Articulatory discoordination is often said to be an important feature of the speech production disorder in dysarthria, but little experimental work has been done to identify and specify the coordination difficulties. The present study evaluated the coordination of labial and lingual gestures for /u/ production in persons with Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS), and in control participants. Both tongue backing/raising and reduction of the area enclosed by the lips can produce the characteristic low F2 of /u/. The timing of these articulatory gestures with respect to the acoustic target of a low F2 was inferred from X-ray microbeam data. Pellet motions of the tongue dorsum and lips revealed the timing of the lingual and labial gestures to be strongly linked together (synchronized), predictive of the temporal location of the lowest F2 within the vocalic nucleus, and scaled proportionately to the overall vowel duration in control participants. Somewhat surprisingly, essentially the same findings were obtained in the speakers with dysarthria. These relationships were noisier among the speakers with dysarthria, but the global synchronization patterns applied to all 3 groups. Further analyses revealed the synchronization to be less well defined and more variable across speakers with ALS, as compared to speakers with PD and the controls. Results are discussed relative to concepts of coordination in dysarthria.

Ces relations &eacute;taient plus bruyantes entre les enceintes avec dysarthrie, mais les mod&egrave;les de synchronisation globale appliqu&eacute;es &agrave; tous les groupes de 3. De nouvelles analyses ont r&eacute;v&eacute;l&eacute; la synchronisation &agrave; &ecirc;tre moins bien d&eacute;finis et plus variable dans les haut-parleurs &agrave; ALS, par rapport aux haut-parleurs avec PD et les contr&ocirc;les. Discoordination articolatorio &egrave; spesso detto di essere un&#39;importante caratteristica del disturbo di produzione vocale in disartria, ma poco lavoro sperimentale &egrave; stato fatto per identificare e specificare le difficolt&agrave; di coordinamento. Il presente studio ha valutato il coordinamento dei movimenti labiali e linguali per produzione /u/ in persone con malattia di Parkinson (PD), sclerosi laterale amiotrofica (SLA) e i partecipanti di controllo. Sia la lingua supporto/innalzamento e riduzione dell&#39;area racchiusa tra le labbra in grado di produrre il caratteristico basso F2 di/u /. La tempistica di questi gesti articolatoria rispetto al bersaglio acustico di un basso F2 era dedotto dai dati microbeam a raggi x. Pellet movimenti del dorso della lingua e labbra ha rivelato la tempistica dei movimenti labiali e linguali a essere fortemente collegati tra loro (sincronizzato), predittivo della posizione temporale della F2 pi&ugrave; basso all&#39;interno del nucleo vocalici e ridimensionato proporzionalmente alla durata complessiva vocale in partecipanti di controllo. Abbastanza sorprendentemente, essenzialmente le stesse conclusioni sono state ottenute negli altoparlanti con disartria. Queste relazioni erano pi&ugrave; rumorose tra gli altoparlanti con disartria, ma i modelli di sincronizzazione globale applicati a tutti i gruppi di 3. Ulteriori analisi hanno rivelato la sincronizzazione per essere meno ben definito e pi&ugrave; variabile attraverso gli altoparlanti con ALS, rispetto agli altoparlanti con PD e i controlli. Risultati sono discussi rispetto a concetti di coordinamento nella disartria.

This article addresses a long-standing clinical and theoretical debate regarding the potential relationship between speech and nonspeech behaviors in the developing system. The review is motivated by the high popularity of nonspeech oral motor exercises (NSOMEs), including alimentary behaviors such as chewing, in the treatment of speech disorders in young children. The similarities and differences in the behavioral characteristics, sensory requirements, and task goals for speech and nonspeech oromotor behaviors are compared. Integrated theoretical paradigms and empirical data on the development of early oromotor behaviors are discussed. Although the efficacy of NSOMEs remains empirically untested at this time, studies of typical developmental speech physiology fail to support a theoretical framework promoting the use of NSOMEs. Well-designed empirical studies are necessary, however, to establish the efficacy of NSOMEs for specific clinical population and treatment targets.

Este art&iacute;culo aborda un debate te&oacute;rico y cl&iacute;nico desde hace mucho tiempo con respecto a la posible relaci&oacute;n entre el discurso y nonspeech comportamientos en el desarrollo del sistema. La revisi&oacute;n est&aacute; motivada por la alta popularidad de nonspeech motor ejercicios orales (NSOMEs), incluyendo comportamientos alimentarios como la masticaci&oacute;n, en el tratamiento de trastornos del habla en ni&ntilde;os peque&ntilde;os. Se comparan las similitudes y diferencias en el comportamiento caracter&iacute;sticas, requisitos sensoriales y metas de tarea para el discurso y nonspeech del oromotor comportamientos. Paradigmas integrado te&oacute;ricos y datos emp&iacute;ricos sobre el desarrollo de los principios del oromotor comportamientos se discuten. Aunque en este momento a&uacute;n no comprobada emp&iacute;ricamente la eficacia de NSOMEs, estudios de Fisiolog&iacute;a de la t&iacute;pica expresi&oacute;n del desarrollo no apoyar un marco te&oacute;rico, promoviendo el uso de NSOMEs. Estudios emp&iacute;ricos bien dise&ntilde;ados son necesarios, sin embargo, para establecer la eficacia de NSOMEs para objetivos cl&iacute;nicos espec&iacute;ficos de la poblaci&oacute;n y tratamiento.

The goal of this study was to investigate the deterioration of lip and jaw movements during speech longitudinally in three individuals diagnosed with bulbar amyotrophic lateral sclerosis (ALS). The study was motivated by the need to understand the relationship between physiologic changes in speech movements and clinical measures of speech performance such as speaking rate and speech intelligibility. Movements of the lip and jaw were quantified with respect to their size speed, and duration. The data revealed several changes in lip and jaw movement that coincided with ALS progression. In two out of three speakers, the changes in measures of path distance and speed anticipated the drop in speech intelligibility by approximately 3 months. With disease progression, increases in movement duration coincided with declines in speech intelligibility. Overall, the movement measures appeared to be sensitive to disease progression in ALS. Learning outcomes: By the end of the manuscript, the reader should be able to: (1) describe the changes that occur in articulatory movements of the jaw and lower lip in ALS; (2) understand the relationship between physiologic measures of movement and speech intelligibility and speaking rate; (3) identify critical points in the disease progression and understand which quantitative measures reveal the state of the bulbar system at these time points.

El objetivo de este estudio fue investigar el deterioro de los labios y movimientos de la mand&#xED;bula durante el discurso longitudinalmente en tres personas diagnosticadas con esclerosis bulbar lateral amiotr&#xF3;fica (ELA). El estudio fue motivado por la necesidad de entender la relaci&#xF3;n entre los cambios fisiol&#xF3;gicos en los movimientos del habla y las medidas cl&#xED;nicas de rendimiento de la palabra tales como velocidad de la voz y la inteligibilidad del habla. Los movimientos del labio y la mand&#xED;bula se cuantificaron con respecto a su velocidad, tama&#xF1;o y duraci&#xF3;n. Los datos revelaron varios cambios en el labio y movimiento de la mand&#xED;bula, que coincidieron con la progresi&#xF3;n de la ELA. En dos de cada tres oradores, los cambios en las medidas de distancia de la trayectoria y la velocidad anticip&#xF3; la ca&#xED;da de la inteligibilidad de la palabra por aproximadamente 3 meses. Con la progresi&#xF3;n de la enfermedad, el aumento en la duraci&#xF3;n de movimiento coincidi&#xF3; con la disminuci&#xF3;n de la inteligibilidad del habla. En general, las medidas de movimiento que parec&#xED;a ser sensible a la progresi&#xF3;n de la enfermedad en la ALS. The purpose of this study was to assess the effect of speaking rate variation on aerodynamic and acoustic measures of velopharyngeal (VP) function. Twenty-seven healthy adult speakers (14 males, 13 females) participated in the study. The modified pressure-flow method was used to collect aerodynamic data of /m/ and /p/ segments in the word 'hamper' and the utterances 'Mama made some lemon jam' (MMJ) and 'Buy Bobby a puppy' (BBP). SPL was collected simultaneously with aerodynamic data for all utterances. A Nasometer was used to obtain nasalance scores and nasalance distance for MMJ and BBP. Sentences were produced at normal, fast, slow, and slowest speaking rates. The results showed that nasal airflow and VP orifice area were unaffected by speaking rate variations in males and females, whereas intra-oral pressure appeared to decrease as speaking rate slowed for both speaker groups. However, this effect was removed by statistically controlling SPL. Nasalance and nasalance distance (MMJ-BBP) did not change with speaking rate variation. There was a statistical difference between nasalance scores produced by male and female speakers. The results suggested that aerodynamic and acoustic measures of velopharyngeal function are not affected by variation in speaking rate in healthy males and females.
