin zymography was used to determine the effect on reducing secretions of MMP-2 and MMP-9 of the transfected cells. Boyden chamber was employed to test the invasion of U251 cells in vitro. We found that downregulation of CD147 resulted in reducing secretions of MMP-2, MMP-9, and VEGF. Moreover, the invasion of stable antisense transfectants was inhibited. Wound-induced migration assay also showed decreased migration in stable antisense transfectants compare to parental- and empty vector-transfected cells. Taken together, these results provide evidence that invasion of human glioblastoma cells can be inhibited by antisense RNA of CD147. Basigin/CD147 may be used as a potential target of drugs for anti-invasion and metastasis of human glioblastoma cells.

Basigin/CD147, anche chiamato induttore di extracelluar matrix metalloproteinase (EMMPRIN), &egrave; stato implicato in una partita molto importanti ruoli in diversi aspetti della progressione del tumore. In questo studio, abbiamo esaminato gli effetti inibitori di RNA antisenso di CD147 sull&#39;invasione e l&#39;angiogenesi di glioblastoma umano U251 cellule in vitro. La linea di cellulari U251 era transfected con un plasmide contenente antisenso CD147 cDNA. Gelatina zimogramma &egrave; stato utilizzato per determinare l&#39;effetto sulla riduzione delle secrezioni di MMP-2 e MMP-9 delle celle transfected. Previously, safety and immunogenicity of human papillomavirus type 16 (HPV16) or 18 E7-pulsed dendritic cells (DC) vaccinations were demonstrated in a dose-escalation Phase I clinical trial which enrolled ten patients diagnosed with stage IB or IIA cervical cancer (nine HPV 16-positive, one HPV 18-positive). The goal of the study was to define the T-cell epitopes of HPV 16 or 18 E7 protein in these patients in order to develop new strategies for treating HPV-associated malignancies. This was accomplished through establishing T-cell lines by stimulating peripheral blood mononuclear cells with autologous mature DC pulsed with the HPV 16 or 18 E7 protein, examining the T-cell responses using ELISPOT assays, and isolating E7-specific T-cell clones based on IFN-gamma secretion. Then, the epitope was characterized in terms of its core sequence and the restriction element. Twelve T-cell lines from eight subjects (seven HPV 16-positive, one HPV 18-positive) were evaluated. Positive T-cell responses were demonstrated in four subjects (all HPV 16-positive). All four were positive for the HPV 16 E7 46-70 (EPDRAHYNIVTFCCKCDSTLRLCVQ) region. T-cell clones specific for the E7 47-70 region were isolated from one of the subjects. Further analyses revealed a novel, naturally processed, CD4 T-cell epitope, E7 58-68 (CCKCDSTLRLC), restricted by the HLA-DR17 molecule.

Anteriormente, la seguridad y la inmunogenicidad de papiloma humano tipo 16 (HPV16) o 18 E7-pulsado las c&#xE9;lulas dendr&#xED;ticas (DC) las vacunas se ha demostrado en un ensayo de fase I de escalada de dosis estudio cl&#xED;nico que incluy&#xF3; a diez pacientes diagnosticados en estadio IB o II del c&#xE1;ncer cervicouterino (VPH nueve 16-positivo, un VPH 18-positivo). Entonces, el ep&#xED;topo se caracteriza en t&#xE9;rminos de su secuencia de n&#xFA;cleo y el elemento de restricci&#xF3;n. Doce l&#xED;neas de c&#xE9;lulas T de ocho sujetos (siete VPH 16-positivo, un VPH 18-positivo) fueron evaluados. Human papillomavirus (HPV)-specific T-cell response to the HPV type 16 (HPV16) E6 protein has been shown to be associated with successful viral clearance. The patterns of CD8 T-cell epitopes within HPV16 E6 protein were previously studied in two women with HPV16 clearance. The goal of this study was to characterize these epitopes in terms of their minimal and optimal amino acid sequences and the human leukocyte antigen (HLA) restriction molecules. The presence of the epitope-specific memory T cells after viral clearance was also examined. In subject A, the dominant epitope was characterized to be E6 75-83 (KFYSKISEY), restricted by the HLA-B62 molecule, while that of subject B was E6 133-142 (HNIRGRWTGR), restricted by the HLA-A6801 molecule. Homologous epitopes were identified in five other high-risk HPV types for both of these epitopes, but they were not recognized by respective T-cell clone cells. An enzyme-linked immunospot assay or tetramer analysis was performed on peripheral blood mononuclear cells from blood samples collected after viral clearance but prior to isolation of the T-cell clones. The presence of epitope-specific memory T cells was demonstrated. These data suggest that HPV-specific memory T cells were generated in vivo and that they may remain in circulation many months, if not years, after viral clearance. Our findings broaden the spectrum of the CD8 T-cell epitopes of the HPV16 E6 protein. The characterization of novel T-cell epitopes and long-lasting epitope-specific memory T cells may be useful for the development of a potential epitope-based vaccine.

Unsere Erkenntnisse erweitern das Spektrum der CD8-T-Zell-Epitope des Proteins HPV16 E6. Die Charakterisierung der Roman T-Zell-Epitope und langlebige Epitop-spezifische Ged&auml;chtniszellen T m&ouml;glicherweise n&uuml;tzlich f&uuml;r die Entwicklung von einem potenziellen Epitop-basierten Impfstoff.

T Ged&auml;chtniszellen spezifisch f&uuml;r neuartige humanes Papillomavirus geben 16 (HPV16) E6 Epitopes bei Frauen, deren HPV16-Infektion nicht nachweisbar geworden ist.

Papillomavirus umano (HPV)-risposta specifica delle cellule T alla proteina di HPV tipo 16 (HPV16) E6 ha dimostrato di essere associati con successo clearance virale. I modelli di cellule T CD8 epitopi all&#39;interno della proteina E6 HPV16 precedentemente sono stati studiati in due donne con clearance HPV16. L&#39;obiettivo di questo studio era di caratterizzare questi epitopi in termini di loro sequenze di aminoacidi ottimale e minimo e le molecole di restrizione human leukocyte antigen (HLA). La presenza delle cellule di memoria specifico epitopo T dopo clearance virale &egrave; stato anche esaminato. In un soggetto, l&#39;epitopo dominante &egrave; stato caratterizzato per essere E6 75-83 (KFYSKISEY), limitato dalla molecola HLA-B62, mentre quella del soggetto B era E6 133-142 (HNIRGRWTGR), limitato dalla molecola HLA-A6801. Omologhi epitopi sono stati identificati in cinque altri tipi HPV ad alto rischio per entrambi questi epitopi, ma essi non sono stati riconosciuti dai rispettive cellule clone di cellule T. Un&#39;analisi di enzyme-linked immunospot assay o tetramero &egrave; stata eseguita su cellule mononucleate di sangue periferico da campioni di sangue raccolti dopo la clearance virale, ma prima dell&#39;isolamento dei cloni di cellule T. &Egrave; stata dimostrata la presenza di cellule di memoria specifico epitopo T. Questi dati suggeriscono che le cellule di memoria T-specifici per HPV sono state generate in vivo e che pu&ograve; rimanere in circolazione molti mesi se non anni dopo la clearance virale. I nostri risultati di ampliare lo spettro delle cellule T CD8 epitopi della proteina E6 HPV16. Virus del papiloma humano (VPH)-espec&#xED;fica de c&#xE9;lulas T respuesta al tipo de VPH 16 (VPH16) E6 prote&#xED;na se ha demostrado que se asocia con la eliminaci&#xF3;n del virus con &#xE9;xito. Los patrones de c&#xE9;lulas T CD8-ep&#xED;topos dentro de HPV16 E6 prote&#xED;na fueron estudiados previamente en dos mujeres con VPH16 aclaramiento. El objetivo de este estudio fue caracterizar los ep&#xED;topos en t&#xE9;rminos de sus secuencias de &#xE1;cido m&#xED;nimos y &#xF3;ptimos amino&#xE1;cidos y los ant&#xED;genos leucocitarios humanos (HLA) las mol&#xE9;culas de restricci&#xF3;n. La presencia de las c&#xE9;lulas T espec&#xED;ficos para el ep&#xED;topo de memoria despu&#xE9;s de la eliminaci&#xF3;n del virus tambi&#xE9;n fue examinado. En sujeto A, el ep&#xED;topo dominante se caracteriza por ser E6 75-83 (KFYSKISEY), restringido por el HLA-B62 mol&#xE9;cula, mientras que la de sujeto B era E6 133-142 (HNIRGRWTGR), restringido por el HLA-A6801 mol&#xE9;cula. Hom&#xF3;loga ep&#xED;topos fueron identificados en otros cinco tipos de alto riesgo de HPV para ambos de estos ep&#xED;topes, pero no fueron reconocidos por las c&#xE9;lulas respectivas clones de c&#xE9;lulas T. Un ensayo enzim&#xE1;tico o inmunoensayo an&#xE1;lisis tetr&#xE1;mero se realiz&#xF3; en c&#xE9;lulas mononucleares de sangre perif&#xE9;rica a partir de muestras de sangre recogidas despu&#xE9;s de la eliminaci&#xF3;n del virus, pero antes del aislamiento de los clones de c&#xE9;lulas T. La presencia de ep&#xED;topos espec&#xED;ficos de las c&#xE9;lulas T de memoria se demostr&#xF3;. Estos datos sugieren que el VPH espec&#xED;ficos de las c&#xE9;lulas T de memoria se generaron en vivo y que pueden permanecer en los meses de circulaci&#xF3;n de muchos, si no a&#xF1;os, despu&#xE9;s de la eliminaci&#xF3;n del virus. Nuestros resultados ampliar el espectro de las c&#xE9;lulas T CD8-ep&#xED;topos de la prote&#xED;na E6 VPH16. Peripherally inserted central catheters (PICCs) have been used for many years in developed countries, but few studies have been focused on children with cancer in developing countries. In this study, we assessed the feasibility of PICCs and determined the rate of PICC-related complications in children with cancer. We prospectively followed all children with cancer over 3 years of age who received chemotherapy and PICC placement in our cancer center between June 2003 and May 2007. The date of last follow-up was January 31, 2008. A total of 119 PICCs were inserted into 116 patients during the 48-month period. PICCs were placed in 113 of 119 attempts, yielding an insertion rate of 95.0%. The 113 PICCs were in place for a total 26,721 catheter days (median time, 246 d; range, 8 to 455 d). The 113 PICCs had 53 overall complications, for a rate of 1.98/1000 catheter days. Twenty-one (18.6%) PICCs were removed because of complication with a rate of 0.79/1000 catheter days. The most common reason for PICC removal was breakage/leakage. An infection requiring PICC removal occurred in 4 patients. This study demonstrated relatively low complication rate and long duration for PICCs in children with cancer over 3 years of age in our hospital.

Cat&eacute;ter central insertado perif&eacute;ricamente (PICC) ha utilizado durante muchos a&ntilde;os en los pa&iacute;ses desarrollados, pero pocos estudios se han centrado en los ni&ntilde;os con c&aacute;ncer en los pa&iacute;ses en desarrollo. En este estudio, evalu&oacute; la viabilidad del PICC y determina la tasa de complicaciones relacionadas con el PICC en ni&ntilde;os con c&aacute;ncer. Seguimos prospectivamente a todos los ni&ntilde;os con c&aacute;ncer durante 3 a&ntilde;os de edad que recibieron quimioterapia y colocaci&oacute;n del PICC en nuestro centro de c&aacute;ncer entre junio de 2003 y mayo de 2007. La fecha de la carta recordativa pasada fue el 31 de enero de 2008. Un total de 119 PICC fueron insertados en 116 pacientes durante el per&iacute;odo de 48 meses. PICC se colocaron en 113 de 119 intentos, alcanzando una tasa de inserci&oacute;n de 95.0%. La 113 PICC estaban en lugar por d&iacute;as 26.721 cat&eacute;ter total (medio tiempo, 246 d; gama, d 8 a 455). El 113 PICC ten&iacute;a 53 complicaciones totales, con una tasa de d&iacute;as cat&eacute;ter de 1,98/1000. Veinti&uacute;n (18,6%) PICC fueron retirado debido a complicaciones con una tasa de 0,79/1000 d&iacute;as de cat&eacute;ter. La raz&oacute;n m&aacute;s com&uacute;n para el retiro del PICC fue rotura/de la salida. Una infecci&oacute;n requiriendo el retiro del PICC ocurri&oacute; en 4 pacientes. Este estudio demostr&oacute; tasa de complicaciones relativamente baja y larga duraci&oacute;n para el PICC en ni&ntilde;os con c&aacute;ncer durante 3 a&ntilde;os de edad en nuestro hospital.

Toll-like receptors (TLRs) initiate and maintain host defenses. These receptors play important roles in innate immunity and in various diseases. Different TLRs bind to diverse ligands that trigger distinct protein expression patterns. Few studies have focused on the interaction between different TLRs. We found that TLR2 priming downregulates TLR4 transcription, and expression of TLR4 activation induced major histocompatibility complex II (MHC II), adhesion molecule intercellular adhesion molecule-1 (ICAM-1), phagocytosis marker CD11b/CD18, and Fcgamma receptor (FcgammaR) expression. In contrast, TLR4 priming increases TLR2 transcription and expression. In addition, TLR4 priming increases secretion of certain proinflammatory mediators. Expression of costimulatory molecules CD80/CD86 increases with TLR2 or TLR4 activation sequences. Our results reveal that TLR2/TLR4 activation may determine disease pathogenesis and prognosis.

Abbiamo trovato che sottoregola adescamento TLR2 TLR4 trascrizione, e l&#39;espressione di TLR4 attivazione indotta istocompatibilit&agrave; complesso II (MHC II), adesione molecola adesione intercellulare molecola-1 (ICAM-1), indicatore di fagocitosi CD11b/CD18 e dell&#39;espressione dei recettori (FcgammaR) Fcgamma. Al contrario, TLR4 innesco aumenta TLR2 trascrizione e espressione. Inoltre, TLR4 innesco aumenta la secrezione di alcuni mediatori proinfiammatori. Espressione delle molecole di costimolazione CD80/CD86 aumenta con le sequenze di attivazione TLR2 o TLR4. I nostri risultati rivelano che attivazione TLR2/TLR4 pu&ograve; determinare la prognosi e la patogenesi della malattia.

Receptores Toll-like (TLRs) inician y mantienen las defensas del anfitri&oacute;n. Estos receptores juegan un papel importante en la inmunidad innata y en varias enfermedades. Nuestros resultados revelan que TLR2/TLR4 activaci&oacute;n puede determinar pron&oacute;stico y patog&eacute;nesis de la enfermedad.
