Our research focuses on non-surgery therapy for pigmented disease. We tested the efficacy of roller microneedle combined with tranexamic acid solution in treating melasma. We will investigate the effectiveness and the maximization of using a combination of roller microneedles and the tranexamic acid solution for treating melasma.

To begin, prepare a quiet and well-lit treatment room and disinfect it with ultraviolet light for 30 minutes before the procedure. Instruct patients to wear shoe covers and hats before they enter the treatment room. If the treatment area is covered with makeup, remove makeup thoroughly with makeup remover.

Cleanse the treatment area with a gentle cleansing product for individuals not wearing makeup. Use the multispectral dermoscope to capture images of the patient's front, left, and right from three different angles before initiating treatments and after the third session. Document the findings, including the intensity of ultraviolet shallow, deep spots, and brown spots.

Then, administer 5%lidocaine cream over the treatment area evenly. Wrap the area with cling film to potentiate the numbing effect for a period of 40 to 60 minutes. Next, draw out 5 milliliters of 5%tranexamic acid solution with a syringe for later usage.

To prepare a 0.5 millimeter roller microneedle, submerge it in a 0.1%benzalkonium bromide solution for 20 minutes to disinfect it. Perform a triple rinse of the microneedle with physiological saline to eliminate any traces of the disinfectant. After an adequate period of anesthetic application, peel away the cling film.

Use a spatula to remove the anesthetic cream and then cleanse any remnants with gauze drenched in 0.9%saline solution. Make the treatment area accessible, securing the hair back with a disinfectant towel when treating the facial areas. Sterilize the facial skin with a cotton ball saturated in 0.1%benzalkonium bromide solution from the top of the face to the hairline, then to the lower jaw edge and chin.

To begin the procedure, the therapist should wash their hands, don a hat, mask, and sterile gloves to ensure proper hygiene. Instruct the patients to close their eyes. Use a syringe to administer the tranexamic acid solution onto the skin and distribute it evenly by hand.

Cleanse the roller microneedle with physiological saline and roll the microneedle in short distances in horizontal, vertical, and diagonal patterns across the treatment area. Treat different areas, integrating the application of the solution with the rolling action to facilitate absorption. Repeat the process three to five times while applying even pressure without dragging the microneedle, ensuring the skin flushes but does not bleed.

Use the multispectral dermoscope to assess treatment effectiveness before and after each session. Evaluate the efficacy of treatment by monitoring the reduction of UV shallow and deep spots, as well as brown spots, which indicate a lightning of melasma. Conduct statistical analysis to compare the changes in ultraviolet shallow and deep spots and brown spots before and after the treatment sessions.
