O protocolo deste estudo foi aprovado pelo Comitê de Ética em Pesquisa em Seres Humanos do Hospital Longhua Afiliado à Universidade de Medicina Tradicional Chinesa de Xangai (Número de aprovação: 2023-LHXS-030). Antes do estudo, cada paciente incluído no estudo assinou o termo de consentimento livre e esclarecido. Um total de 76 pacientes com ombro congelado estágio I-II no departamento de reabilitação do Hospital Longhua afiliado à Universidade de Medicina Tradicional Chinesa de Xangai de dezembro de 2022 a maio de 2023, que preencheram os critérios de inclusão, foram divididos aleatoriamente em um grupo controle e um grupo de tratamento, com 38 casos em cada grupo. O operador deve estar familiarizado com a anatomia da articulação do ombro e com o método cirúrgico específico.
1. Recrutamento de pacientes
<ol><li>Utilizar os seguintes critérios de inclusão para incluir os pacientes neste estudo: Estar de acordo com os critérios diagnósticos de ombro congelado; ombro congelado estágio I ou II; independente do sexo, 40-65 anos; escore da EVA ≤ 7; sem outro tratamento ao mesmo tempo.</li>
	<li>Utilizar os seguintes critérios de exclusão para não incluir pacientes neste estudo: Pacientes com disfunção da coagulação sanguínea; pacientes diabéticos com controle glicêmico instável; gestantes e lactantes; pacientes com locais de tratamento danificados ou infecção grave; Pacientes com alergia à anestesia.</li>
</ol>2. Regime de exercícios para o grupo de controle
<ol><li>Submeter os pacientes do grupo controle a um curso de tratamento de exercícios funcionais conforme descrito a seguir19.<ol><li>Escalada manual na parede: Peça ao paciente que fique de frente para a parede com os dois pés abertos e levantando as duas mãos para subir ao longo da parede. Peça ao paciente que leve as mãos para as escalas mais altas com a máxima tolerância e registre isso para cada paciente.</li>
		<li>Mão assistida: Em uma posição ortostática, peça ao paciente para manter o membro superior afetado no estado de extensão para trás e realizar rotação interna para o lado oposto, puxando o punho do membro superior afetado com o membro superior saudável, e então mantenha o estado de tração para cima até a tolerância máxima para o paciente.</li>
		<li>Oscilações pendulares: Na posição em pé e inclinada, peça ao paciente que apoie o braço saudável de cada paciente e caia para o lado afetado e realize os movimentos de ida e volta, esquerda e direita, bem como os movimentos circulares. Aumente a amplitude de movimento quando os sintomas melhorarem e desenhe círculos segurando um objeto de 1 a 2 kg (como garrafas de água, halteres) para aumentar a intensidade dos exercícios de alongamento.</li>
	</ol></li>
	<li>Curso de exercício funcional: Garantir que os pacientes do grupo controle realizem exercícios funcionais todos os dias, incluindo escalada de parede, mão assistida e balanços pendulares, 30 vezes/dia durante 4 semanas. OBS: O exercício funcional deve ser realizado dentro da faixa de tolerância; caso contrário, pode causar tensão de tecidos moles no ombro e levar à deterioração da condição.</li>
	<li>Siga o princípio de - do leve ao pesado, do pequeno ao grande, passo a passo e continuamente - durante o exercício funcional. Peça ao paciente para evitar trabalho de ombro a longo prazo, trabalho pesado ou atividades extenuantes, embora o paciente possa realizar todas as atividades diárias durante o tratamento.</li>
</ol>3. Regime de exercícios para o grupo de tratamento
<ol><li>Submeter os pacientes do grupo de tratamento à terapia com faca de agulha pequena com exercícios funcionais.</li>
	<li>Preparo pré-operatório<ol><li>Preparo do material (Figura 1): Esterilizar copo plástico médico descartável, pinça médica, algodão absorvente, bloco de curativo, faca de agulha pequena estéril descartável de 40 mm x 40 mm, caneta marcadora, desinfetante cutâneo tipo II, cloridrato de lidocaína a 2%, água estéril para injeção, seringa estéril descartável de 10 mL, agulha de injeção estéril descartável, luvas cirúrgicas de borracha, curativo autoadesivo.</li>
		<li>Verificação de informações do paciente: Verifique as informações básicas de cada paciente (nome, idade, ombro afetado). Explique o processo e as vantagens da terapia com faca de agulha pequena. Certifique-se de que os pacientes compreendam e relaxem completamente durante o procedimento operacional.</li>
		<li>Preparo do paciente: Expor totalmente o ombro afetado na posição sentada.</li>
		<li>Posicionamento (Figura 2): Marcar os pontos de sensibilidade óbvios do ombro afetado, como o ponto do processo coracoide (ponto de fixação da cabeça curta do bíceps), tuberosidade menor ou maior (ponto de fixação do músculo subescapular), sulco intertubercular (ponto de fixação da cabeça longa do músculo bíceps), bursas subacromiais (ponto de fixação do músculo supraespinal), 2 cm atrás da tuberosidade maior do úmero (ponto de fixação do músculo redondo menor)20.</li>
		<li>Desinfecção: Preparar bolas de algodão em desinfetante de pele tipo II. Desinfetar rotineiramente a pele 3x com diâmetro gradualmente reduzido a partir de 15 cm em torno do ponto de marcação.</li>
	</ol></li>
	<li>Procedimento operacional (Figura 2)<ol><li>Carregar 5 mL de lidocaína a 2% mais 5 mL de água estéril em uma seringa de 10 mL para preparar uma solução de trabalho de 10 mL de lidocaína a 1%.</li>
		<li>Substitua a agulha 22G por uma agulha 25G (use agulhas mais finas para aliviar a dor). Use as luvas estéreis na mão esquerda e segure a seringa na mão direita.</li>
		<li>Insira a agulha na marca e certifique-se de que nenhum sangue retorna após a aspiração. Injetar perpendicularmente 1-2 mL de solução de trabalho de lidocaína a 1% em cada ponto marcado.</li>
		<li>Inserção de faca de agulha pequena (Figura 2): Use luvas estéreis na mão direita para segurar a faca de agulha pequena. Insira rapidamente a pequena faca de agulha no local de injeção de lidocaína perpendicularmente com uma profundidade de cerca de 1-3 cm. Certifique-se de que a lâmina da faca de agulha pequena é paralela às fibras musculares para evitar danos a vasos sanguíneos e nervos importantes.</li>
		<li>Realizar a manipulação em 3 passos da incisão longitudinal, desnudando e liberando a aderência 7,15. O paciente sentirá dor e dor locais, e o operador sentirá resistência sob a faca.</li>
		<li>Retire a agulha quando o operador sentir uma sensação de frouxidão sob a faca.</li>
	</ol></li>
	<li>Cuidados pós-operatórios (Figura 2)<ol><li>Pressione o local de injeção por 1 min com uma bola de algodão estéril seca. Cubra a área cirúrgica com um curativo autoadesivo estéril.</li>
		<li>Auxiliar os pacientes a se vestirem bem. OBS: Coloque os resíduos hospitalares na lixeira de lixo médico.</li>
		<li>Peça ao paciente que fique parado por cerca de 15 minutos antes de sair da sala de cirurgia para evitar desconforto pós-operatório. Orientar o paciente a manter o curativo autoadesivo seco até que seja retirado 48 h depois para evitar sangramento e infecção.</li>
	</ol></li>
	<li>Curso de terapia com faca de agulha pequena21: Submeter os pacientes do grupo de tratamento à terapia com faca de agulha pequena, uma vez por semana, com um total de 4 sessões. Realizar os mesmos exercícios funcionais para os pacientes do grupo de tratamento que foram realizados para o grupo controle.</li>
</ol>4. Avaliação dos resultados pós-intervenção
<ol><li>Índice de avaliação 22,23,24 <ol><li>Utilizar os escores visual analógico (EAV), os escores de Constant e Murley (CMS) e a espessura da CHL sob ultrassonografia para avaliar o desfecho. OBS: As medidas basais foram realizadas antes e uma semana após o tratamento.</li>
	</ol></li>
	<li>Instrumentos de avaliação<ol><li>Use o GJJDC01 instrumento eletrônico de medição de movimento articular de alta precisão para medição do movimento articular do ombro e um sistema portátil de ultrassom colorido (Figura 3).</li>
	</ol></li>

			<li>Peça ao paciente que exponha totalmente o ombro afetado na posição sentada. Mova passivamente a articulação do ombro do paciente para localizar a CHL. Alinhe a sonda de ultrassom com a CHL.</li>
			<li>Pressione o botão Congelar para salvar a imagem do ultrassom. Pressione o botão Paquímetro para testar a espessura CHL. Registre a medição e desligue-a.</li>

</ol>5. Análise estatística
<ol><li>Apresentar as variáveis contínuas como média ± desvio padrão (DP). Use o teste t para amostras independentes para comparar as diferenças entre os dois grupos. Todos os testes estatísticos foram bicaudais, sendo o P<0,05 considerado estatisticamente significante. Todas as análises foram realizadas por meio de software estatístico.</li>

O ombro congelado é uma doença caracterizada por dor e limitação dos movimentos ativos e passivos do ombro, o que causa grandes transtornos ao trabalho e à vida dospacientes1. A incidência global de ombro congelado é de 2%-5%2,3 e maior no sexo feminino do que no masculino, mostrando certa tendênciagenética4. A maioria dos pacientes com ombro congelado tem entre 40 e 60 anos5. Além disso, o ombro congelado acarreta uma pesada carga social devido ao ciclo de recuperação mais longo e à ausência de consenso de tratamento estabelecido na prática clínica.
Os principais métodos de tratamento para ombros congelados incluem uso oral ou externo de fitoterápicos, acupuntura, massagem, exercício funcional, e assim por diante6; no entanto, a adesão dos pacientes é pobre devido à multifreqüência de tratamento e maior demora, não podendo suprir as necessidades urgentes dos pacientes para melhorar a dor e as limitações funcionais. Portanto, é urgentemente necessário identificar terapêuticas mais eficazes no manejo clínico de pacientes com ombro congelado.
A terapia com facas de agulha pequena é um tratamento externo especial e eficaz da medicina tradicional chinesa, que surgiu rapidamente no campo da medicina tradicional chinesa nas últimas décadas7. Tem as vantagens de efeito rápido, efeito curativo definitivo e alta repetibilidade. Além disso, o instrumento de operação é barato e fácil de obter, o processo de operação é simples e o método não é limitado pelo ambiente e condição. O trabalho e a vida normal dos pacientes não são afetados após a terapia com faca de agulha pequena, uma vez que a micro-incisão será recuperada rapidamente, sem danos ao organismo, causando infecção ou outros efeitos colaterais 8,9,10,11. Revisões sistemáticas mostraram que a eficácia do bisturi com agulha pequena no ombro congelado foi significativamente superior à acupuntura, massagem e medicina oral chinesa12,13. Estudos modernos mostram que o mecanismo do tratamento com faca de agulha pequena do ombro congelado é dragar os meridianos e colaterais, promover a circulação de qi e sangue, cortar e retirar o tecido da lesão, remover aderências, promover o reparo tecidual e restaurar o equilíbrio dinâmico do corpo humano14,15.
O protocolo aqui apresentado demonstra o funcionamento da faca de agulha pequena no tratamento de ombros congelados, incluindo manejo do paciente, preparação do material, posicionamento, operação e cuidados pós-operatórios. O objetivo deste protocolo é aliviar a dor e as limitações funcionais e melhorar a capacidade de vida de pacientes com ombros congelados.
Com o desenvolvimento da terapia com facas de agulha pequena, sua aplicação tem sido estendida desde o tratamento de dores no pescoço, ombro, cintura, pernas e outras doenças musculares e ósseas até a medicina interna, cirurgia, ginecologia, pediatria e outras doenças, desempenhando um papel cada vez mais importante no campo da medicina de reabilitação16. Nos últimos anos, o bisturi de agulha pequena apresenta eficácia terapêutica mais precisa, científica e segura devido à aplicação da terapia de visualização com agulha pequena guiada por ultrassom, cujo desenvolvimento entrou em nova etapa17,18.

O ombro congelado é um tipo de doença inflamatória asséptica do ombro causada por tensão, trauma e outros motivos, resultando em dor na articulação do ombro e função limitada. O protocolo aqui apresentado demonstra o funcionamento de uma pequena faca de agulha no tratamento de ombros congelados, incluindo o manejo do paciente, preparação do material, posicionamento, operação e cuidados pós-operatórios. O objetivo deste protocolo é aliviar a dor e as limitações funcionais e melhorar a capacidade de vida de pacientes com ombros congelados. Em nosso estudo, 76 pacientes de ombro congelado estádio I-II que preencheram os critérios de inclusão foram divididos aleatoriamente em grupo controle e grupo tratamento (n=38). Os pacientes do grupo controle receberam exercício funcional, enquanto o grupo tratamento recebeu terapia com faca de agulha pequena com exercício funcional. Foram avaliados os escores visual analógico (EAV), de Constant e Murley (CMS) e a espessura do ligamento coracoumeral (LCU) à ultrassonografia. Após a terapia com agulha pequena, o escore EVA foi significativamente menor no grupo de tratamento (5,11 ± 0,89) do que no grupo controle (5,49 ± 0,65; t=-2,065, p<0,05); o escore CMS foi significativamente maior no grupo tratamento (64,72 ± 4,78) do que no grupo controle (60,97 ± 6,00; t=2,947, p<0,05); a espessura da CHL foi significativamente menor no grupo de tratamento (2,38 ± 0,36) do que no grupo controle (2,57 ± 0,42; t=-2,117, p<0,05). Esses resultados indicam que o bisturi de agulha pequena aliviou significativamente os sintomas de dor, melhorou a função do ombro, reduziu a espessura da CHL e melhorou a qualidade de vida e, portanto, teve eficácia terapêutica significativa em pacientes com ombro congelado estágio I-II.

Frozen shoulder is a disease characterized by pain and limitation of active and passive shoulder movement, which causes great inconvenience to the work and life of patients1. The global incidence of frozen shoulder is 2%-5%2,3 and higher in females than in males, showing a certain genetic tendency4. The majority of frozen shoulder patients are 40-60 years old5. Moreover, the frozen shoulder brings about a heavy social burden due to the longer recovery cycle and the absence of established treatment consensus in the clinic practice.
The main treatment methods for frozen shoulders include oral or external use of herbal medicine, acupuncture, massage, functional exercise, and so on6; however, the compliance of the patients is poor due to the multiple treatment frequency and longer time-consuming, and cannot meet the urgent necessities of patients to improve pain and functional limitations. Therefore, it is urgently needed to identify more effective therapeutics in the clinical management of frozen shoulder patients.
Small needle knife therapy is a special and effective external treatment of traditional Chinese medicine, which has arisen rapidly in the field of traditional Chinese medicine in recent decades7. It has the advantages of quick effect, definite curative effect, and high repeatability. Moreover, the operation instrument is cheap and easy to obtain, the operation process is simple, and the method is not limited by the environment and condition. The normal work and life of patients are not affected after the small needle knife therapy since the micro-incision will be recovered quickly without damage to the body, causing infection, or other side effects8,9,10,11. Systematic reviews showed that the efficacy of small needle knife on frozen shoulder was significantly superior to acupuncture, massage, and oral Chinese medicine12,13. Modern studies show that the mechanism of small needle knife treatment of frozen shoulder is to dredge the meridians and collaterals, promote the circulation of qi and blood, cut and strip the lesion tissue, remove adhesion, promote tissue repair, and restore the dynamic balance of the human body14,15.
The protocol presented here demonstrates the operation of small needle knife in treating frozen shoulders including patient management, material preparation, positioning, operation, and postoperative care. The purpose of this protocol is to relieve the pain and functional limitations and improve the living ability of patients with frozen shoulders.
With the development of small needle knife therapy, its application has been extended from the treatment of neck, shoulder, waist, leg pain, and other muscle and bone diseases to internal medicine, surgery, gynecology, pediatrics, and other diseases, playing an increasingly important role in the field of rehabilitation medicine16. In recent years, the small needle knife shows a more accurate, scientific, and safe therapeutic efficacy due to the application of ultrasound-guided small needle knife visualization therapy, whose development has entered a new stage17,18.

Our protocol has the advantages of quicker and definite therapeutic effects, high repeatability, broader availability, also there are less physical injury in patients.

No grupo controle, 1 paciente recebeu outro tratamento (injeção intra-articular de esteroide) devido ao agravamento da dor, sendo o estudo encerrado, e os 37 pacientes restantes do grupo controle foram incluídos na análise estatística final. Frozen shoulder is a kind of aseptic inflammatory disease of the shoulder caused by strain, trauma, and other reasons, resulting in shoulder joint pain and limited function. The protocol presented here demonstrates the operation of a small needle knife in treating frozen shoulders, including patient management, material preparation, positioning, operation, and postoperative care. The purpose of this protocol is to relieve the pain and functional limitations and improve the living ability of patients with frozen shoulders. In our study, 76 stage I-II frozen shoulder patients who met the inclusion criteria were randomly divided into a control group and a treatment group (n=38). Patients in the control group received functional exercise, while the treatment group received small needle knife therapy with functional exercise. The visual analogue scores (VAS), the Constant and Murley scores (CMS), and the thickness of the coracohumeral ligament (CHL) under ultrasound were evaluated. After small needle knife therapy, the VAS score was significantly lower in the treatment group (5.11 &plusmn; 0.89) than in the control group (5.49 &plusmn; 0.65; t=-2.065, p&lt;0.05); the CMS score was significantly higher in the treatment group (64.72 &plusmn; 4.78) than in the control group (60.97 &plusmn; 6.00; t=2.947, p&lt;0.05); the CHL thickness was significantly decreased in the treatment group (2.38 &plusmn; 0.36) than in the control group (2.57 &plusmn; 0.42; t=-2.117, p&lt;0.05). These results indicate that the small needle knife significantly relieved the pain symptoms, improved the shoulder function, reduced the CHL thickness, and improved the quality of life and, therefore, had significant therapeutic efficacy in stage I-II frozen shoulder patients.

The protocol presented here describes the detailed operation method and proves the effectiveness of a small needle knife in the treatment of frozen shoulders.

Performing the Small Needle Knife Therapy on Patients with Frozen Shoulder

Begin the preoperative preparations by gathering all the necessary materials. Then proceed to check the basic information of each patient, including name, age, and the affected shoulder. Once the patient assumes a sitting position, fully expose the affected shoulder and mark the obvious tenderness points on the shoulder.Then, pour type two skin disinfectant on cotton balls, and with those, disinfect the affected area in circular motions, starting from about 15 centimeters around each marked point, and gradually reducing the diameter. Next, load five milliliters of 2%lidocaine into a 10 milliliter syringe with a 22 gauge needle. Then, load five milliliters of sterile water into the same syringe to prepare 10 milliliters of 1%lidocaine working solution.Replace the 22 gauge needle with a 25 gauge needle. While wearing sterile gloves on the left hand, hold the syringe in the right hand, insert the needle into a marked point, and ensure no blood enters the syringe after suction. Perpendicularly, inject one to two milliliters of 1%lidocaine working solution at the marked point.To perform the small needle knife insertion, hold the small needle in the right hand while wearing sterile gloves. Rapidly insert the small needle knife into the lidocaine injection site perpendicularly, maintaining a depth of about one to three centimeters. Then, perform the three step manipulation, beginning with the longitudinal incision, followed by stripping, and finally releasing the adhesion.Withdraw the needle when there is a feeling of looseness under the knife. Using a dry, sterile dressing block, press the injection site for one minute. Then cover the operating area with a sterile self-adhesive dressing.After assisting the patient in dressing properly, ask the patient to sit still for about 15 minutes before leaving the operating room to avoid postoperative discomfort. After the small needle knife therapy, the visual analog score was significantly lower in the treatment group than in the control group. The Constant and Murley score was significantly higher in the treatment group than in the control group.

Assessing the Post-Intervention Outcome of Small Needle Knife Therapy on Frozen Shoulder

To begin assessing the range of motion of the shoulder of the patients who received the small needle therapy intervention, first, ask the patient to lift the shoulder to the maximum angle possible. Then press the On button to switch on the assessment instrument. Rotate the measuring ruler and obtain the measured value.Set the measuring instrument scale back to zero before repeating the measurement two times. Press and hold Off, to turn the power off. To measure the thickness of the coracohumeral ligament, first turn on the ultrasound machine by pressing the power button.Then open the ultrasound software. Press the patient button and enter the name of the subject. Ask the patient to fully expose the affected shoulder in a seated position.Move the patient's shoulder joint passively, to locate the coracohumeral ligament. After applying an appropriate amount of medical coupler to the probe, align the ultrasound probe with the coracohumeral ligament. Press the Freeze button to freeze the screen, and then press the Store button to save the ultrasound image.Continue to record two different images as demonstrated. After pressing the patient button, look for the recorded images in the study list. Press the Caliper button to enter the measurement mode and assess the coracohumeral ligament thickness.Finally, after recording the measurements, turn off the machine. After the treatment, the coracohumeral ligament thickness was significantly decreased in the treatment group, compared to the control group.
