여기에 설명 된 프로토콜은 비엔나 의과 대학의 인간 연구 윤리위원회의 지침을 준수합니다.
1. 준비
<ol><li>프린터 또는 레코딩 소프트웨어에 연결된 디지털 X선 형광소시스템(도1A),모니터시스템(도1B),압력 레코더 및 트랜스듀서(도1C)및 초음파 장치(도1D)를갖춘 X선을 이용한 절차에 전념/승인된 전문실을 사용한다. 또한, 환자 벤치 옆에 멸균 작업영역(도 1E)을위한 충분한 공간이 있는지 확인하십시오.</li>
	<li>HVPG의 측정에 의해 CSPH에 대한 의심되는 향상된 만성 간 질환 (ACLD)을 가진 환자를 평가합니다. 특히, 다음 기능 중 하나를 가진 환자는 HVPG 측정을 겪을 수 있습니다: 혈전 세포성 페니아 & 150 G/L, 단면 이미징에 대한 포르토전신 담보, 비선택적 betablocker 치료, 선동, 및 절단 종양의 개시 전에 위장 식도 가변.</li>
	<li>절차에 대한 다음 위반을 사용하십시오: (i), 경정맥 혈전증으로 인한 혈관 접근의 부재, (ii) 임상적으로 명백한 출혈 장애(예를 들어, 보급된 인트라바스큘러응압), (iii) 반대측 폐렴구균, (iv) 중요한 심장 부정맥.</li>
	<li>환자가 절차의 목적과 잠재적 위험에 대해 통보 받은 후 환자가 고정되고 서면 으로 통보된 동의를 얻었습니다.</li>
	<li>절차는 전신 마취없이 수행되어야한다고 설명한다. 그러나, midazolam의 낮은 복용량 (최대 0.02 mg/kg 체중) 불안 한 환자에 게 제공 될 수 있습니다.</li>
	<li>표준 모니터 시스템을 사용하여 비침습적 동맥 혈압 측정, 맥박 산소 측정 및 전기 장술에 의해 환자의 활력 징후를 모니터링합니다.</li>

	<li>작업자가 손과 팔뚝을 씻고 소독하십시오.</li>

	<li>멸균 테이블 커버를 사용하고 중앙 정맥 접근에 필요한 장비에 대한 멸균 작업 영역을 준비(그림 2A).</li>
</ol>2. 멸균 조건 하에서 중앙 정맥 접근
<ol><li>환자에게 머리를 왼쪽으로 약간 돌린 다음 소독제로 오른쪽 전방 및 측면 목 부위를 소독하도록 지시합니다.</li>
	<li>경동맥 삼각형 주위에 피부를 덮는 멸균 수건을 놓습니다.</li>

	<li>블레이드 11 메스를 사용하여 가이드와이어를 따라 2-3mm 절개를 하여 확장기로 로드된 혈관 접근 칼집을 쉽게 도입할 수 있습니다.</li>
	<li>확장기와 함께 로드된 혈관 접근 경피 칼집을 가이드와이어 를 통해 내부 경정맥에 삽입합니다.</li>
	<li>혈관 액세스 칼집의 가이드와이어와 확장기를 제거합니다.</li>
	<li>혈관 접근 칼집이 안정적인 내트라바스 위치에 남아 있는지 확인하고 측면쪽으로 혈관 액세스 칼집의 주입 포트를 방향을 지정(그림 3B). 참고: 봉합 이나 테이핑이 필요하지 않습니다.</li>
</ol>3. 간 정맥에 풍선 카테터의 배치
<ol><li>풍선 카테터를 간 정맥에 배치하는 데 필요한 모든 재료를 준비합니다(그림2B).</li>
	<li>콘트라스트 미디어(염료)로 풍선 카테터를 플러시하고 식염수 용액에 담근 카테터 팁으로 반복적인 부동/디플레이션으로 풍선의 무결성을 확인합니다. 기포가 발생하지 않아야 합니다. 참고: X선에 카테터 자체의 가시성에 따라, 당신은 또한 카테터의 혈관 루멘을 플러시 하기 위해 단지 멸균 식염수 또는 희석 염료를 사용할 수 있습니다.</li>

	<li>열등한 카바 정맥(IVC)으로 불소 유도하에 풍선 카테터를 전진시키게 한다. 풍선 카테터의 끝을 환자의 뒤쪽으로 정렬하고 카테터의 약간의 회전은 일반적으로 오른쪽 아트리움에서 IVC로 진행할 수 있습니다. 환자에게 깊은 흡입을 지시하면 어떤 경우에는 도움이 될 수 있습니다(그림 3B).</li>
	<li>IVC에서 간 정맥으로 풍선 카테터를 진행합니다. 간 정맥과 IVC(도 3C)의접합의 의심 영역을 향해 오른쪽으로 향하는 풍선 카테터의 끝을 반복적으로 이동하여 간 정맥에 대한 액세스를 시도하십시오.</li>

	<li>팽창된 풍선에 정맥이 팽창될 때까지 풍선(풍선 루멘내 약 2mL의 공기)과 조영제 주입(혈관 루멘내 약 5mL)을 팽창시킴으로써 정맥(wedge position)의 적절한 폐색을 확인한다(도3D).</li>
	<li>콘트라스트 미디어의 정체를 관찰하고 풍선에 의한 정맥 루멘의 폐색이 불충분하거나 정맥 맥통신의 존재로 인해 대조 매체의 세척을 배제한다. 중요한 대조 미디어 세척이 관찰되면 풍선 카테터를 재배치하십시오.</li>
	<li>풍선을 수축하고 카테터의 루멘을 식염수로 플러시합니다.</li>
</ol>4. HVPG 평가를 위한 혈역학 판독값
<ol><li>풍선 카테터의 혈관 루멘을 주입 선을 사용하여 압력 변환기와 연결합니다.</li>
	<li>간 정맥의 개구부에서 IVC에 2-4cm 풍선의 끝으로 FHVP를 기록하기 시작합니다. 곡선의 파형은 시간이 지남에 따라 변형없이 안정되어야합니다. 참고: 안정적인 값은 일반적으로 15s 이후 가져옵니다.</li>
	<li>풍선을 팽창시키고 측정이 시간이 지남에 따라 변동없이 안정적인 수평 선이 될 때까지 WHVP를 계속 기록합니다. 참고: 쐐기 압력의 안정적인 추적은 일반적으로 >40 s 후에 얻어진다.</li>
	<li>FHVG(>15s)와 WHVP(>40s)의 측정을 세 번 이상 반복하여 고품질 판독값(그림3E)을획득합니다. 참고: 단일 FHVP/WHVP 판독값 간에 ≥2mm Hg의 상당한 불일치가 관찰되는 경우 추가 측정을 얻어야 합니다. 혈역학 적기록의 각 시점에서 기침, 이동 또는 대화와 같은 잠재적인 유물의 이유를 유의하십시오.</li>
	<li>IVC에서 올바른 심방 압력(RAP)뿐만 아니라 간 정맥의 타튬 수준에서 압력을 기록합니다.</li>

</ol>5. 경질 간 생검 준비
참고: 두 개의 다른 생검 방법론은 경정맥 간 생검을 얻기 위하여 이용될 수 있습니다: 포부 (단계 6) 또는 핵심 생검 (단계 7). 먼저 어떤 시스템을 사용할지 결정한 다음 적절한 생검 바늘 소개자 칼집을 선택한 다음 5.1 에서 5.3 단계를 진행합니다. 대안적으로, 코어 TJBLX 세트의 바늘은 또한 포부 세트 (NOTE : 적절한 직경사용)에 삽입 될 수 있으며, 이는 간 정맥에 도입하기 쉬울 수있는 보다 유연한 코어 생검 시스템을 초래합니다 (참고 : 이 방법은 여기에 자세히 설명되지 않습니다).

</ol>6. 경정맥 포부 간 생검
<ol><li>10mL 루어 락 주사기를 사용하여 포부를 멸균 식염수로 포부를 플러시하지만, 포부를 용이하게 하기 위해 주사기에 약 3mL을 둡니다.</li>
	<li>생검 바늘을 생검 바늘을 통해 부드럽게 전진하여 바늘의 끝이 소개자 칼집의 끝에 도달할 때까지 칼집을 소개합니다. 참고: 바늘을 전진시키면서 어떤 힘이나 빠른 움직임을 피하십시오. 환자에게 심호흡을 하도록 요청하면 생검 바늘의 쉽게 발전하기 위해 IVC와 간 정맥 사이의 각도가 감소합니다.</li>
	<li>환자에게 숨을 참아 달라고 부탁한다.</li>
	<li>10mL 루어 락 주사기를 사용하여 흡입을 적용하고 바늘을 간 parenchyma로 전진시다.</li>
	<li>흡입 (음압)을 적용하는 동안 바늘을 철회합니다.</li>
	<li>환자에게 정상적으로 호흡을 계속하도록 조언하십시오.</li>

간경변증 환자는 주로 위 또는 식도 가변1,2,3에서발진 또는 출혈과 같은 포털 고혈압(PHT)과 관련된 합병증을 일으킬 위험이 있다. 간 보상의 위험은 PHT2의정도와 관련이 있습니다. 간 정맥 간 압력 그라데이션(HVPG)의 측정은 간경변 환자에서 포털 정맥 압력을 추정하는 금본위제, 즉 부비동성 포털 고혈압4의중증도를 평가한다. HVPG≥10mm Hg는 CSPH를 정의하는 동안 ≥6mm Hg에서 9mm Hg의 HVPG는 높은 포털 압력('subclinical portal 고혈압')을 나타냅니다. 이 프로토콜은 장비 및 절차에 대한 자세한 설명을 제공하며 잠재적인 함정을 강조하고 문제 해결에 대한 조언을 제공합니다.

HVPG는 HVPG의 감소가 임상적으로 의미 있는 이점5로변환되기 때문에 간 섬유증/기능의 개선 및/또는 악화를 위한 확립된 대리 마커로 사용되며, 반면 HVPG 값이 증가된 variceal 출혈 위험6과관련이 있다. 비선택적 베타 차단제(NSBB) 또는 병인학 치료 하에 있는 환자에서 HVPG의 변화에 대한 관찰에 기초하여, HVPG의 감소는 임상적으로 관련있는것으로간주됩니다7,8.
현재까지 HVPG와 유사한 정확도로 포털 압력의 정도를 반영하는 대체 비침습적 매개 변수는 없습니다. HVPG가 실제로 포털 압력을 측정하는 '간접적' 방법이라 할지라도, 그것은 강하게 상관관계가 있어 간경변 환자에서 측정된 포털 압력을 정확하게 반영한다9. 중요한 것은, HVPG 측정은 간parenchyma10,11,12의평가량을 최대화하기 위하여 풍선 카테터를 사용하여 수행되어야 합니다. 혈소판 수와 같은 간단한 실험실 값은 CSPH의 가능성을 추정하는 데 도움이 될 수 있습니다. 그러나 혈소판 수 또는 혈소판 수를 포함하는 비침습적 점수는 예측값(16)을제한한다. 간경변 환자에서 비장17 또는 포르토전신담보(18)를 보여주는 이미징 양식은 CSPH의 존재를 시사하지만, 실제 포털 고혈압의 정도를 정량화하는 데 는 도움이 되지 않는다. 간(19) 및/또는 비장의 엘라스토그래피와 같은 새로운 비침습적 이미징도구는 CSPH의 존재를 판결하거나 배제하는 데 유용합니다. 그러나 사용 가능한 방법 중 어느 것도 포털압력(21)의동적 변화를 직접 측정할 수 없습니다.

HVPG 반응은 (재발성) variceal 출혈을 방지하는 NSBs의 효과에 대한 유일한 확립 된 대리입니다. 더욱이, HVPG의 측정은 소라페닙33, 34,심바스타틴35,36,타우린37,또는 엠리카산38과같이 벤치에서 침대 곁으로 번역되는 간경변 및/또는 포털 고혈압에 대한 새로운 치료법의 효과를 평가하는 개념 증명 연구를 위한 대리 종점역할을 할 수 있다. 궁극적으로, HVPG의 측정은 또한 HCC39의 발달을 위한 리스크에 관하여 중요한 예후 정보를 제공할 수 있고 간 부전 후 간 부전40에대한.
HVPG를 측정하는 인프라는 중등 및 고등 요양원에서 쉽게 사용할 수 있어야 합니다. HVPG 측정 기술은 특수 교육 및 장비가 필요하기 때문에 학술 및 이식 센터가 간 혈역학 실험실을 설립하여 포털 고혈압의 최첨단 진단 및 관리를 촉진하는 것이 합리적인 것으로 보입니다. 대용량 센터는 연간 수백 개의 HVPG 측정을 수행합니다. 우리의 경험을 바탕으로, 정확한 HVPG 측정을 수행하기위한 충분한 전문 지식은 일반적으로 50-100 감독 HVPG 측정 후 얻을 수 있습니다.

Epidural Intracranial Pressure Measurement in Rats Using a Fiber-optic Pressure Transducer

Elevated intracranial pressure (ICP) is a significant problem in several forms of ischemic brain injury including stroke, traumatic brain injury and cardiac arrest. This elevation may result in further neurological injury, in the form of transtentorial herniation1,2,3,4, midbrain compression, neurological deficit or increased cerebral infarct2,4. Current therapies are often inadequate to control elevated ICP in the clinical setting5,6,7 . Thus there is a need for accurate methods of ICP measurement in animal models to further our understanding of the basic mechanisms and to develop new treatments for elevated ICP.
In both the clinical and experimental setting ICP cannot be estimated without direct measurement. Several methods of ICP catheter insertion currently exist. Of these the intraventricular catheter has become the clinical 'gold standard' of ICP measurement in humans8. This method involves the partial removal of skull and the instrumentation of the catheter through brain tissue. Consequently, intraventricular catheters have an infection rate of 6-11%9. For this reason, subdural and epidural cannulations have become the preferred methods in animal models of ischemic injury. 
Various ICP measurement techniques have been adapted for animal models, and of these, fluid-filled telemetry catheters10 and solid state catheters are the most frequently used11,12,13,14,15. The fluid-filled systems are prone to developing air bubbles in the line, resulting in false ICP readings. Solid state probes avoid this problem (Figure 1). An additional problem is fitting catheters under the skull or into the ventricles without causing any brain injury that might alter the experimental outcomes. Therefore, we have developed a method that places an ICP catheter contiguous with the epidural space, but avoids the need to insert it between skull and brain. 
An optic fibre pressure catheter (420LP, SAMBA Sensors, Sweden) was used to measure ICP at the epidural location because the location of the pressure sensor (at the very tip of the catheter) was found to produce a high fidelity ICP signal in this model. There are other manufacturers of similar optic fibre technologies13 that may be used with our methodology. Alternative solid state catheters, which have the pressure sensor located at the side of the catheter tip, would not be appropriate for this model as the signal would be dampened by the presence of the monitoring screw. 
Here, we present a relatively simple and accurate method to measure ICP. This method can be used across a wide range of ICP related animal models.

A novel technique to record the pressures within the skull is described. The minimally invasive method uses a fibre-optic pressure sensing system to accurately measure intracranial pressure (ICP) in anaesthetized rats without causing significant brain trauma. The technique may be used in a wide range of experimental models.

Measurement of the Pressure-volume Curve in Mouse Lungs

In recent decades the mouse has become the primary animal model of a variety of lung diseases. In models of emphysema or fibrosis, the essential phenotypic changes are best assessed by measurement of the changes in lung elasticity. To best understand specific mechanisms underlying such pathologies in mice, it is essential to make functional measurements that can reflect the developing pathology. Although there are many ways to measure elasticity, the classical method is that of the total lung pressure-volume (PV) curve done over the whole range of lung volumes. This measurement has been made on adult lungs from nearly all mammalian species dating back almost 100 years, and such PV curves also played a major role in the discovery and understanding of the function of pulmonary surfactant in fetal lung development. Unfortunately, such total PV curves have not been widely reported in the mouse, despite the fact that they can provide useful information on the macroscopic effects of structural changes in the lung. Although partial PV curves measuring just the changes in lung volume are sometimes reported, without a measure of absolute volume, the nonlinear nature of the total PV curve makes these partial ones very difficult to interpret. In the present study, we describe a standardized way to measure the total PV curve. We have then tested the ability of these curves to detect changes in mouse lung structure in two common lung pathologies, emphysema and fibrosis. Results showed significant changes in several variables consistent with expected structural changes with these pathologies. This measurement of the lung PV curve in mice thus provides a straightforward means to monitor the progression of the pathophysiologic changes over time and the potential effect of therapeutic procedures.

Here we present a protocol to simply and reliably measure the lung pressure-volume curve in mice, showing that it is sufficiently sensitive to detect phenotypic parenchymal changes in two common lung pathologies, pulmonary fibrosis and emphysema. This metric provides a means to quantify the lung&#8217;s structural changes with developing pathology.

Isolated Hepatic Perfusion as a Treatment for Liver Metastases of Uveal Melanoma

Isolated hepatic perfusion (IHP) is a procedure where the liver is surgically isolated and perfused with a high concentration of the chemotherapeutic agent melphalan. Briefly, the procedure starts with the setup of a percutaneous veno-venous bypass from the femoral vein to the external jugular vein. Via a laparotomy, catheters are then inserted into the proper hepatic artery and the caval vein. The portal vein and the caval vein, both supra- and infrahepatically, are then clamped. The arterial and venous catheters are connected to a heart lung machine and the liver is perfused with melphalan (1 mg/kg body weight) for 60 min. This way it is possible to locally perfuse the liver with a high dose of a chemotherapeutic agent, without leakage to the systemic circulation.
In previous studies including patients with isolated liver metastases of uveal melanoma, an overall response rate of 33-100% and a median survival between 9 and 13 months, have been reported. The aim of this protocol is to give a clear description of how to perform the procedure and to discuss IHP as a treatment option for liver metastases of uveal melanoma.

Generation of a Humanized Mouse Liver Using Human Hepatic Stem Cells

A novel animal model involving chimeric mice with humanized livers established via human hepatocyte transplantation has been developed. These mice, in which the liver has been repopulated with functional human hepatocytes, could serve as a useful tool for investigating human hepatic cell biology, drug metabolism, and other preclinical applications. One of the key factors required for successful transplantation of human hepatocytes into mice is the elimination of the endogenous hepatocytes to prevent competition with the human cells and provide a suitable space and microenvironment for promoting human donor cell expansion and differentiation. To date, two major liver injury mouse models utilizing fumarylacetoacetate hydrolase (Fah) and uroplasminogen activator (uPA) mice have been established. However, Fah mice are used mainly with mature hepatocytes and the application of the uPA model is limited by decreased breeding. To overcome these limitations, Alb-toxin receptor mediated cell knockout (TRECK)/SCID mice were used for in vivo differentiation of immature human hepatocytes and humanized liver generation. Human hepatic stem cells (HpSCs) successfully repopulated the livers of Alb-TRECK/SCID mice that had developed lethal fulminant hepatic failure following diphtheria toxin (DT) treatment. This model of a humanized liver in Alb-TRECK/SCID mice will have functional applications in studies involving drug metabolism and drug-drug interactions and will promote other in vivo and in vitro studies.

Method of Direct Segmental Intra-hepatic Delivery Using a Rat Liver Hilar Clamp Model

Major hepatic surgery with inflow occlusion, and liver transplantation, necessitate a period of warm ischemia, and a period of reperfusion leading to ischemia/reperfusion (I/R) injury with myriad negative consequences. Potential I/R injury in marginal organs destined for liver transplantation contributes to the current donor shortage secondary to a decreased organ utilization rate. A significant need exists to explore hepatic I/R injury in order to mediate its impact on graft function in transplantation. Rat liver hilar clamp models are used to investigate the impact of different molecules on hepatic I/R injury. Depending on the model, these molecules have been delivered using inhalation, epidural infusion, intraperitoneal injection, intravenous administration or injection into the peripheral superior mesenteric vein. A rat liver hilar clamp model has been developed for use in studying the impact of pharmacologic molecules in ameliorating I/R injury. The described model for rat liver hilar clamp includes direct cannulation of the portal supply to the ischemic hepatic segment via a side branch of the portal vein, allowing for direct segmental hepatic delivery. Our approach is to induce ischemia in the left lateral and median lobes for 60 min, during which time the substance under study is infused. In this case, pegylated-superoxide dismutase (PEG-SOD), a free radical scavenger, is infused directly into the ischemic segment. This series of experiments demonstrates that infusion of PEG-SOD is protective against hepatic I/R injury. Advantages of this approach include direct injection of the molecule into the ischemic segment with consequent decrease in volume of distribution and reduction in systemic side effects.

Measurement of Natural Killer Cell-Mediated Cytotoxicity and Migration in the Context of Hepatic Tumor Cells

Natural killer (NK) cells are a subset of the cytotoxic lymphocyte population of the innate immune system and participate as a first line of defense by clearing pathogen-infected, malignant, and stressed cells. The ability of NK cells to eradicate cancer cells makes them an important tool in the fight against cancer. Several new immune-based therapies are under investigation for cancer treatment which rely either on enhancing NK cell activity or increasing the sensitivity of cancer cells to NK cell-mediated eradication. However, to effectively develop these therapeutic approaches, cost-effective in vitro assays to monitor NK cell-mediated cytotoxicity and migration are also needed. Here, we present two in vitro protocols that can reliably and reproducibly monitor the effect of NK-cell cytotoxicity on cancer cells (or other target cells). These protocols are non-radioactivity-based, simple to set up, and can be scaled up for high-throughput screening. We also present a flow cytometry-based protocol to quantitatively monitor NK cell migration, which can also be scaled up for high-throughput screening. Collectively, these three protocols can be used to monitor key aspects of NK cell activity that are necessary for the cells&#39; ability to eradicate dysfunctional target cells.

Understanding the mechanisms of liver injury, hepatic fibrosis, and cirrhosis that underlie chronic liver diseases (i.e., viral hepatitis, non-alcoholic fatty liver disease, metabolic liver disease, and liver cancer) requires experimental manipulation of animal models and in vitro cell cultures. Both techniques have limitations, such as the requirement of large numbers of animals for in vivo manipulation. However, in vitro cell cultures do not reproduce the structure and function of the multicellular hepatic environment. The use of precision-cut liver slices is a technique in which uniform slices of viable mouse liver are maintained in laboratory tissue culture for experimental manipulation. This technique occupies an experimental niche that exists between animal studies and in vitro cell culture methods. The presented protocol describes a straightforward and reliable method to isolate and culture precision-cut liver slices from mice. As an application of this technique, ex vivo liver slices are treated with bile acids to simulate cholestatic liver injury and ultimately assess the mechanisms of hepatic fibrogenesis.

This protocol provides a simple and reliable method for the production of viable precision-cut liver slices from mice. The ex vivo tissue samples can be maintained under laboratory tissue culture conditions for multiple days, providing a flexible model to examine liver pathobiology.

A Three-Dimensional Digital Model for Early Diagnosis of Hepatic Fibrosis Based on Magnetic Resonance Elastography

Hepatic fibrosis is an early stage of liver cirrhosis, and there are no better non-invasive and convenient methods for the detection and evaluation of the disease. Despite the good progress made with the liver stiffness map (LSM) based on magnetic resonance elastography (MRE), there are still some limitations that need to be overcome, including manual focus determination, manual selection of regions of interest (ROIs), and discontinuous LSM data without structural information, which makes it impossible to evaluate the liver as a whole. In this study, we propose a novel three-dimensional (3D) digital model for the early diagnosis of hepatic fibrosis based on MRE.
MRE is a non-invasive imaging technique that employs magnetic resonance imaging (MRI) to measure the liver stiffness at the scanning site through human-computer interaction. Studies have indicated a significant positive correlation between the LSM obtained through MRE and the degree of hepatic fibrosis. However, for clinical purposes, a comprehensive and precise quantification of the degree of hepatic fibrosis is necessary. To address this, the concept of Liver Stiffness Distribution (LSD) was proposed in this study, which refers to the 3D stiffness volume of each liver voxel obtained by the alignment of 3D liver tissue images and MRE indicators. This provides a more effective clinical tool for the diagnosis and treatment of hepatic fibrosis.
