1. 全血制备

在本研究中，比较了三种 PBMC 分离方案:密度梯度分离、手动和基于磁珠的自动分离。对制造商的手动基于微珠的方案进行了修改，包括在 PBMC 分离之前去除血沉棕黄层稀释，以及消除离心时中断的要求，使研究人员能够遵循适应性强、经济高效且高通量的 PBMC 分离方案。首先，使用 8 个匹配的全血样品分离 PBMC，以比较密度梯度分离和基于微珠的手动技术。重要的是，两种比较方法之间的细胞群分布、细胞活力和 PBMC 回收率没有显著差异，分别如图 2A-F、图 3A 和补充图 1 所示。在代表性数据中，使用台盼蓝排除法的密度梯度分离方法的细胞计数较高，但使用血液学细胞分析仪时则不然。细胞计数仪上的 PBMC 细胞类型设置采用 8-50 μm 的细胞直径范围，因此，当使用台盼蓝排除法时，计数将包括 PBMC 以及粒细胞（直径约 12-15 μM）17。虽然血液细胞计数仪的特异性高于台盼蓝排除法，但一些回收率计算结果高于 100%，反映了仪器的误差范围（参见补充图 1）。因此，建议研究人员在比较 PBMC 分离方案的产量时应用细胞计数技术的组合，因为大多数技术不提供特异性和高灵敏度的差异细胞计数。此外，没有进行分析以比较两种技术产生的 PBMC 的功能活性，这是我们分析的一个局限性。
接下来，比较了基于微珠的手动和自动方法，从 8 个匹配样品中未发现 PBMC 产量或活力之间的显着差异（图 4A、B）。由于两种方法使用相同的抗体分离混合物，因此未单独比较细胞群。重要的是，使用自动化方案，处理 8 个样品的手动操作时间从 43 分钟减少到 22 分钟（图 4C）。虽然使用自动化确保了通量、防止技术人员倦怠和样本处理的一致性，但试剂和耗材的成本明显高出 3-4 倍，比基于微珠的手动方法高出 3-4 倍。这是在修改制造商的方案以使用 1 mL 血沉棕黄层（来自 10 mL 的全血体积）而不是推荐的 2-5 mL 范围（来自 10 mL 和/或更大的全血体积）之后。如果预算有限，选择手动方法可以将处理时间缩短 ~25%，同时保持与密度分离方法相当的试剂和耗材成本。建议每位技术人员一次处理不超过 8 个样品，以确保在 5 分钟的孵育期间（步骤 3.7、3.11 和 3.14）内充分交错放置样品（~30 秒/样品）。

isolating human peripheral blood mononuclear cells from buffy coats via high throughput immunomagnetic bead separation

PBMC 分离的基于微珠的工作流程（全血和血浆等分）进行了 9 个月的试点，其中前 820 名参与者使用手动平台从未经筛选的人血中分离 1410 个 PBMC 样本，然后在接下来的 590 名参与者使用自动化方法。根据研究验收标准，处理在采集当天或采集后最多 10 天内进行，处理延迟主要是由于样品运输时间。使用手动或自动方法，在收集后 24 小时内处理的 PBMC 具有最高的活力和回收率（图 5）。5 天内加工的 PBMC 的平均细胞活力为 &gt;90%，10 天内加工的 PBMC 的平均细胞活力为 &gt;70%（图 5A）。与在 24 小时内处理的标本相比，5 天后平均 PBMC 产量下降了 50%（图 5B）。

In order to obtain cells suitable for clinical application, transgene-free iPSCs need to be generated to avoid transgene reactivation, altered gene expression and misguided differentiation. Moreover, a highly efficient and inexpensive reprogramming method is necessary to derive sufficient iPSCs for therapeutic purposes. Given this need, an efficient non-integrating episomal plasmid approach is the preferable choice for iPSC derivation. Currently the most common cell type used for reprogramming purposes are fibroblasts, the isolation of which requires tissue biopsy, an invasive surgical procedure for the patient. Therefore, human peripheral blood represents the most accessible and least invasive tissue for iPSC generation.
In this study, a cost-effective and viral-free protocol using non-integrating episomal plasmids is reported for the generation of iPSCs from human peripheral blood mononuclear cells (PBMNCs) obtained from frozen buffy coats after whole blood centrifugation and without density gradient separation.

Generation of Human Monocyte-derived Dendritic Cells from Whole Blood

Dendritic cells (DCs) recognize foreign structures of different pathogens, such as viruses, bacteria, and fungi, via a variety of pattern recognition receptors (PRRs) expressed on their cell surface and thereby activate and regulate immunity. 
The major function of DCs is the induction of adaptive immunity in the lymph nodes by presenting antigens via MHC I and MHC II molecules to na&#239;ve T lymphocytes. Therefore, DCs have to migrate from the periphery to the lymph nodes after the recognition of pathogens at the sites of infection. For in vitro experiments or DC vaccination strategies, monocyte-derived DCs are routinely used. These cells show similarities in physiology, morphology, and function to conventional myeloid dendritic cells. They are generated by interleukin 4 (IL-4) and granulocyte-macrophage colony-stimulating factor (GM-CSF) stimulation of monocytes isolated from healthy donors. Here, we demonstrate how monocytes are isolated and stimulated from anti-coagulated human blood after peripheral blood mononuclear cell (PBMC) enrichment by density gradient centrifugation. Human monocytes are differentiated into immature DCs and are ready for experimental procedures in a non-clinical setting after 5 days of incubation.

Isolating Human Peripheral Blood Mononuclear Cells and CD4+ T cells from S&#233;zary Syndrome Patients for Transcriptomic Profiling

Cutaneous T-cell lymphomas (CTCL) are derived from the transformation and uncontrolled proliferation of mature skin-homing T cells, and mycosis fungoides (MF) and S&#233;zary syndrome (SS) represent the most common subtypes. Despite a number of studies on characterizing gene expression, genetic alterations, and epigenetic abnormalities of CTCL, the molecular pathogenesis of MF/SS remains unclear. MF refers to the more common CTCL with a skin-predominance, and is usually limited to skin, whereas&#160;SS is an aggressive leukemic variant of CTCL with widespread skin involvement and is characterized by neoplastic distribution mainly involving blood, skin, and lymph node. Focusing on clinical practice, the identification of gene expression biomarkers has&#160;enormous potential to improve diagnosis and treatment of MF/SS. Indeed, recent transcriptomic studies have identified potential diagnostic biomarkers from differences in gene expression between normal and malignant T cells, which may improve our understanding of SS biology, and reveal potential therapeutic targets. This manuscript describes a detailed reproducible protocol for the isolation of peripheral blood mononuclear cells from fresh whole blood from patients diagnosed with SS, selection of CD4+ memory T cells (CD4+CD45RO+ T cells), chemical stimulation, and preparation of RNA suitable for transcriptomic profiling to discover novel prognostic molecular markers to gain additional insight in disease etiology. The stimulation using chemical agonist to activate nuclear regulation provides more specific assessment for pathways important in the dynamic transcription regulation and gene expression and eliminates confounding defects that may arise from upstream signaling defects arising from TCR antigen loss at the cell membrane. The data obtained from comparison of transcriptome of unstimulated to stimulated SS T cells unmasks functional regulatory gene expression defects not evident from analysis of quiescent unstimulated cells. Furthermore, the method outlined from this approach can be adapted for studying T cell gene expression defects in other T cell immune diseases.

Isolation of Human Neutrophils from Whole Blood and Buffy Coats

Neutrophils (PMNs) are the most abundant leukocytes in human circulation, ranging from 40 to 70% of total blood leukocytes. They are the first cells recruited at the site of inflammation via rapid extravasation through vessels. There, neutrophils perform an array of functions to kill invading pathogens and mediate immune signaling. Freshly purified neutrophils from human blood are the model of choice for study, as no cell line fully replicates PMN functions and biology. However, neutrophils are short-lived, terminally differentiated cells and are highly susceptible to activation in response to physical (temperature, centrifugation speed) and biological (endotoxin, chemo- and cytokines) stimuli. Therefore, it is crucial to follow a standardized, reliable, and fast method to obtain pure and non-activated cells. This protocol presents an updated protocol combining density gradient centrifugation, red blood cell (RBC) sedimentation, and RBC lysis to obtain high PMN purity and minimize cell activation. Furthermore, methods to assess neutrophil isolation quality, viability, and purity are also discussed.

Separation of Immune Cell Subpopulations in Peripheral Blood Samples from Children with Infectious Mononucleosis

Infectious mononucleosis (IM) is an acute syndrome mostly associated with primary Epstein&#8211;Barr virus (EBV) infection. The main clinical symptoms include irregular fever, lymphadenopathy, and significantly increased lymphocytes in peripheral blood. The pathogenic mechanism of IM is still unclear; there is no effective treatment method for it, with mainly symptomatic therapies being available. The main question in EBV immunobiology is why only a small subset of infected individuals shows severe clinical symptoms and even develop EBV-associated malignancies, whilemost individuals are asymptomatic for life with the virus.
B cells are first involved in IM because EBV receptors are presented on their surface. Natural killer (NK) cells are cytotoxic innate lymphocytes that are important for killing EBV-infected cells. The proportion of CD4+ T cells decreases while that of CD8+ T cells expands dramatically during acute EBV infection, and the persistence of CD8+ T cells is important for lifelong control of IM. Those immune cells play important roles in IM, and their functions need to be identified separately. For this purpose, monocytes are separated first from peripheral blood mononuclear cells (PBMCs) of IM individuals using CD14 microbeads, a column, and a magnetic separator.
The remaining PBMCs are stained with peridinin-chlorophyll-protein (PerCP)/Cyanine 5.5 anti-CD3, allophycocyanin (APC)/Cyanine 7 anti-CD4, phycoerythrin (PE) anti-CD8, fluorescein isothiocyanate (FITC) anti-CD19, APC anti-CD56, and APC anti-CD16 antibodies to sort CD4+ T cells, CD8+ T cells, B cells, and NK cells using a flow cytometer. Furthermore, transcriptome sequencing of five subpopulations was performed to explore their functions and pathogenic mechanisms in IM.
